CAS CLINIQUE 1
Patient de 65 ans, polyvasculaire, à J3 de pontages aorto- coronariens.
Présente une fièvre à 38,5 °C sans point d’appel clinique clair ni signe de mauvaise tolérance, pour laquelle des hémocultures, un ECBU et une radio de thorax ont été effectués.
A 12 heures, le patient est discrètement marbré, la lactatémie artérielle est à 4,5 mmol/L. Le patient se plaint d’une douleur au point de perfusion veineuse périphérique, et le laboratoire de bactériologie appelle pour dire que les hémocultures poussent à cocci gram+ en amas.
