Gastro-enterology and perineology...

in perfect harmony

I s t i l l remember these words from one o f your compatriots : ~ There is no diffe- rence between the human mind and a parachute.., to function both must remain well open... ~. Acta Endoscopica demonstrated much initiative when they asked a gastro- enterologist f r o m Quebec (jftst back f r o m a one-year sabbatical in Perineology in Europe) to prepare a special issue on Perineology in a Journal o f Digestive Endo- scopy.

Clinical gastro-enterologists regularly treat problems o f constipation, fecal incon- tinence, dyschesia, anismus, pelvi-perineal pain. Rarely do they treat associated uri- nary, gynecological or sexual pathologies since they are already submerged by the complexity o f some o f these cases. Perineology is a new specialty which proposes an integrated and global approach to the pelvic floor. It is multidisciplinary and asso- ciates specialists in gastro-enterology, urology, colo-proctology, surgery, obstetrics- gynecology, pediatrics, geriatrics, sexology, radiology, neurology and neurophysio- logy, specialists in functional exploration, psychology~psychiatry and finally, kinesitherapists specializing in pelvi-perineal re-education, all o f whom share the same unifying vision, the same integrated approach to pelvi-perineal pathologies.

Peers f r o m various countries have come together f o r this special issue on Perineology to discuss its concept (D r Richard Villet) and to review various basic anatomical principles (pr Maurice Laude). The pediatric perineum is also over- viewed (D r Raymond Reding) since sequellae o f its pathologies are encountered in our adult patients. Functional exploration o f the pelvic floor will be reviewed by Dr Jean-Pierre Galaup who will examine with us certain elements o f urological phy- siology while describing innovative techniques in the clinical evaluation o f perineal function now available to gastro-enterologists.

D r Laurent Siproudhis will update our notions on dyschesia.

Since pregnancy and childbirth are long and short term triggering factors f o r many pelvioperineal pathologies, we thought it necessary to review details o f the childbirth pelvic floor association (D r Annie Beaudoin).

Perineology is born... For the gastro-enterologist, it represents a new avenue o f intervention where he will play a primordial role. We will most certainly have to adjust in major ways but the effort is well worth it.
