t h e n e e d for g i v i n g p r i o r i t y to r e s e a r c h in the s u b j e c t , for i m m u n i z a t i o n p r o g r a m m e s , a n d for a t t a i n i n g s e l f - r e l i a n c e in v a c c i n e s b y the c o u n t r i e s of the R e g i o n . In a l l h e a l t h p r o g r a m m e s r e a l i s m s h o u l d b e the k e y n o t e a n d this w a s a n a r e a in w h i c h f u n d a m e n t a l h e a l t h t e c h n o l o g y s h o u l d b e d e v e l o p e d a n d a p p l i e d .

c o u n t r i e s , and n a t i o n a l a n d i n t e r n a t i o n a l e f f o r t s m u s t j o i n t l y d e v i s e e f f e c t i v e a n d r e a l i s t i c p o l i c i e s a n d s t r a t e g i e s w h i c h c o u l d form the c o r n e r s t o n e of p r i m a r y h e a l t h c a r e . W h i l e it w a s n e c e s s a r y for the p e o p l e to a c h i e v e s e l f - r e l i a n c e in d e t e r m i n i n g their o w n h e a l t h d e v e l o p - m e n t , t h e y r e q u i r e d the s t i m u l a t i o n a n d g u i d a n c e of h e a l t h w o r k e r s w h o s e e d u c a t i o n a n d
t r a i n i n g h a d b e e n a t t u n e d to the s o c i a l n e e d s of the p o p u l a t i o n . G o v e r n m e n t s s h o u l d t h e r e f o r e m a k e a s t a r t i n r e v o l u t i o n i z i n g the e d u c a t i o n a n d t r a i n i n g of their h e a l t h p e r s o n n e l . I n this c o n n e x i o n , n a t i o n a l a d v i s o r y c o u n c i l s , or c o o r d i n a t i n g c o m m i t t e e s , as w e l l as r e g i o n a l a d v i s o r y c o m m i t t e e s o n m e d i c a l r e s e a r c h , c o u l d b e m o s t v a l u a b l e m e a n s of f a c i l i t a t i n g p r o g r a m m e
d e v e l o p m e n t .
C o n c l u d i n g , the D i r e c t o r - G e n e r a l said t h a t the r e g i o n a l c o m m i t t e e s h a d a n i m p o r t a n t r o l e to p l a y i n g e n e r a t i n g the p o l i t i c a l d e t e r m i n a t i o n r e q u i r e d to a t t a i n the h e a l t h t a r g e t s to w h i c h g o v e r n m e n t s a n d W H O h a d c o m m i t t e d t h e m s e l v e s . T h e y s h o u l d p r o v i d e the p o l i t i c a l
s t r e n g t h , m o r a l c o u r a g e a n d a d e q u a t e f o r e s i g h t n e c e s s a r y for r e a c h i n g t h e s e t a r g e t s in s p i t e of e n o r m o u s o b s t a c l e s . T h e i r a c t i o n s w o u l d j u s t i f y the v e r y c o n c e p t of W H O h a v i n g a substan- t i a l p o r t i o n of its r e g u l a r b u d g e t d e v o t e d to t e c h n i c a l c o o p e r a t i o n , as there w e r e v e r y strong f o r c e s i n f a v o u r of a c e n t r a l p o o l i n g of a l l o p e r a t i o n a l funds w i t h i n the U n i t e d N a t i o n s s y s t e m . W H O w a s t h e i r o r g a n i z a t i o n a n d t h e y m u s t g i v e the p o l i t i c a l i m p e t u s a n d s u p p o r t for c o n v e r t i n g
the O r g a n i z a t i o n1 s b l u e p r i n t for h e a l t h a c t i o n i n t o a c t i o n i t s e l f .
W h i l e p r e s e n t i n g his a n n u a l r e p o r t , the R e g i o n a l D i r e c t o r s t a t e d that the v i c t o r y a g a i n s t s m a l l p o x h a d b r o u g h t w i t h it n o t o n l y a g r e a t s e n s e of r e l i e f b u t a l s o h i g h h o p e s for a
s i m i l a r b r e a k t h r o u g h a g a i n s t o t h e r m a j o r k i l l i n g d i s e a s e s . R e f e r r i n g to the d a n g e r o u s i n c r e a s e i n the n u m b e r of c a s e s of m a l a r i a , he said t h a t c o n c e r t e d e f f o r t s w o u l d h a v e to b e d i r e c t e d t o w a r d s i n t e r d e p a r t m e n t a l c o o p e r a t i o n a t a l l levels to c o v e r a l l a s p e c t s of m a l a r i a c o n t r o l . A s for the o t h e r d i s e a s e s , l e p r o s y a n d d e n g u e h a e m o r r h a g i c fever h a d b e e n c a u s i n g i n c r e a s i n g c o n c e r n i n the R e g i o r u L e p r o s y had b e e n i d e n t i f i e d as a p r i o r i t y a r e a b y the R e g i o n a l A d v i s o r y C o m m i t t e e o n M e d i c a l R e s e a r c h . D i a r r h o e a l d i s e a s e s , w h i c h took a h e a v y toll i n c h i l d h o o d , c o u l d b e s u c c e s s f u l l y c o n t r o l l e d t h r o u g h i m p r o v e m e n t i n e n v i r o n m e n t a l s a n i t a t i o n , b u t this w a s a n a r e a w h e r e the p r o g r e s s h a d b e e n far from s a t i s f a c t o r y . M o r e o v e r , the
i m p o r t a n c e of n u t r i t i o n in d e v e l o p i n g r e s i s t a n c e to i n f e c t i o n m u s t n o t b e lost s i g h t o f . A l l e f f o r t s t o w a r d s i m p r o v i n g h e a l t h c a r e w o u l d b e i n v a i n if p o p u l a t i o n g r o w t h w e r e n o t k e p t
w i t h i n m a n a g e a b l e l i m i t s .
A n n e x W h i l e c o m m u n i c a b l e d i s e a s e s w e r e of o v e r w h e l m i n g i m p o r t a n c e in this r e g i o n , the
noneommuniсab1e d i s e a s e s c o u l d n o t b e n e g l e c t e d . T h e r e w e r e a few a r e a s in this c a t e g o r y w h i c h did c a l l for n a t i o n a l p r o g r a m m e s w i t h p r i o r i t y s u p p o r t . These i n c l u d e d the p r o b l e m s of m e n t a l h e a l t h , v i s u a l i m p a i r m e n t a n d b l i n d n e s s , c a n c e r , c a r d i o v a s c u l a r d i s e a s e s a n d d r u g
d e p e n d e n c e .
I n this c o n n e x i o n , the c o n c e p t of p r i m a r y h e a l t h c a r e as a d o p t e d b y the T W e n t y - e i g h t h W o r l d H e a l t h A s s e m b l y w a s of c o n s i d e r a b l e r e l e v a n c e . This c o n c e p t w a s b a s e d on the c o m m u n i t y a p p r o a c h , w h i c h e m p h a s i z e d that the h e a l t h s e r v i c e s m u s t b e b u i l t a r o u n d the life p a t t e r n of the p e o p l e and m u s t b e d e v e l o p e d as an i n t e g r a l p a r t of total c o m m u n i t y d e v e l o p m e n t . In c o u n t r i e s w h e r e c o u n t r y h e a l t h p r o g r a m m i n g h a d b e e n c a r r i e d o u t , the h e a l t h p l a n n i n g p r o c e s s h a d s h o w n a n a p p r e c i a b l e i m p r o v e m e n t . E f f e c t i v e t e c h n i c a l c o o p e r a t i o n a n d e x c h a n g e of
a p p r o p r i a t e k n o w l e d g e and e x p e r t i s e a m o n g the c o u n t r i e s of the R e g i o n w o u l d n o t o n l y r e s u l t i n the o p t i m a l u s e of their s c a r c e r e s o u r c e s , b u t a l s o c r e a t e the c l i m a t e for s e l f - r e l i a n c e。
A m o n g the s u b j e c t s t a k e n up b y the R e g i o n a l C o m m i t t e e w a s the r e c e n t m o v e to d e v e l o p a c e n t r a l p l a n n i n g and c o n t r o l a u t h o r i t y w i t h i n the U n i t e d N a t i o n s s y s t e m for s o c i a l and e c o n o m i c d e v e l o p m e n t . A f t e r d i s c u s s i n g this i t e m a t l e n g t h , the C o m m i t t e e s t r o n g l y e x p r e s s e d the v i e w t h a t the c o n s t i t u t i o n a l m a n d a t e of the O r g a n i z a t i o n to f u r n i s h a d e q u a t e t e c h n i c a l a s s i s t a n c e h a d to b e s a f e g u a r d e d , and a d o p t e d a r e s o l u t i o n r e q u e s t i n g the R e g i o n a l D i r e c t o r a n d the D i r e c t o r - G e n e r a l to c o n v e y its c o n c e r n a b o u t t h e s e d e v e l o p m e n t s to M e m b e r S t a t e s . The C o m m i t t e e a l s o n o t e d w i t h c o n c e r n the v e r y s m a l l n u m b e r of a c c e p t a n c e s of the p r o p o s e d a m e n d - m e n t s to the C o n s t i t u t i o n of W H O w h i c h w o u l d lead to an i n c r e a s e in the m e m b e r s h i p of the E x e c u t i v e B o a r d . I t a d o p t e d a r e s o l u t i o n u r g i n g e x p e d i t i o u s a c t i o n on the p a r t of M e m b e r S t a t e s , a s this s h o u l d lead to a m o r e e q u i t a b l e r e p r e s e n t a t i o n for the S o u t h - E a s t A s i a R e g i o n in the E x e c u t i v e B o a r d .
The C o m m i t t e e a d o p t e d a t o t a l of 17 r e s o l u t i o n s , the o t h e r s c o v e r i n g s u c h s u b j e c t s as m e n t a l r e t a r d a t i o n , b i o m e d i c a l and h e a l t h s e r v i c e s r e s e a r c h , t e c h n i c a l c o o p e r a t i o n a m o n g
d e v e l o p i n g c o u n t r i e s , i n f a n t a n d c h i l d m o r t a l i t y , t r a d i t i o n a l s y s t e m s of m e d i c i n e , t r a i n i n g in h e a l t h s e r v i c e s m a n a g e m e n t , a n d h e a l t h i n f o r m a t i o n s y s t e m s . O n the s u b j e c t of t e c h n i c a l c o o p e r a t i o n a m o n g d e v e l o p i n g c o u n t r i e s , the C o m m i t t e e e x p r e s s e d its s a t i s f a c t i o n that c o o p e r a - t i o n a m o n g the c o u n t r i e s in the R e g i o n w a s i n c r e a s i n g l y b e c o m i n g a r e a l i t y a n d w e l c o m e d the steps a l r e a d y t a k e n b y M e m b e r c o u n t r i e s in a c h i e v i n g s e l f - r e l i a n c e in h e a l t h m a t t e r s t h r o u g h c o o p e r a t i o n in the s p i r i t of r e s o l u t i o n s E B 6 0 . R 4 and W H A 3 0 . 4 3 .
O n e u n i q u e p o i n t is the fact that the C o m m i t t e e is v e r y m u c h s e n s i t i v e to and c o n s c i o u s a b o u t the p r i n c i p l e s and a p p r o a c h e s of p r i m a r y h e a l t h c a r e , w h i c h w a s r e f l e c t e d in the d i s c u s - sions t h r o u g h o u t the s e s s i o n .
In r e g a r d to the p l a n n i n g and d e v e l o p m e n t of h e a l t h s e r v i c e s , the C o m m i t t e e s h o w e d g r e a t i n t e r e s t in the v a r i o u s i n t e r r e l a t e d a s p e c t s and r a i s e d a n u m b e r of i m p o r t a n t i s s u e s . The m a i n p o i n t of the d i s c u s s i o n w a s on the c o u n t r y h e a l t h p r o g r a m m i n g e x e r c i s e s w h i c h h a d b e e n
c a r r i e d out in s o m e c o u n t r i e s , and the r e l a t i o n s h i p b e t w e e n c o u n t r y h e a l t h p r o g r a m m i n g a n d n a t i o n a l h e a l t h p l a n n i n g . The s i g n i f i c a n t d i f f e r e n c e s b e t w e e n c o u n t r y h e a l t h p r o g r a m m i n g a n d n a t i o n a l h e a l t h p l a n n i n g w e r e e x p l a i n e d , a n d it w a s s t a t e d t h a t , s i n c e c o u n t r y h e a l t h p r o g r a m - m i n g took into c o n s i d e r a t i o n a s p e c t s of i n t e r s e c t o r a l r e l a t i o n s h i p s of h e a l t h a n d of p r o g r a m m e i m p l e m e n t a t i o n a s w e l l , the c o m m i t m e n t for the d e l i v e r y of the p l a n w a s s t r o n g e r .
T h e r e w a s m u c h d i s c u s s i o n on n a t i o n a l h e a l t h i n f o r m a t i o n s y s t e m s . R e f e r e n c e w a s m a d e to the i m p o r t a n t a n d u r g e n t n e e d for d e v e l o p i n g i n f o r m a t i o n s y s t e m s in the R e g i o n , a n d stress w a s laid on the role a n d e f f e c t i v e n e s s of f e e d i n g r e l e v a n t i n f o r m a t i o n to the p r o g r a m m e a n d b u d g e t f o r m u l a t i o n p r o c e s s . The C o m m i t t e e s h o w e d k e e n i n t e r e s t in the f u n c t i o n i n g of n a t i o n a l c o o r d i n a t i n g c o m m i t t e e s for W H O p r o g r a m m e s as e x e m p l i f i e d b y the R o y a l T h a i G o v e r n m e n t / W H O C o - o r d i n a t i n g C o m m i t t e e .

The d i s c u s s i o n s o n f a m i l y h e a l t h s t a r t e d w i t h a r e f e r e n c e to the i m p o r t a n c e of the e x p a n d e d p r o g r a m m e of i m m u n i z a t i o n ( E P I ) as a n i n t e g r a l p a r t of f a m i l y h e a l t h . I t w a s
s t r e s s e d t h a t , in v i e w of the p r e s e n t l e v e l of t e c h n i c a l k n o w - h o w a n d r e s o u r c e s , E P I w a s g o i n g to b e a p r o g r a m m e w i t h n o end in s i g h t , a n d t h e r e f o r e it m u s t b e f u l l y i n t e g r a t e d w i t h the m a t e r n a l and c h i l d h e a l t h / f a m i l y h e a l t h c o m p o n e n t of the h e a l t h s e r v i c e s . T h e C o m m i t t e e u n a n i m o u s l y a g r e e d that the b e s t a p p r o a c h for the d e l i v e r y of c h i l d h e a l t h w a s t h r o u g h the f a m i l y h e a l t h s e r v i c e , w h i c h s h o u l d i n c l u d e n u t r i t i o n , i m m u n i z a t i o n , r e h y d r a t i o n and d i a r r h o e a l d i s e a s e c o n t r o l , and h e a l t h e d u c a t i o n . It w a s e m p h a s i z e d t h a t this group of s e r v i c e s s h o u l d b e the c o r e of p r i m a r y h e a l t h c a r e .
A w o r k i n g p a p e r on i n f a n t m o r t a l i t y p r e s e n t e d to the R e g i o n a l C o m m i t t e e s t r e s s e d the i m p o r t a n c e of r e l i a b l e i n f o r m a t i o n or s t a t i s t i c s r e l a t i n g to c h i l d r e n . The i m p o r t a n c e of d e m o g r a p h i c d a t a a n d v i t a l s t a t i s t i c s as a b a s i s for c h i l d h e a l t h p r o g r a m m e s w a s a l s o high-
l i g h t e d . In a d o p t i n g a r e s o l u t i o n on this s u b j e c t (S E A / R C 3 O / R1 1 ) , the C o m m i t t e e n o t e d that i n i t i a l s t e p s c o u l d b e t a k e n w i t h o u t w a i t i n g for d e t a i l e d s c i e n t i f i c i n f o r m a t i o n .
N o t i n g t h a t p o p u l a t i o n e x p l o s i o n h a d b e e n i d e n t i f i e d as a m a j o r p r o b l e m in m o s t c o u n t r i e s of the R e g i o n , the C o m m i t t e e a g r e e d t h a t the m o s t s u c c e s s f u l e x a m p l e s of f a m i l y p l a n n i n g p r o g r a m m e s w e r e from those c o u n t r i e s w h e r e f a m i l y p l a n n i n g w a s i m p l e m e n t e d as a n i n t e g r a l p a r t
of the h e a l t h s e r v i c e s。 The C o m m i t t e e n o t e d that there h a d b e e n i n v a l u a b l e a s s i s t a n c e f r o m the U n i t e d N a t i o n s and o t h e r a g e n c i e s i n this f i e l d , a p p r e c i a t e d the c o o r d i n a t i n g r o l e of and t e c h n i c a l i n p u t f r o m W H O in this p r o g r a m m e and e x p r e s s e d the d e s i r e for f u r t h e r h e l p .
I n d i s c u s s i n g the p o s s i b i l i t y of r e n d e r i n g oral h e a l t h a n d m e n t a l h e a l t h s e r v i c e s as a p a c k a g e d e l i v e r e d t h r o u g h p r i m a r y h e a l t h c a r e , the C o m m i t t e e took n o t e of a r e s o l u t i o n of the W o r l d H e a l t h A s s e m b l y ( W H A 3 0. 3 8 ) on m e n t a l r e t a r d a t i o n . It p a s s e d a r e s o l u t i o n (S E A / R C3O / R 4 ) s u g g e s t i n g that c o n s i d e r a t i o n b e g i v e n to e x p l o r i n g and d e v e l o p i n g s u i t a b l e a c t i v i t i e s to t a c k l e m e n t a l r e t a r d a t i o n in the R e g i o n .
O n the s u b j e c t of c o m m u n i c a b l e d i s e a s e s , the C o m m i t t e e n o t e d w i t h s a t i s f a c t i o n the r e m a r k a b l e s u c c e s s that h a d b e e n a c h i e v e d in the e r a d i c a t i o n of s m a l l p o x , b u t v i e w e d w i t h c o n c e r n the i n c r e a s i n g t r e n d of m a l a r i a i n c i d e n c e i n m o s t of the c o u n t r i e s。 It s t r e s s e d the n e e d for the p r o d u c t i o n of i n s e c t i c i d e s a n d a n t i m a l a r i a l s w i t h i n the R e g i o n . The C o m m i t t e e w a s i n f o r m e d of a c t i o n b e i n g t a k e n w i t h r e g a r d to r e s e a r c h on P l a s m o d i u m f a l c i p a r u m r e s i s t a n c e .
It a l s o n o t e d the s u b s t a n t i a l d e c r e a s e in the c a s e - f a t a l i t y f r o m h a e m o r r h a g i c fever as a r e s u l t of steps taken on the b a s i s of the t e c h n i c a l guides p r e p a r e d b y the W H O T e c h n i c a l A d v i s o r y C o m m i t t e e on D e n g u e H a e m o r r h a g i c F e v e r .
The C o m m i t t e e , w h i c h e x p r e s s e d its s a t i s f a c t i o n w i t h the r e s e a r c h a c t i v i t i e s u n d e r t a k e n i n the R e g i o n in the field of l e p r o s y , w a s i n f o r m e d that d r u g trials on r i f a m p i e i n w e r e g o i n g on in B u r m a a n d I n d i a a n d that it w o u l d t h e r e f o r e b e a p p r o p r i a t e to a w a i t the r e s u l t s of t h e s e trials b e f o r e c o n s i d e r i n g the l a r g e - s c a l e u s e and p r o d u c t i o n of this d r u g in this r e g i o n .
C o n s i d e r a b l e d i s c u s s i o n took p l a c e on the q u e s t i o n of v a c c i n e p r o d u c t i o n in the R e g i o n . The C o m m i t t e e w a s i n f o r m e d t h a t a m e e t i n g w o u l d be c o n v e n e d o n the s u b j e c t in order to d e v e l o p a p r o g r a m m e w h i c h m i g h t lead the c o u n t r i e s to s e l f - s u f f i c i e n c y i n v a c c i n e s .
The d e v e l o p m e n t of the h e a l t h m a n p o w e r r e q u i r e d for the i m p l e m e n t a t i o n of the p r i m a r y h e a l t h c a r e p r o g r a m m e r e c e i v e d p a r t i c u l a r a t t e n t i o n . O r i e n t a t i o n of the m e d i c a l c u r r i c u l u m t o w a r d s t r a i n i n g in p r i m a r y h e a l t h c a r e , and the t r a i n i n g of m i d w i v e s a n d o t h e r c o m m u n i t y - l e v e l h e a l t h w o r k e r s as w e l l as m i d d l e - l e v e l h e a l t h w o r k e r s w e r e some of the a r e a s w h i c h w e r e high- l i g h t e d . The r e s o l u t i o n a d o p t e d on this s u b j e c t a l s o s t r e s s e d the n e e d for d e v e l o p i n g s u i t a b l e p r o g r a m m e s for the t r a i n i n g of v o l u n t a r y w o r k e r s . I n a c c o r d a n c e w i t h u s u a l p r a c t i c e , a S u b - c o m m i t t e e on P r o g r a m m e B u d g e t w a s e s t a b l i s h e d to r e v i e w inter alia the p r o p o s e d r e v i s i o n s to the 1978 a n d 1979 p r o g r a m m e b u d g e t . The R e g i o n a l C o m m i t t e e a p p r o v e d the r e p o r t of this S u b - c o m m i t t e e a n d a d o p t e d a r e s o l u t i o n ( S E A / R C 3 O / R 9 ) .
"Health i n f o r m a t i o n s y s t e m s w i t h s p e c i a l r e f e r e n c e to p r i m a r y h e a l t h c a r e a n d c o m m u n i t y d e v e l o p m e n t " w a s the topic for the t e c h n i c a l d i s c u s s i o n s h e l d d u r i n g the s e s s i o n .
