We readwithinterest thearticle byRehman etal. on post-artesunate hemolysis (PADH).1 Hemolysis is indeed commonly associatedwiththeclassofartemisinindrugswhenusedforthe treatment of severe malaria. Their review confirmed the high incidenceofthis adverseeventrelatedtothemodeofactionof artemisinins and the physiological role of the spleen.2 As the authors wisely pointed out, delayed hemolysis should not jeopardizethedeploymentofartesunateasthefirst-linetreatment ofseveremalariaworldwide.WhilePADHisindeedrarely(ifever) fatal, artesunatesignificantly reducesmortality ascompared to quinine,markedlysoinpatientswithparasitemiagreaterthan10% onadmission.3Importantly,PADHhasbeenfurthercharacterized byapathophysiologicalstudyintravelerstreatedwithartesunate forseveremalaria.4Earlypeakconcentrationsofcirculating once-infectederythrocytesarepredictivefortheoccurrenceofPADH andmaythereforeserveasafuturecandidatemarkerforPADH. Rehmanetal.referredtodifferentanemiapatternsintheiranalysis of published reports. This definition was based on initial descriptions by Zoller et al.5 and on a more refined definition proposed in a closed meeting organized by the Medicine for MalariaVenture(MMV)inMarch2013inVienna (http://www. mmv.org/sites/default/files/uploads/docs/events/2013/ InjectableArtesunateExpertGroupMeeting.pdf)6 and later pub-lishedintheabovementionedpathophysiologicalpaper.4Whereas initialreportswereacknowledgedinthearticlebyRehmanetal., theoptimizednosologicalclassificationwasnotreferencedinthe manuscript.Thisomissionis likely duetothedisclosurein the MMVreport(whichbecamepubliclyavailablein2013)ofthethen unpublished, optimized case definition proposed by ourgroup, withoutprecisereferencingofthesourceofthatparticularsetof information. Sharing unpublished results or concepts during subject-specific meetings, like the one efficiently organized by MMVinVienna,isimportantasitallowsthemalariacommunityto adaptrapidlyandefficientlytonewproblemswithapublichealth impact.Whenreportsofsuchmeetingscarefullyacknowledgeall respectivecontributions,thisultimatelycontributesto maintain-ingrichexchangesbetweenattendees.
