Cette annre encore, cette XII e rencontre strasbourgeoise de la S M I E R , m6rite bien son n o m de Cours intensif, tant le programme est dense et de qualit6.

Tous les aspects de la sphinct6rotomie, allant de l'anatomie de la papille aux complications seront abordrs.

Le sujet de l'Helicobacterpylori, ddsormais d e v e n u un classique de tous les congrrs de gastroent6rologie, donnera lieu ~ un ddbat, aussi bien sur l'histologie que le traitement. Les problrmes posrs par le l y m p h o m e de M A L T seront abordrs.

Les MICI et l'oesophage de Barett d o n n e r o n t lieu & u n e nouvelle mise au point.

D e p u i s longtemps, nous avons pens6 que l ' e n d o s c o p i e 6 t a i t une porte permettant un accrs ~t d'autres t e c h n o l o g i e s plus sophistiqudes et cette ann6e, seront envisagrs les problrmes de la biologie et du futur de l'onco- logie.

Enfin, sujets peut-~tre plus marginaux dans notre discipline, l'aspect e n d o s c o p i q u e des helminthiases et la proctologie feront partie de notre programme.

D a n s la signification du mot S M I E R figure l'imagerie (<< I >,) et nous ne p o u v i o n s pas faire abstraction d'une pr6sentation de cas cliniques vidro.

Aprrs avoir c o n n u durant 5 matinres complrtes, au drbut de notre cours la transmission en live, depuis un certain n o m b r e d'ann6es driP, aprrs mfire r6flexion et un long d6bat, nous avons ddcid6 de l'abandonner pour privilrgier des m o n t a g e s vidros p r d a g o g i q u e s aussi drmonstratifs sinon plus de la technique envisagde et ne comportant aucun des alras 6thiques ou 6conomiques lirs ~ la vid6o transmission.

Aprbs le << I ,, vient le grand << E >, d ' e n s e i g n e m e n t , off n o u s pourrons voir ce que les guide lines, les confrrences de consensus, le web et enfin, l'utilisation des s i m u l a t e u r s p e u v e n t apporter h la c o n n a i s s a n c e des b o n n e s pratiques en endoscopie.

Enfin, apr~s le <~ E >>vient le << R >, de la S M I E R qui signifie recherche. Le futur de l'endoscopie sera vu sous l'angle de son aspect vrtdrinaire et les technologies de p o i n t e c o m m e la biopsie virtuelle et les phototh6rapies, seront analys6es dans tous leurs drtails.

N o u s ne saurions conclure le panorama de ce programme sans insister u n e fois de plus sur l'6valuation faite rrgulirrement m a i n t e n a n t depuis 12 ans et ce, bien avant l'institution d'une F M C obligatoire et encore mal- gr6 l'absence de rbgles bien ddfinies tant les pouvoirs publics font preuve de d6sinvolture et de n6gligence h notre 6gard.

E u r o p d e n , ce cours l'a t o u j o u r s 6t6 et l'est e n c o r e . L'origine et le nombre particulirrement 61ev6 de ses orateurs et de son assistance en sont la preuve.

Aprbs avoir c o n n u u n e participation latine majoritaire, depuis quelques annres, nos collrgues allemands n o u s rejoignent de plus en plus massive- ment.

L'ann6e, le sircle, se terminent et Acta Endoscopica, cr66e il y a 30 ans par la S M I E R , va conna]tre d~s le d r b u t de Fan 2000, d e u x 6volutions majeures :

- - la cr6ation d'un site W E B pour lequel n o u s vous tiendrons rapide- ment inform6 et qui sera 6galement largement utilis6 tout au long de la prrparation du prochain cours de ddcembre 2000 ;

- - la diffusion sur le plan international par la prestigieuse firme d'6dition allemande << S P R I N G E R Verlag ,> avec tout ce que cela peut comporter c o m m e int6rSt : prrsence sur le stand de l'rditeur dans toutes les grandes manifestations internationales, prdsence sur le site W E B de S P R I N G E R . Ceci se fera dans un esprit de partenariat constructif tout en laissant la plus grande libert6 aux 6diteurs et aux rddacteurs d'Acta Endoscopica.

This year once again, the X I I t h S M I E R meeting in Strasburg well deserves the qualification o f being an intensive course, considering the den- sity and quality of the programme.

All aspects of sphincterotomy, from the anatomy of papilla to complica- tions will be treated.

The topic

ofHelicobacter pylori

The state of the art in IBD and Barett's esophagus is also part o f the programme.

For quite a long time, we have thought o f endoscopy as a gateway allowing access to other more sophisticated technologies, and this year we will envisage the problems of biology and the future o f oncology.

Finally, other topics which are related to our discipline although in a more distant way, such as the endoscopic aspect o f helminths and procto- logy, will also have their place in our course.

A t the center of the acronym SMIER, we find Image (I) and therefore we could not leave out the presentation o f video clinical reports. For several years now we have devoted 5 whole mornings, at the beginning o f the course, to the viewing of live video transmissions. After deep thought and long debate, we have decided to abandon this formula and give preference to training video tapes which are even more demonstrative and focus on the technique, and do not carry the ethical or economic problems linked to video transmission.

After ~I~ comes ~ E ~ like "enseignement" (training session), where we can appreciate what guidelines, consensus conferences, the internet and the use o f simulators can bring to the knowledge o f good practice in endoscopy.

Finally, after ~ E ~ comes ~ R ~ as research. The future o f endoscopy will be seen from the angle o f its contributions to veterinary medecine and cut- ting edge technologies such as virtual biopsy and phototherapies, which will be analysed in detail.

We could not conclude this panorama without insisting once more on the evaluation which we have regularly carried out for 12 years, even before the institution o f the obligation o f post-graduate training and in spite o f the absence of well defined rules due to the negligence o f the authorities.

European, this course has always been and still is. The origin and the remarkably high number o f orators and registrants are good p r o o f o f this.

A t one time we saw a majority o f Latin participants; for several years now our German colleagues have been joining us in ever growing numbers.

This year the millennium comes to an end and

- - the creation of a website o f which you will be regularly informed and which will be of great help in the preparation o f our December 2000 course;

- - international diffusion by the prestigious German editing c o m p a n y S P R I N G E R Verlag with its valuable contributions, presence o f the editor at the exhibitions of all major international congresses and presence on the Springer website. This will be progressively settled in a constructive colla- borative spirit while leaving great freedom to the editors and writers o f our Journal.
