results to the three hazelnut components spotted
on the microchip. Figure 2 displays the
correla-tion between sIgE to Cor a 9 on the microarray
and sIgE hazelnut by traditional ImmunoCAP.
From this figure, it emerges that sIgE to Cor a 9
is highly related to sIgE hazelnut (r = 0.88,
p < 0.001). Except for one, all Cor a 9-positive
sera spotted on the microarray, had a sIgE to
hazelnut exceeding 5.5 kUA/l, whereas all sIgE
to hazelnut for the Cor a 9-negative sera were
< 2.0 kUA/l. To confirm the Cor a 9 microarray
data, sera were also analyzed by a singleplexed
ImmunoCAP assay coated with the samenCor a
9 component as used in the microarray. This
comparative analysis demonstrated a high
corre-lation (r = 0.91, p < 0.001).
Next to nCor a9, themicroarray containsthree
other 11S legumin-like seed-storage proteins, i.e.,
nAra h 3 from peanut (Arachis hypogea), nGly m
6 from soy (Glycin max), and rAna o 2 from
cashew (Anacardium occidentale). Figure 1
dis-plays the percentages of positive results for these
different components. Sensitization to these
leg-umin-like proteins was restricted to the infants
sensitized to nCor a 9, except for oneinfant with
a sensitization to nAra h 3 but not to nCor a 9.
Actually, half of thepatients sensitized to nCor a
9demonstrated IgE reactivity to nArah 3. Fiveof
these six nAra h 3-sensitized infants also
demon-strated a positive sIgE result to nGly m 6 from
soy. Only one nCor a 9-sensitized infant showed
IgE reactivity to rAna o 2.

the percentage of activated basophils, i.e., co-expressing
CD63, was measured. Results were expressed as percentages
of CD63
+basophils after subtraction of the negative control.
Statistical analysis
Results were expressed as median (range). Non-parametric
tests were used where appropriate. IBM SPSS 17 (IBM ,
Chicago, IL, USA) software was used for data analysis.
Chi-squared analysis was performed to compare frequencies.
A p value of < 0.05 was regarded as statistically significant.
Results
Demographics and clinics
Table 1 displays the results of all 103 purified and
recombi-nant components spotted on the microarray. Demographics
and results (median and range) of total IgE and singleplexed
sIgE to hazelnut and birch pollen of patients and control
individuals are summarized in Table 2. Twenty-nine patients
in our cohort suffered from a systemic reaction [17 preschool
children (median age of 2 years), six school children, and six
adults] and 36 patients reported an oral allergy syndrome
(three preschool children, nine school children, and 24
adults).
Sensitization to hazelnut components
The percentages of sensitization to the different purified and
recombinant hazelnut components (rCor a 1.04, rCor a 8,
and nCor a 9) and rBet v 1 from birch pollen are displayed
in Fig. 1.
The major observation is that in our birch-endemic region,
65% of preschool children and 50% of the school children
with systemic reactions on ingestion of processed hazelnut
(chocolate or chocolate spread) are sensitized to Cor a 9. In
hazelnut allergic adults with systemic reactions, sensitization
to Cor a 9 was demonstrable in 17%. The clinical data and
sensitization profiles of the Cor a 9 sensitized patients are
dis-played in Table 3. All our patients sensitized to Cor a 9
reported systemic reactions on ingestion of hazelnut in its
