Les gastro-ent6rologues doivent-ils s'occuper de la chimioth6rapie des patients atteints de cancer du tractus

L e s cancers du tractus digestif ont une importance clinique consid6- rable pour les gastro-ent6rologues du fait de leur pr6valence 61ev6e et du pronostic souvent dEfavorable des conditions concern6es [1-3].
Ce sont les gastro-entErologues qui fournisseut aux patients la majorit6 des soins et sont les plus ~ m~me d'6valuer le patient dans son ensemble.
A pr6sent, l'int6r~t se porte principalement sur les mesures de pr6ven- tion et de diagnostic ainsi que sur le traitement endoscopique.
Le r61e du gastro-entErologue dans le traitement pharmacologique est encore discutE.
En 1999 h la Semaine Europ6enne des gastro-ent6rologie (SEGE) Rome, ce sujet a 6t6 soulev6 et discut6 sur base des donnEes collectEes grfice ~ un questionnaire ~ choix multiple adress6 ~ 200 gastro- entEro- logues europ6ens et aux donn6es des 1328 formulaires d'enregistrement la SEGE.
Selon le questionnaire, la chimioth6rapie des patients porteurs de can- cer du tractus GI est directement prescrite par 30 % des gastro-ent6ro- logues des pays de la Communaut6 europ6enne, par 20,5 % des gastro- ent6rologues des pays europ6ens qui ne font pas partie de la Communaut6 europEenne et par 46,1% des gastro-ent6rologues asiatiques (Tableau I).
En comparant les diffErents pays, on observe de grandes variations dans les rEponses ~ la question : ~ Prescrivez-vous directement la chimioth6rapie votre patient porteur d'un cancer du tractus GI ? >~. Les rEsultats ont donn6 92 % de r6ponses positives en GrEce, 65 % en Allemagne, 51% en France et 45 % en Belgique et 97 % de r6ponses negatives au Royaume-Uni.
Le faible pourcentage (3 %) en Am6rique du Nord est surprenant mais il faut tenir compte de la petite taille de l'6chantillon, ce qui est aussi valable pour les autres pays.
I1 est intEressant de noter la corr61ation positive entre le pourcentage de centres qui ont un gastro-entErologue spEcialis6 en oncologie ou qui se consacre sp6cifiquement aux probl~mes des cancers du tractus digestif et le taux de gastro-entErologues directement impliqu6s dans le traitement thErapeutique de ce type de cancer.
En Italie, un grand nombre de gastro-entErologues (88,5 %) sont impli- qu6s mais peu (38 %) sont spEcialisEs dans les problbmes lies aux cancers du tractus digestif, alors qu'en France on observe une situation inverse (respectivement 34,5 % et 68 %) (Tableau II).
I1 est clair que la situation influence d'autres points importants tels que l'unit6 ofa la chimioth6rapie est effectuEe et le sp6cialiste responsable du suivi du patient.
Si l'oncologue est directement impliqu6 en tant que personne unique responsable du protocole de traitement (et non en tant que consultant dans la d6cision finale discut6e avec le gastro-ent6rologue), la chimiothE- rapie a tendance ~ ~tre effectuEe dans l'unit6 d'oncologie et le suivi est

effectu6 par de nombreux sp6cialistes (Tableau III) ne laissant qu'un tr6s petit r61e au gastro-ent6rologue (10,4 %).
L'analyse des r6sultats montre clairement qu'~
l'heure actuelle, les gastro-ent6rologues n'ont qu'un petit r61e dans le traitement pharmacologique de leurs patients atteints de cancer, m~me si 45,5 % d'entre eux participent h des projets de d6pistage du cancer et 69,5 % s'impliquent activement dans le trai- tement adjuvant (alimentation parent6rale ou ent6-
rale, suppression de la douleur, soins h domicile) des patients porteurs de cancer du tractus digestif.
En conclusion, les gastro-ent6rologues devraient avoir des formations plus sp6cifiques en chimioth6ra- pie et dans l'6valuation et le suivi des patients. Le m6decin oncologue peut bien agir en tant que consul- tant, plus particuli6rement lots de traitement tr6s complexe, mais le gastro-ent6rologue devrait ~tre responsable de la globalit6 du traitement des patients atteints de cancer du tractus gastro-intestinal.

C a n c e r s o f the digestive tract are an area o f considerable clinical concern for gastroenterologists, due to the high prevalence o f this pathology and often poor prognosis o f the conditions concerned [1-3].
Gastroenterologists provide most of the care to these patients and there- fore are best able to assess the patient comprehensively.
Up to present, interest has focused mainly on preventive and diagnostic procedures, as well as on endoscopic treatment.
The role o f the gastroenterologist in the area o f pharmacological treat- ment is still a matter of debate.
A t the 1999 United European Gastroenterology Week ( U E G W ) in Rome, this issue was raised and discussed on the basis o f data collected using a multiple choice questionnaire completed by 200 European gas- troenterologists and using data from 1328 U E G W registration forms.
According to the questionnaire, chemotherapy for GI cancer patients is directly prescribed by 30 % o f gastroenterologists in the European Union, by 20.5 % o f gastroenterologists in non-EU European countries and by 46.1% of Asian gastroenterologists (Table I).
Wide variations are also seen when different countries are compared - - with positive answers to the question "Do you directly prescribe chemothe- rapy to your GI cancer patients ?" ranging from 92 % in Greece, 65 % in Germany, 5 1 % in France and 45 % in Belgium, and with negative answers reaching 97 % in United Kingdom.
The low percentage (3 %) in North America is stricking, but the small size of the sample should be taken into account-- as in the other countries as well.
An interesting observation is the positive correlation between the percen- tage of centers which have a gastroenterologist specialized in oncology, or with a gastroenterologist specifically dedicated to GI cancer problems, and the rate o f direct involvement by gastroenterologists in the therapeutic management of GI cancer.
In Italy there is a high rate o f involvement by the oncologist (88.5 %), with a low percentage o f gastroenterologists specifically dedicated to GI cancer (38 %), while in France the converse is true (34.5 % and 68 %, res- pectively) (Table II).
Clearly the situation also influences other important aspects, such as the unit in which chemotherapy is performed and the specialist responsible for following the patient up. When the oncologist is directly involved as the per- son solely responsible for the treatment protocol (and not as a consultant sharing in the final decision along with the gastroenterologist), chemothe- rapy tends to be carried out within an oncology unit, with a wide variety of specialists being involved in the follow-up (Table III) and only minor invol- vement by gastroenterologists (10.4 %).

o f their cancer patients, even though 45.5 % o f them have been involved in chemotherapy research projects, 64 % o f them have been involved in can- cer screening projects, and 69.5 % are actively involved in support treatment (parenteral or ente- ral nutrition, pain relief, home care) for GI cancer patients.
In conclusion, we are convinced that gastroente- rologists are in a position to play a more direct part
in the comprehensive treatment o f GI cancer patients.
To achieve this, they would need more specific trai- ning in chemotherapy and in the evaluation and fol- low-up of the patient. The medical oncologist may well act as a consultant, especially for the more complex therapeutic regimens, but gastroenterologists should keep the global care of gastrointestinal cancer patients firmly in their own hands.
