The Director-General has the honour to present to the Executive Board the report on the nineteenth session of the Regional Committee for the A m e r i c a s / X V I I Meeting of the Directing Council of the P a n A m e r i c a n Health O r g a n i z a t i o n . This report is p r e c e d e d by an introduction which summarizes the discussions and action taken at this m e e t i n g .

INTRODUCTION

T h e XVII M e e t i n g of the Directing Council of the Pan American Health O r g a n i z a t i o n , n i n e t e e n t h session of the Regional Committee of the World Health Organization for the A m e r i c a s , was held at the Trinidad Hilton H o t e l , Port-of-Spain, Trinidad and Tobago, from 2 to 12 October 1 9 6 7 .

A l l the Member States of the R e g i o n , w i t h the exception of B o l i v i a , were represented at the M e e t i n g as were also F r a n c e , the N e t h e r l a n d s and the United Kingdom of Great Britain and N o r t h e r n Ireland, acting on behalf of certain territories of the R e g i o n .

A l s o represented w e r e the Governments of Barbados and Guyana w h i c h , at the second plenary session, were admitted as M e m b e r s of РАНО.

T h e Meeting was also attended by representatives of the United N a t i o n s , UNICEF, F A O , the Organization of American States, the Inter-American Development B a n k , five non-

governmental o r g a n i z a t i o n s , and by the Deputy Director-General of W H O . Canada appointed an official o b s e r v e r .

T h e Meeting considered the annual report of the Director of the P a n American Sanitary B u r e a u / R e g i o n a l Director of W H O .

T h e appropriations for РАНО for the financial year 1968, amounting to US$ 10 190 000, w e r e a p p r o v e d , as was the proposed regional programme and budget estimates of WHO for 1969, which w e r e to be transmitted to the D i r e c t o r - G e n e r a l . The Directing Council took note of the provisional draft of the proposed programme and budget estimates of РАНО for the same y e a r .

T h e financial report of the Director and the report of the External Auditor for the fiscal year 1966 were also a p p r o v e d .

In considering the status of malaria e r a d i c a t i o n , the Directing Council noted that, although progress had been m a d e and some technical problems had been solved through research, serious administrative and financial difficulties still p e r s i s t e d . In accordance with the r e s o l u t i o n of the World H e a l t h Assembly calling for a re-examination of the global strategy of m a l a r i a e r a d i c a t i o n , the Directing Council recommended to the Director of РАНО that he intensify his co-operation with the Director-General of WHO to this e n d . Similarly, it re-emphasized the need to redouble efforts to eradicate this disease and to give special a t t e n t i o n to the financing, administration and co-ordination of services and to the training of personnel; and recommended that special attention should be given to problem areas and to operational r e s e a r c h . . Similarly, it was decided to request international lending agencies m a k i n g loans to avoid requirements that might weaken the prime purpose of eradica- tion p r o g r a m m e s .

T h e Council reaffirmed the policy of PAHO/WHO w i t h respect to the importance of inter- national meetings of a technical character and recommended to governments and to the Director of РАНО that they give their support to t h e s e .

T h e problem of poliomyelitis was considered and, as it continued to pose a threat to h e a l t h , largely because of insufficient immunization, it was recommended to governments that they should conduct systematic immunization programmes, in order to achieve the highest immunity level as quickly as p o s s i b l e . Governments were urged to improve their

epidemiological and statistical services so as to obtain prompt notification of cases and РАНО w a s requested to continue to provide the countries with assistance in bringing epidemic o u t b r e a k s under c o n t r o l .

W i t h reference to the w a t e r - s u p p l y programme, the Directing Council examined the report of the Director of РАНО, which covered the first half of the A l l i a n c e for P r o g r e s s d e c a d e . It urged the need to continue to give priority attention to programmes of water supply, sewerage and water pollution control, in close association with national and inter- national lending agencies, with a view to ensuring that proposals for programmes of this nature were given p r i o r i t y . It also recommended that the programme of research in engineering e d u c a t i o n should be expanded and that governments re-evaluate their plans for urban and rural w a t e r supply and sewerage services and analyse the status of water pollu- tion w i t h a view to strengthening national structures and accelerating p r o g r a m m e s .

The n u t r i t i o n programme was examined together w i t h the proposals for its expansion and it was recommended that all aspects of this programme should continue to be d e v e l o p e d , special emphasis being given to the establishment of a data collection and analysis system for nutrition in the A m e r i c a s , the formulation of national nutrition policies and the intensification of r e s e a r c h . It was also recommended that the programme should be developed in close collaboration w i t h other international and bilateral agencies, intensifying co-operation with FAO in the study of possibilities for increased food pro- duction; it w a s recommended furthermore that grants should be sought from governments and private sources to expand the programme and that an integrated programme of research should be organized into the factors that contributed to pre-school m o r t a l i t y .

The Council noted that in the preceding five years the campaign for the eradication of A e d e s aegypti had m a d e progress only in rather limited areas, owing to administrative and

financial d i f f i c u l t i e s , and urged governments to fulfil their commitments and provide one another with reciprocal assistance, in the form of loans, grants and m a t e r i a l s and to m a i n t a i n strict surveillance against reinfestation. It also recommended to the lending agencies that they m a k e funds available to governments for the eradication of the v e c t o r .

Consideration was given to the decisions of the Inter-American Committee on the

A l l i a n c e for P r o g r e s s and of the Inter-American Economic and Social Council with respect to the Pan American Foot and Mouth Disease Center, recommending that РАНО should continue to assume technical and administrative responsibility for the Center and that it should be financed by m e a n s of contributions by the governments of the Member States of O A S . T h e Council expressed its appreciation to O A S , CIAP and IA-ECOSOC for the assistance they had rendered to the Center and accepted the r e c o m m e n d a t i o n s .

It was recommended to the Member countries that they undertake as part of economic and social development the planning of the health sector and that all the agencies c o n c e r n e d , especially social security institutes, should participate in t h i s .

T h e importance of the programme for the supply of textbooks for m e d i c a l students was reaffirmed and it was recommended that negotiations to obtain funds should continue w i t h the Inter-American D e v e l o p m e n t Bank and with other possible sources for the financing of the p r o g r a m m e .

The Directing Council recommended to the governments that they give smallpox eradica- tion the priority it deserved, that they maintain a high level of immunization and constant vigilance against the introduction of the disease whilst paying special attention to the preparation of smallpox v a c c i n e s , and that the countries concerned co-ordinate their activities to reduce the risk of the spread of the d i s e a s e .

Within the limitations of the information available on C h a g a s ' disease and of the therapeutic p o s s i b i l i t i e s , it was recommended that research on the biology and control of the vector as well as on d i a g n o s t i c methods and therapeutic products be intensified.

It w a s recommended that РАНО p r e p a r e a report on the relations between health and l a w , o u t l i n i n g a p l a n for c o l l a b o r a t i o n with governments that request i t , in this new field of activity for the O r g a n i z a t i o n .

T h e Council reviewed the Organization's proposal for co-operative programmes to

s t r e n g t h e n multinational activities for advanced training and research in the health sciences and instructed the Director of РАНО to continue to stimulate such activities by all possible m e a n s . It also emphasized the importance of ensuring that the best fellowship applicants w e r e selected and the fullest u s e m a d e of the services of fellows w h e n they completed their

s t u d i e s .

T h e Council considered the D e c l a r a t i o n of the A m e r i c a n Presidents and the Action Program approved by them at Punta del Este in April 1967 and expressed its satisfaction with the r e c o g n i t i o n given to health in the D e c l a r a t i o n . It also expressed its thanks to the G o v e r n m e n t of Argentina for its invitation to hold at Buenos A i r e s a special meeting of M i n i s t e r s of Health of the H e m i s p h e r e , w i t h a view to drawing up a plan of operations for implementing the decisions of the Presidents prior to the XVIII Meeting of the Directing Council of РАНО and Twentieth Meeting of the Regional Committee of W H O for the A m e r i c a s , a m e e t i n g that, in response to the invitation of the Argentine G o v e r n m e n t , would be held at Buenos A i r e s .

A n amendment w a s proposed to A r t i c l e 15-A of the Constitution of РАНО that would increase to nine the number of Member governments forming the Executive Committee and it was decided that the item should be included in the draft agenda of the XVIII M e e t i n g of the Directing C o u n c i l , Twentieth M e e t i n g of the Regional Committee of WHO for the A m e r i c a s .

T h e Committee resolved to appoint M r Lars Breie as the External A u d i t o r of РАНО, on the resignation from this appointment of M r Uno B r u n s k o g .

T h e topic considered at the Technical Discussions during the Meeting w a s "Methods for Increasing Health Service Coverage in the Rural A r e a s " • The topic selected for the T e c h n i c a l Discussions to be held in 1968 was "Participation of the Health Sector in P o p u l a t i o n P o l i c y " .
