The electronic video endoscope of tomorrow, but first, its present status
Les ann~es 70 ont vu appara~tre les endoscopes fibres de verre et leur utilisation dans l'exploration de l'appareil digestif a c o n n u le succ~s extraordinaire que l'on sait.
La maison Welch Allyn a cr66 les premiers vid6o, endoscopes et les a pr6sent6 ~ la Soci6t6 am6ricaine de Gastro-ent6rologie il y a 4 ans.
Depuis de n o m b r e u x appareils fonctionnent de par le monde. En 1986, 25 appareils de ce type sont ven- d u s en France p r o b a b l e m e n t une centaine en 1987.

Ce materiel fut invent6 en 1969 par W.-S. Boyle et G.-E. Smith, se b a s a n t sur les propri6t6s du silicone, corps semi-conducteur sensible ~ la lumi~re.
En effet, un p h o t o n qui f r a p p e une surface de sili- cone d6termine la lib6ration d ' u n $1ectron et la cr6a- tion d ' u n t r o u o p t i q u e charg6 positivement, le tout l'origine d ' u n e f o r m e de c o u r a n t 61ectrique.
Les techniques actuelles de miniaturisation permet- tent la fabrication de cibles a la surface pr6-r6duite.
L ' e n s e m b l e de ces cibles en silicone p e r m e t t r a l'analyse des images. Les cibles sont s6par6es entre elles par des lignes de circuit p e r m e t t a n t le transfert des charges :
- - l'ensemble des cibles et lignes de circuit r6alisent
u n micro-processeur appel6 C . C . D . (Couple-Charge- Device) ;
- - l a cible elle-mSme ou p h o t o - d i o d e est appel6e
Pixels (ou picture element). L e u r n o m b r e d6termine la qualit6 de l'image : plus il y a de Pixels, plus les signaux 61ectriques transmis seront riches et com- plexes, refl6tant ainsi plus fid~lement la r6alit6.
Mais plus un C . C . D . est riche en Pixels, plus il sera volumineux, plus il sera difficile ~ miniaturiser.
E n endoscopie u n 6quilibre devra donc ~tre trouv6 entre la richesse en Pixels et le v o l u m e du C . C . D . Le viddo endoscope dlectronique (V.E.E.) :
Les volumes r6duits de C . C . D . (2,8 m m de diam6- tre p o u r le C . C . D . ) utilis6 p a r Welch Allyn permettent de le placer ~ l'extr~mit6 distale de l'endoscope juste derriere un objectif d o n t le r61e est de concentrer sur la surface sensible, la lumi~re qui sera analys6e.
Le C . C . D . est ~t l'origine de signaux de vid6o- fr6quence qui sont recueillis et transmis par un banal c o r d o n 61ectronique au d6codeur d ' u n moniteur de T.V.

- - le C . C . D . poss6de en outre un spectre tr~s large de 300 n m ~ 1 000 nm, sup6rieur aux possibilit6s de l'analyse par l'oeil h u m a i n d'o/~ la possibilit6 th6ori- que de traitement voire de cr6ation d ' i m a g e p a r ordi- nateur.
Creation d ' u n e rubrique sp~cialisde :
La relative complexit6 de l'6tude de ce mat6riel, les possibilit6s diagnostiques immenses qu'il a p p o r t e sont
apparu suffisantes h la r6daction d ' A c t a E n d o s c o p i c a p o u r n6cessiter la cr6ation d ' u n e rubrique sp6ciale pluri-annuelle d o n t elle a bien voulu m ' a t t r i b u e r la responsabilit6.
Cette rubrique c o m p o r t e r a :
- - u n e p r e m i e r e p a r t i e c o n s a c r 6 e h la vid6o- endoscopie 61ectronique (principes th6oriques, appli- cation pratique, recherche et d6veloppement...) ;
- - u n e seconde partie qui recensera, analysera et
critiquera les diff6rentes r6alisations frangaises et 6trang~res en films vid6o.
Les crit~res de s61ection, retenus sont les suivants :
- - sujet : e n d o s c o p i e digestive et application ;
- - dur6e de 10 ~t 30 minutes ;
- - qualit6 technique correcte, n o t a m m e n t possibi-
lit6 de projection en U M A T I C 3 / 4 de pouce ;
- - langue frangaise o u ~trang6re.
L ' e n d o s c o p e utilis6 peut &re indiff6remment : - - soit un f i b r o s c o p e et syst~me de cam6ra 61ectro- nique ;
- - soit un vid6o-endoscope.

The Welch A i l y n C o m p a n y which developed the first video-endoscopes presented this instrument to the A m e r i c a n Society o f Gastroenterology 4 years ago.
Since that time, m a n y o f these instruments are cur- rently in use worldwide. In 1986, 25 o f these instru- ments were sold in France, a n d it is likely that a b o u t one h u n d r e d more will be acquired in 198Z

The operation o f an electronic video endoscope is similar to that o f a ~ Solid-State ~ camera or ~ Chip Camera ~. This f u n c t i o n requires the use o f C . D . C . or ~ Charge Device Coupling ~.
I4I.-I4I. Boyle a n d G.-E. S m i t h invented this equip- m e n t in 1969, using a technique based on the proper- ties o f silicon, a light-sensitive semi-conductor.
Indeed, a p h o t o n which strikes an area o f silicon causes the release o f an electron a n d creates a positively-charge optic hole, a n d the entire process generates an electric current.
Current m e t h o d s miniaturization m a k e it possible to manufacture electronic targets having a reduced surface area.
These silicon targets together p e r m i t image analy- sis. Targets are separated f r o m each other by electri- cal circuits allowing the transfer o f electric charges :
- - the entire configuration o f targets a n d electrical circuits f o r m s a microprocessor k n o w n as a C.D.C.
(Charge device coupling) ;
h the target itself or p h o t o - d i o d e is k n o w n as Pixels (picture element). The n u m b e r o f pixels deter- mines the quality o f the image : the more pixels, the greater the c o m p l e x i t y a n d detail o f the electronic signal, thereby reproducing reality more closely.
B u t the more there are pixels in a C.D.C., the larger its size, and the m o r e difficult to miniaturize.

Thus, in e n d o s c o p y a balance m u s t be f o u n d bet- ween the n u m b e r o f pixels a n d the size o f the C . D . C . T h e electronic video e n d o s c o p e ( E . V . E . ) :
T h e reduced size o f the C . D . C . (2.8 m m in diame- ter) e m p l o y e d by Welch A l l y n m a k e s it possible to place it at the distal e n d o f the e n d o s c o p e j u s t behind an objective whose f u n c t i o n is to concentrate the light on the area sensitive to it, which will then be analyzed.
T h e C . D . C . g e n e r a t e s v i d e o f r e q u e n c y signals which are received a n d t r a n s m i t t e d b y a simple elec- tronic cable to the d e c o d e r o f a television monitor.
A n E. V.E. includes the f o l l o w i n g c o m p o n e n t s :
-- an endoscope with a c o n v e n t i o n a l mechanical e l e m e n t holding a C . D . C . ;
- - a video-processor also e q u i p p e d with a system o f insufflation a n d light source ;
- - a T V m o n i t o r w h o s e definition determines the quality o f the picture.
A d v a n t a g e s o f this s y s t e m are e v i d e n t :
- - p i c t u r e definition is superior to that obtained with optic f i b e r s a n d theoretically, m a y be increased indefinitely ;
-- all o f the techniques available with electronics : storage, a n d transmission o f signals m a y be used.
Their value is vital to investigational a n d teaching applications f o r e n d o s c o p y a n d its increased use ;
- - in addition, the C . D . C . possesses a very wide spectrum, f r o m 300 n m to 1 000 nm, which is f a r greater than what is p o s s i b l e b y the h u m a n eye a n d
t h u s the theoretical p o t e n t i a l f o r computer-ass&ted image processing or image generation.
C r e a t i o n o f a specialized r e p o r t :
The relative c o m p l e x i t y required to learn the opera- tion o f this system, the e n o r m o u s diagnostic facilities which it p r o v i d e s are such that the editors o f A c t a E n d o s c o p i c have decided to p r i n t a special column to appear t h r o u g h o u t the y e a r a n d h a v e entrusted m e with this task.
This c o l u m n will include the f o l l o w i n g :
- - p a r t ! d e v o t e d to electronic video e n d o s c o p y (theoretical principles, practical applications, deve- l o p m e n t . . . ) ;
- - p a r t H will list, analyze, a n d p r o v i d e opinions on d i f f e r e n t French a n d f o r e i g n investigations on video cassette.
To be eligible, f i l m s m u s t include the f o l l o w i n g ele- m e n t s :
- - s u b j e c t : digestive e n d o s c o p y a n d its applica- tions ;
-- length : 10 to 30 m i n u t e s ;
- - g o o d technical quality, n o t a b l y with the ability to s h o w the f i l m using a Umatic 3 / 4 inch video cas- sette recorder ;
- - the f i l m is to be p r o d u c e d in French or a foreign language.
The endoscope used m a y be either o f the f o l l o w i n g types :
- - a f i b r o s c o p e a n d electronic camera s y s t e m ; - - or a video endoscope.
