Summary We report the management of a spontaneous ovarian hyperstimulation syndrome in a patient of 26 years. It is diagnosed at eight weeks gestation. The etiological investigation of a spontaneous ovarian hyperstimulation syndrome should seek hypersecretion of glycoprotein hormones (hCG, TSH, FSH and LH) and/or mutation of FSH and LH receptors. It will eliminate incipient ovarian neoplasia. The laparoscopic exploration can be performed if diagnostic doubt persists.
