Notre étude est limitée par le fait qu’elle est rétrospective et monocentrique. De plus,
le faible nombre de patients inclus limite sa puissance et donc la mise en évidence de
différences significatives.
Il aurait été intéressant de mesurer l’évolution du diamètre aortique au niveau du collet
distal de l’endoprothèse qui est le point de départ de plusieurs reprises chirurgicales.
Or il est n’est pas aisé de trouver une mesure reproductible sur les scanners
préopératoires et qui pourrait anticiper la zone d’ancrage distal des endoprothèses.
Nous aurions pu nous axer uniquement sur les patients qui avaient nécessité une
reprise afin d’étudier de manière plus précise l’évolution des diamètres aortiques et
leur chronologie exacte ainsi que les complications ayant engendré ces reprises
chirurgicales, notamment en étudiant plusieurs angioscanners post-opératoires. Ceci
aurait de plus pu donner une meilleure homogénéité sur le plan temporel. En effet, on
remarque que les délais entre la chirurgie et les scanners de contrôle étudiés dans
notre étude sont très variables, notamment à cause des reprises précoces. Ceci influe
sur la nécessité d’avoir des délais suffisants pour étudier le remodelage de l’aorte sur
le long terme.
6 Conclusion
L’endoprothèse thoracique est le traitement de choix pour les dissections aortiques
compliquées, aiguës ou chroniques. Cette intervention permet une fermeture de la
porte d’entrée proximale et donc un remodelage de l’aorte à ce niveau avec une
expansion satisfaisante du VC et une bonne régression du FC. De plus cela permet
de diminuer la morbi-mortalité liée à une prise en charge chirurgicale conventionnelle.
Malgré ces bons résultats, dans la littérature ainsi que dans notre étude, 48% des
patients de notre série nécessitent une reprise chirurgicale et dans une grande
majorité des cas pour une évolution anévrysmale de l’aorte. Notre étude permet de
montrer une tendance à l’expansion du FC au niveau abdominal après la pose de
TEVAR, mais celle-ci n’est pas significative, même si l’on retrouve rarement une
thrombose du faux chenal à ce niveau. Cette expansion au niveau abdominal n’est
pas non plus significative chez les patients ayant spécifiquement nécessité une reprise
chirurgicale devant une évolution anévrysmale. On voit donc que les diamètres au
niveau thoracique sur la zone stentée et au niveau abdominal sont bien contrôlés. En
analysant les indications de reprises chirurgicales de manière plus détaillée, on note
que la plupart d’entre elles ont nécessité l’adjonction d’un module distal à
l’endoprothèse initiale. La zone la plus à risque serait alors la partie distale de l’aorte
thoracique descendante juste en amont de l’aorte abdominale. Il est important de
rappeler que malgré une forte proportion de reprises chirurgicales, la survie des
patients est comparable à celle retrouvée dans la littérature. Il serait alors intéressant
d’étudier plus spécifiquement la zone d’ancrage distal des endoprothèses qui semble
être une zone d’intérêt mais aussi de chercher à obtenir une meilleure exclusion du
FC afin d’en assurer une thrombose complète chez une proportion plus importante de
patients. Pour finir, le suivi n’est donc pas à négliger afin de dépister au plus tôt toute
évolution anormale qui pourrait faire poser l’indication d’une nouvelle intervention sur
l’aorte.

Introduction :
Les dissections de type B compliquées sont traitées par endoprothèse thoracique, cette intervention induit un
remodelage aortique pour éviter une évolution anévrysmale. Malgré ce traitement, une réintervention est
parfois nécessaire pour contrôler les diamètres. L’objectif est d’étudier ces diamètres au niveau
thoraco-abdominal après traitement par TEVAR.
Matériel et méthodes :
Les scanners de tous les patients ayant bénéficié d’un traitement par TEVAR pour une dissection aortique de
type B entre 2010 et 2018 au CHU de Caen ont été étudiés. Les surfaces aortiques du vrai et du faux chenal
ont été comparées avant et après l’intervention chirurgicale au niveau thoracique et abdominal.
Résultats :
29 patients ont été traités par TEVAR entre 2010 et 2018. Pendant le suivi, on observe un bon remodelage
au niveau thoracique après l’intervention avec une expansion du vrai chenal (p<0 ,001) et une régression du
faux chenal (p=0,009). Au niveau cœliaque, on met en évidence une stabilité du VC et une tendance à
l’augmentation du FC. Cette augmentation abdominale n’est pas significativement plus importante chez les
14 patients ayant nécessité une reprise chirurgicale alors que 80% des reprises se font pour évolution
anévrysmale.
Conclusion :
On retrouve un bon remodelage aortique au niveau de l’endoprothèse thoracique. Au niveau cœliaque, on
observe une tendance à l’expansion des diamètres sans différence significative. De plus, on ne retrouve pas
d’expansion plus importante chez les 48% de patients ayant nécessité une reprise chirurgicale. La zone
d’évolution la plus à risque se situe à la partie distale de l’endoprothèse sur l’aorte thoracique descendante.

Introduction:
Complicated type B aortic dissections are treated with TEVAR which induce aortic remodeling to avoid
aneurysmal evolution. Despite this treatment, reoperation is sometimes required to control aortic diameters.
How is the evolution of the diameters in the abdominal part?
Methods:
CT scans of all the patients treated with TEVAR for a type B aortic dissection from 2010 to 2018 in CHU de
Caen have been reviewed. Surfaces of the true lumen (TL) and false lumen (FL) have been compared before
and after the surgery at the thoracic and abdominal levels.
Results:
29 patients have been treated with TEVAR in CHU de Caen from 2010 to 2018. During the follow-up, we
observe a significant thoracic remodeling after the surgery with TL expansion (p<0,001) and FL regression
(p=0,009). At the coeliac level, we can observe a stability of the TL and a tendency for the FL to increase.
This abdominal growth is not significantly more important for the 14 patients who needed a reintervention
while 80% of the secondary interventions are for aneurysmal growth.
Conclusion:
We show a satisfying thoracic remodeling. At the coeliac trunk, we observe a trend in the aortic diameter
growth but not significant. This trend is not significative either in the 48% who needed a secondary
intervention. The most-at-risk evolution zone is the distal part of the graft on the descending thoracic aorta.
