After receiving and reviewing your completed forms, we will call or email you to schedule a time to conduct the phone interview, which will last approximately 30-40 minutes. We would like to schedule the interviewas soon as possible while things are still fresh in your mind. Please remember to have a copy of your completed worksheets and Participant Activity Tracking Form with you for the interview.

Once the phone interview is completed, Phoenix will mail you a cheque for $200 in appreciation of your time and effort.

La période prévue pour effectuer les tâches demandées dans le cadre de l'étude de Santé Canada est maintenant terminée. Veuillez envoyer vos feuilles de travail remplies dès que possible ou au plus tard le (préciser la date et l'heure). Au moment d'envoyer vos feuilles de travail, rappelez-vous:

- d'en garder une copie;

- de vous assurer que votre écriture est lisible si vous avez rempli les feuilles à la main;

- de joindre une copie de votre Grille de suivi des activités du participant ou de la participante.

Après avoir reçu et passé en revue vos documents, nous communiquerons avec vous, par téléphone ou par courriel, pour fixer la date et l'heure d'une l'entrevue téléphonique d'une durée de 30 à 40 minutes. Nous aimerions réaliser cette entrevue le plus tôt possible alors que l'expérience est encore fraîche en mémoire. Lors de l'entrevue, rappelez-vous d'avoir en main une copie de vos feuilles de travail remplies et de votre Grille de suivi des activités du participant ou de la participante.

Après l'entrevue téléphonique, Phœnix vous enverra un chèque au montant de 200 $, par la poste, en guise de reconnaissance du temps et de l'énergie que vous aurez consacrés à l'étude.

Enfin, veuillez nous indiquer, en réponse au présent courriel, comment vous nous ferez parvenir vos feuilles de travail, c'est-à-dire par courriel (avec fichier PDF), par télécopieur ou par la poste. Online Tools & Information to Help People Quit Smoking

& Stay Smoke- Free Instruction Sheet for Participants

This document sets out the tasks that you will need to undertake as a participant in this study.

Research participants are asked to:

1. Review and use a set of on-line tools and information related to quitting smoking and staying smoke-free.

2. Take part in a phone interview about their impressions of, and experience with, the online materials (an additional 10-15 minute follow-up interview may be conducted about six months later).

Sections ta Read:

When you access the site you wiU see a number of links to information sections under the heading "Staying Quit", followed by a set ofWorksheets.

::::::> Participants should read aU of the information in the first section "Creating a quit plan" (which has a number of sub-sections).

::::::> In addition, please read at least two (2) other information sections of your choice (to be accessed through the main page under the heading "Staying Quit"). y ou should choose sections that seem most relevant or interesting to you personally.

Please read aIl of the information in each of the sections that you read. In doing this, note the following:

• Read the information as if you had come to it on your own and based on your own habits. For example, you may decide to read a section all at once, read sorne of it at one time then return to it later, or read the section more than once.

• Note anything that is unclear or about which you would like to ask a question or make a comment during the phone interview. One way to do this is by printing the page in question and making notes directly on it (and then saving this for the interview).

• Sorne parts of what you read may seem more relevant to CUITent smokers than former smokers, and vice versa. Please just continue reading the information and determine the extent to which it applies to you.

There are a number of worksheets associated with the first section you will be reading,

"Creating a quit plan". Please complete each ofthese. These worksheets include:

• Confidence Questionnaire (to Create Your Difficulty Ladder)

• Daily Record (to be completed by former smokers only)

• Tracking Form (to be completed by current smokers only)

• Rewards List

In addition, please choose one (1) other worksheet to complete from the following list. You should pick the one most relevant or interesting to you, or related to the information sections that you read. W orksheets include:

• Problem Solving

• What would you buy?

• Dealing with Lack of Support

General Instructions for Worksheets:

When you select and complete the worksheets, please note the following:

• Take the time you need to complete your worksheets. You do not necessarily need to complete a worksheet all at once.

• The 'Daily Record' does not have to be completed every day, but should be completed for 3-4 days or more.

• Note anything that is unclear or about which you would like to ask a question or make a .comment during the phone interview. Again, this can be do ne by printing the page in question and making notes on it (and then saving it for the interview).

• You will need to print off the worksheets, except for the 'What would you buy?' worksheet. AlI of the other worksheets are available in PDP format, except for the Confidence Questionnaire (to Create Your Difficulty Ladder). While this latter worksheet is not in PDP format, it can still be printed from your computer. If you have trouble printing anything, please let us know right away.

Work Period to Complete Tasks:

You can work on these activities over a three-week period. It is anticipated that the total amount of time spent on aIl aspects of this research will be about 10-15 hours. A Phoenix staff member will check in with you after about 10 days to see how things are going.

However, please contact us right away if you have any difficulties - do not wait until the end of the research period. At the end of the work period, Phoenix will contact you by email to inform you that the work period is over, remind you to send in your completed worksheets, and schedule the phone interview. When sending in your worksheets (contact coordinates provided below), please remember the following:

Telephone Interview:

After reviewing your completed forms, Phoenix will call you at the scheduled time to conduct the phone interview, which will last approximately 30-40 minutes. The interview will focus on the usefulness of the information and worksheets, are as of confusion or problems, and related issues. Please remember to have a copy of your completed worksheets and your Participant Activity Tracking Form with you for the interview. An additional 10-15 minute follow-up interview may be conducted about six months later (no decision has been made about this yet).

Payment: Once all research activities are completed, Phoenix will mail you a cheque for $200 in appreciation of your time and effort. This will be sent after the main phone interview (i.e.

we will not wait for the additional follow-up interview, ifthis happens).
