3) Prévention de la transmission/contagion :
Depuis les années 90, le Royaume du Maroc adopte la stratégie DOT supportée par l’OMS, dans le but de réduire de l’impact de la tuberculose sur la population, et trouver des solutions d’autres problèmes tels que la co-infection tuberculose/VIH, les souches de TB multi-résistantes, ou encore la mise en œuvre de méthodes modernes de diagnostic et de traitement.
Une chose est sure: afin de venir à bout de la maladie tuberculeuse, une lutte acharnée doit continuellement et impérativement être organisée par le Gouvernement en association avec le Ministère de la Santé et les différentes unités de Santé Publique, par la mise en place de programmes bien définis de lutte antituberculeuse, ainsi que par l’application de mesures rigoureuses, comportant essentiellement le dépistage de la maladie et la déclaration des sujets tuberculeux dans la collectivité, un traitement précoce et efficace les rendant non contagieux et aboutissant à leur guérison complète, l’amélioration des niveaux socio-économiques, en trouvant des solutions pour les sujets vivant en promiscuité, dans les lieux lugubres, les bidons-villes.
Ces mesures favorisent la diminution de la tuberculose toute forme, y compris les tuberculoses ostéo-articulaires. Il ressort donc que le traitement de base des tuberculoses est la prévention [100].

Dans un pays à forte endémie tuberculeuse comme le Royaume du Maroc, et devant toute symptomatologie évocatrice d’une atteinte tuberculeuse, telle une symptomatologie articulaire insidieuse d’autant plus qu’il s’agit d’une hanche ou d’un genou chez un jeune patient connu tuberculeux, des investigations cliniques et para-cliniques doivent le plus rapidement possible être mises en route.
Malgré le nombre important des cas où l’examen direct reste négatif, l’orientation diagnostique reste biologique et bactériologique, mais la pierre angulaire de confirmation reste l’histologie.
La radiologie standard joue un rôle primordial dans la démarche diagnostique. A celle-ci s’ajoutent la TDM et l’IRM, éléments clés pour confirmer et dépister ce qui a pu échapper aux clichés radiologiques.
Le traitement de la tuberculose ostéo-articulaire nécessite la collaboration entre médecin, bactériologiste et chirurgien. L’observance thérapeutique est une nécessité absolue. Un traitement précoce est la seule garantie d’une guérison sans séquelles, ainsi qu’une durée plus courte qui améliorerait l’observance du traitement.
Le dépistage et le traitement de la TOA ainsi que l’amélioration du niveau socio-économique, restent des éléments capitaux. Cependant, toute enquête antituberculeuse doit être impérativement complétée par un dépistage systématique du SIDA.

La tuberculose ostéo-articulaire (TOA) a connu une recrudescence au cours de ces dernières années. Le Maroc, pays de forte endémie, cette maladie y est toujours d’actualité.
Le but de notre travail est l’analyse des aspects épidémiologiques, diagnostiques, l’évolution et les moyens thérapeutiques de cette affection.
Il s’agit d’une étude rétrospective sur 5 cas de tuberculose ostéo-articulaire hospitalisés au service de Traumatologie Orthopédie du CHU IBN SINA de RABAT, de 2008 à 2017.
Après une étude basée sur des critères spécifiques, et des données recueillies suivant une fiche d’exploitation, les résultats nous ont permis de conclure que :
- La pathologie évolue insidieusement;
- Elle touche le sujet jeune, âgé de 20 à 50 ans, et le sujet âgé de plus de 50 ans, avec une moyenne d’âge de 49,4 ans.
- L’examen anatomopathologique des biopsies synoviale et osseuse, est d’un grand intérêt, posant le diagnostic de la TOA dans 100% des cas.
- Tous les patients ont bénéficié d’un traitement anti-bacillaire, selon le Protocole National de Lutte Antituberculeuse, aucun cas de résistance ne fut rapporté.
- Le protocole antituberculeux a été modifié optant pour une réduction de la durée de traitement à 6 mois : 2RHZE/4RH, ou encore à 9 mois : 2RHZE/7RH.
- Le pronostic fonctionnel de l’articulation dépend de la rapidité du diagnostic et de la précocité de mise en route du traitement.
- La chirurgie précoce assure un pronostic favorable de la fonction articulaire (mobilité, stabilité, fonctionnalité) et la vitalité osseuse.
- Dans le cadre de la chirurgie tardive, l’arthroplastie donne un nouvel espoir de récupération fonctionnelle et une espérance de mobilité quasi normale.

Osteo-articular tuberculosis has experienced a resurgence in recent years. MOROCCO, being a country with strong endemic tuberculosis, this disease is still relevant.
The purpose of our work is therefore to analyze the epidemiological, diagnostic and evolutionary aspects of OAT and the means of treating this condition. This is a retrospective study that has collected 5 cases of osteoarticular tuberculosis hospitalized at the Orthopedic Traumatology department of the University Hospital Center AVICENNA in RABAT, from 2008 to 2017.
After a study based on specific criteria, and data collected according to a well-defined participant information sheet, the results allowed us to conclude that :
- The disease has an insidious evolutionary character;
- It affects both young subjects, aged 20 to 50 years, and subjects aged over 50, with an average age of 49,4 years.
- Anatomopathological examination of the synovial and bone biopsy is of great interest, since it allowed the diagnosis of TOA in 100% of cases.
- All patients of our serie received antibacillary treatment, according to the National Antituberculosis Control Protocol, and no case of resistance was reported.
- The anti-tuberculosis protocol was modified recently, opting for a reduction in the duration of treatment at 6 months: 2RHZE / 4RH, or at 9 months: 2RHZE / 7RH.
- The functional prognosis of the joint depends essentially on the earliness of diagnosis and precocious start of treatment.
- Early surgery ensures a favorable prognosis of joint function (mobility, stability, functionality) and bone vitality.
- In late surgery, arthroplasty provides new hope for functional recovery and near-normal mobility expectancy.
