The twelfth session of the Regional Committee for the formally opened at the Queen Margaret C o l l e g e , W e l l i n g t o n , Right Honourable the Prime M i n i s t e r , M r K . J . The agenda is given in A n n e x 1 and the list of participants in Annex 2 . A t its first plenary session the Committee established its Sub-Committee on Programme and B u d g e t , composed of representatives from the following countries:

Cambodia, China, J a p a n , M a l a y a , New Z e a l a n d , the Philippines, the United K i n g d o m and V i e t N a m . The Sub-Committee held two m e e t i n g s , following which it submitted a

report to the main Committee. Further details are given in P a r t 工I and A n n e x 3 of this r e p o r t .

The thirteenth session of the Committee w i l l be held in M a n i l a w i t h the Government of the Philippines as h o s t . As decided by the Committee at its tenth session (resolution W P / R C 1 0 . R 6 ) , the fourteenth session of the Committee w i l l be h e l d in Port Moresby, P a p u a . The Committee accepted the invitation from the Government

•of K o r e a to hold its sixteenth session in S e o u l# (See resolution WP/RC12.R11,) In the course of five plenary sessions, the Committee adopted eleven resolutions which are set out in Part V .

PART

• ANMJAL REPORT OF THE REGIONAb DIRECTOR

T h e Committee examined in detail the Eleventh Annual Report of the Regional D i r e c t o r , w h i c h , in addition to covering the work from 1 July of the previous year to 50 J u n e of the p r e s e n t , also 'summarizes the main developments in the health field in the R e g i o n during the first d e c a d e . It noted that national health administrations h a d been strengthened; rural health services initiated; the education and training of h e a l t h personnel had expanded, in quality as well as in quantity; much progress h a d been made in the control of communicable diseases; environmental sanitation had b e e n i m p r o v e d . The malaria eradication programme had continued to make progress b o t h in scope and q u a l i t y .

I n 196О-61, emphasis had been placed on assistance in strengthening national h e a l t h administrations, and in particular on the incorporation of health services in

community development s c h e m e s . In L a o s , M a l a y a , the Philippines and V i e t N a m , community development had as a component the expansion of rural health services, and the Committee rioted that health planning and administration at various levels had increasingly made use of consultation and co-ordination between departments of health, agriculture and e d u c a t i o n . A t the international level, community development in L a o s h a d seen a joint effort of a number of international agencies, including W H O .

T h e r e was also evidence of much interest in nutrition by health departments in t h e R e g i o n , and the R e g i o n a l Office was being increasingly called on for general and detailed technical advice in this field # Here again

,special emphasis had been

placed on the value of co-operative action by the international agencies^ and during the past year the closest liaison had been maintained with FAO and U N I C E F .

I n environmental sanitation the major development had been the increased

interest shown in the provision of community water supplies, stimulated by the global w a t e r supply programme of W H O .

BCG vaccination programmes in China (Taiwan), the Philippines, Hong K o n g , Malaya, N e t h e r l a n d s New G u i n e a , Singapore and V i e t Nam had continued smoothly, and a

tuberculosis control programme was started in Western S a m o a . Furthermore, there had b e e n a marked increase in the number of countries and territories planning to embark on public health programmes for tuberculosis control, and several countries,

including A u s t r a l i a , New Zealand and J a p a n , had indicated their interest in

co-operating with W H O in studies on the isolation and identification of mycobacteria from h u m a n sources in tropical and subtropical areas.

During the d i s c u s s i o n , the importance of a flexible approach to programme

p l a n n i n g , bearing in mind the great differences in the socio-economic development of the countries in the R e g i o n,w a s stressed. Mention was made of the need to develop r u r a l public health services as part of a comprehensive and integrated health

p r o g r a m m e ; and the suggestion made that a special section in the Regional D i r e c t o rTs A n n u a l R e p o r t should be devoted to rural health»

I t w a s hoped that the p r o m o t i o n of m e n t a l h e a l t h would r e c e i v e the n e c e s s a r y recognition in the long-range h e a l t h p r o g r a m m e for the R e g i o n and t h a t l e p r o s y w o u l d be a n early subject for the t e c h n i c a l d i s c u s s i o n s .

The S u b - C o m m i t t e e on Programme and B u d g e t (established in a c c o r d a n c e w i t h r e s o l u t i o n WP/RC7.R7 adopted by the C o m m i t t e e a t its seventh s e s s i o n ) h e l d two m e e t i n g s , during w h i c h it made a d e t a i l e d e x a m i n a t i o n of the p r o p o s e d p r o g r a m m e and budget estimates in the W e s t e r n Pacific R e g i o n . T h e d i s c u s s i o n w a s b r o k e n d o w n into: (l) review of the regular p r o g r a m m e and b u d g e t for 1962 in the l i g h t o f budgetary adjustments and amendments p r o p o s e d by M e m b e r countries; (2) p r o p o s e d programme and b u d g e t estimates for the 1 9 6 3 r e g u l a r p r o g r a m m e ; ⑶ M a l a r i a E r a d i - cation S p e c i a l A c c o u n t ; (4) s u p p l e m e n t a r y list of projects; (5) E x p a n d e d P r o g r a m m e of T e c h n i c a l A s s i s t a n c e ; and (6) S p e c i a l A c c o u n t f o r the C o m m u n i t y W a t e r S u p p l y .

The report of the S u b - C o m m i t t e e , w h i c h is contained in A n n e x ),was c o n s i d e r e d and accepted by the m a i n C o m m i t t e e . (See r e s o l u t i o n s W P/R C1 2 ,R 7 and W P / R C 1 2 . H 8 . )

T h e Committee considered the q u e s t i o n of p r i o r i t i e s in p r o g r a m m e in a c c o r d a n c e w i t h the request of the F o u r t e e n t h W o r l d H e a l t h A s s e m b l y . It also r e v i e w e d the p r i o r i t i e s established at its n i n t h s e s s i o n in c o n n e x i o n w i t h the p r o g r a m m e and

budget and decided that n o change w a s r e q u i r e d i n the p r e s e n t p r o c e d u r e b e i n g f o l l o w e d in the R e g i o n .

T h e Committee u n a n i m o u s l y adopted r e s o l u t i o n IAÍP/RC12.R1.

2 . M a l a r i a eradication programme (document W P / R C1 2/ 6 )

T h e Committee reviewed the report of the R e g i o n a l D i r e c t o r o n the m a l a r i a

eradication programme in the R e g i o n . I t n o t e d w i t h g r e a t interest t h a t t h e m a l a r i a eradication programme in C h i n a (Taiwan) h a d r e a c h e d the last stage o f the c o n s o l i d a t i o n phase and that e r a d i c a t i o n of the d i s e a s e w a s close a t h a n d , that m a l a r i a e r a d i c a t i o n programmes were continuing in the P h i l i p p i n e s and R y u k y u Islands and t h a t two n e w

countries h a d launched m a l a r i a e r a d i c a t i o n p r o g r a m m e s d u r i n g 1 9 6 1 , n a m e l y . N o r t h B o r n e o and S a r a w a k . T h e report showed that considerable p r o g r e s s w a s b e i n g m a d e i n o t h e r countries either in the planning of e r a d i c a t i o n or in the d e v e l o p m e n t of p r o j e c t s aiming in the n e a r future at the e s t a b l i s h m e n t o f m a l a r i a e r a d i c a t i o n p r o g r a m m e s .

T h e C o m m i t t e e n o t e d the s t e p s t a k e n b y the R e g i o n a l D i r e c t o r in o r d e r to ensure t h e i n t e n s i f i c a t i o n of a n t i m a l a r i a a c t i v i t i e s in b o r d e r areas of K a l i m a n t a n .

A r e s o l u t i o n w a s a d o p t e d c a l l i n g the a t t e n t i o n of all g o v e r n m e n t s operating or p l a n n i n g e r a d i c a t i o n p r o g r a m m e s to the i m p o r t a n c e of critical e v a l u a t i o n at a l l s t a g e s , t h e n e e d f o r a w e 1 1 - o r g a n i z e d and w e l l - s t a f f e d m a l a r i a service and for full a n d c o n t i n u i n g s u p p o r t f r o m the g o v e r n m e n t and the people u n t i l e r a d i c a t i o n is

a c h i e v e d . T h e r e s o l u t i o n also e m p h a s i z e d the d a n g e r of r e l a x a t i o n w h e n early success a p p e a r e d t o m a k e c o n t i n u e d e x p e n s i v e s u p p o r t u n n e c e s s a r y . T h e Committee noted that u n d e r the n e w country p r o g r a m m i n g p r o c e d u r e s of t h e E x p a n d e d Programme o f T e c h n i c a l A s s i s t a n c e , specific i n f o r m a t i o n w a s r e q u i r e d a s f a r as the content of the p r o j e c t p r o p o s a l s w a s c o n c e r n e d . It also n o t e d t h a t p r o j e c t s must be planned for t h e i r full d u r a t i o n and that although the T e c h n i c a l A s s i s t a n c e Committee w o u l d only give f i n a n c i a l a u t h o r i z a t i o n for f # u r y e a r s at a t i m e , infor- m a t i o n on the estimated d u r a t i o n m u s t be submitted at the time of the o r i g i n a l r e q u e s t . 2.1 • Panel presentation and discussion

The subject ”Dental Health" was discussed in three plenary sessions• In the first two sessions, the topic for the session was presented by a panel of three members, after which general discussion took p l a c e . The subject was considered under two main headings: (1) the significance and importance of dental disease w i t h special reference to the problems of the Western Pacific Region; and ⑵ the

organization and administration of a dental health service and the use of trained personnel including dental auxiliaries. A special field session was held during a visit to the fluoridation plant at Lower Hutt C i t y .

At the third and concluding session, views and opinions were consolidated and conclusions were d r a w n .

2.2 Conclusions

(1) An organized treatment service reduces the consequences of dental disease but does not reduce the magnitude of the public dental health programme•

(2) In the first instance, if at all possible in any developing country, the dental programme should be based on a number of fully•-trained graduate dentists.

(3) It was realized that in some countries dental auxiliaries are required. The type of dental auxiliary required varies from country to country and the type of dental health programme must be adapted to the local conditions of each country.

(4) Whe^i for economic and other reasons dental auxiliaries only are trained, provision should be made for these people to receive further training to become fully-qualified dentists.

(5) There is a need to strengthen existing national dental health services and to ensure that in each programme there is a proper balance between the provisions made for research, prevention, health education

and treatment. • (6) There is a need to strengthen existing training programmes for

dentists, assistant dental practitioners and dental auxiliaries.

(7) T h e r e is a n e e d for M e m b e r countries to make a more thorough survey of the size and severity of national dental problems, to assess their s e r i o u s n e s s relative to other health problems and to determine the causes for d i f f e r e n c e s m a n i f e s t e d between and within different ethnic or

g e o g r a p h i c g r o u p s of p e o p l e .

(8) T h e r e is an u r g e n t n e e d for the interchange of information and e x p e r i e n c e b e t w e e n M e m b e r countries and this could be greatly facilitated b y the a p p o i n t m e n t of a d e n t a l consultant to the Regional O f f i c e . This c o n s u l t a n t should be a d e n t a l epidemiologist.

2 . 5 E v a l u a t i o n

O f the completed q u e s t i o n n a i r e s r e c e i v e d , all 3 5 assessed the choice of the t o p i c " D e n t a l Health" as t i m e l y ; 21 rated the Technical Discussions as excellent and 1 1 as r e a s o n a b l y g o o d ; 20 said they had gained new ideas in concept, and 13 had g a i n e d s o m e ,

REPORT OF THE SUB-COMMITTEE ON PROGRAMME AND BUDGET 1. INTRODUCTION

The Sub-Committee on Programme and B u d g e t , composed of representatives from

Cambodia, China, J a p a n , Malaya, New Zealand, Philippines, United Kingdom and V i e t N a m , held two meetings on 31 August and 4 September 1961 under the chairmanship of

2 . DISCUSSION

2.1 Review of the regular programme and budget for 1962 in the light of present budgetary adjustments and consideration of amendments proposed by Member countries (document WP/RCI2/))

The Sub-Committee noted that the modifications made were due to the following factors: the decision of the World Health Assembly to transfer the administrative and operational costs of the Malaria Eradication Special Account to the regular budget as from January 1961, which would mean an increase of nine posts including one inter- national post; the decision of the Director-General to close the Singapore Epidemio- logical Intelligence Station as of January 1962, which would result in the abolishment of six local posts; the need to provide for an additional medical officer in 1961,

which would be a continuing charge• These modifications accounted for an additional requirement of $ 17 9 2 4 . The increased provision for duty travel related to the duty travel costs for staff transferred from the Malaria Eradication Special Account and did not indicate an increase in duty travel for the Regional Office s t a f f .

A s far as field activities were concerned, there were no significant changes in the staffing pattern other than the addition of a secretary for a regional adviser and two clerk-typists in the A r e a RepresentativeT s office in Singapore resulting in an increase of approximately $ 2 0 0 0 .

The modifications made to the country programmes were the result of discussions and agreements between the Organization and Member governments, the advance award of some fellowships to 1961, revised requirements in connexion with the contributions to the U n i t e d Nations Pension Fund and, finally^ changes in the average costing for short- term consultants•

T h e Representative of the Netherlands referred to the deletion of the dental h e a l t h fellowship for Netherlands New Guinea. Assistance in the field of dental h e a l t h was now being provided by the Netherlands Government in the form of fellowships at the D e n t a l Training S c h o o l , S u v a , Fiji• WHO assistance was not therefore now required for dental health education.

A question was raised as to the extent the services provided by the Singapore Epidemiological Intelligence Station would be affected by the Director-Generalfs d e c i s i o n to curtail its operation.

T h e Assistant Director-General assured the Sub-Committee that with the new arrangement there would be no reduction in the functions or responsibilities under the International Sanitary R e g u l a t i o n s .

D e c i s i o n : T h e Sub-Committee agreed to recommend to the plenary session that these modifications should be incorporated into the 1962 programme, and that the R e g i o n a l Director should be requested to implement the pro- gramme as a m e n d e d .

2,2 Proposed programme and budget estimates for 1963 regular programme

The Regional Office (pages 3-8)

There was no change in the number of posts proposed in the Regional O f f i c e , which remained at 5 8 .

Duty travel costs remained the same as in previous years; h o w e v e r , home travel costs showed a slight increase as a result of the home leave schedule of the inter- national staff• It might, in future, be necessary to increase the item "allowances"

as a result of future changes in the cost of living in Manila• T h i s , h o w e v e r , could not be foreseen at the time of the presentation of the document and the same level as in previous years h a d , therefore, been maintained.

Attention was drawn to a change in the title of some staff members in the Regional Office. Formerly the Director of the Bureau of Health Services was assisted by three public health administrators. Recently, there had been many questions regarding the nomenclature of the posts, and it had been thought more realistic to give this post a functional title rather than something v a g u e . The Director-General had approved the suggestion that there should be two or three

assistants to the Director of Health Services and, as the public health administrators actually served in this capacity, it seemed more appropriate to call them w h a t they actually w e r e . A change had therefore been made in the functional titles of the staff members concerned, The three public health administrators had thus been replaced by two assistant directors of health services and one regional adviser on public health administration (the latter appearing under "Regional Advisers") to handle all matters pertaining to public health administration.

Regional Advisers (pages 9-12)

The post of regional adviser in health education had been suppressed in 1959•

The Regional Director now proposed to reinstall this in 1963. In addition, an

increase in the number of сlerk-stenographers and clerk-typists was proposed in order to deal with the ever-growing volume of work resulting from the wider distribution of technical and information material to Member governments. Duty travel estimates would be increased by $ 2000. There w o u l d , on the other h a n d , be a slight decrease

in home leave requirements. The total requirements, therefore, represented merely a slight increase over that of 1962.

Area Representatives (pages 15-l6)

No change was proposed in the staffing pattern. Duty travel expenses would be maintained at the same level as in 1961 and 1 9 6 2 .

Country projects (pages 17-116)

Note was taken of a working document WP/rc 12/p&B/2 (see Annex 1) which contained an analysis of field activities for 1963. As in previous y e a r s , the field of public health administration had the largest share of funds, with a percentage of Jl.75^

followed by nursing 11.55 per cent., tuberculosis 9*56 per cent., maternal and child health 6.6 per cent. and environmental sanitation 5 - 9 per cent.

Inter-country programmes (pages 118-133)

In addition to such inter-country projects as the yaws and tuberculosis teams which would visit various countries, the following inter-country seminars or

training courses were proposed by the Regional Director: (l) seminar on immunization in the control of communicable diseases, (2) seminar on the role of the hospital in the public health programme^ and (5) training course on epidemiology and bacteriology of enteric infections.

D e c i s i o n : The sub-Committee decided to rocommend to the main Cominittee the approval of the programme and budget for 1963 as proposed by the R e g i o n a l D i r e c t o r .

2.5 M a l a r i a Eradication Special Account (document WP/RC12/4)

T h e Sub-Committee was informed that the Malaria Eradication Special Account (MESA) had- been established to supplement the malaria eradication programmes already included in the regular and Technical Assistance budgets. The total shown under MESA for malaria eradication projects in I963 was $ 6l8 896; an amount of $ 49 795 w a s allocated under the regular budget, and $ 1斗9 701 under the Technical Assistance budget•

T h e M a l a r i a Eradication Special Account was a voluntary fund and the implemen- tation of the proposed programmes depended upon the availability of funds; also, further agreement had to be reached between the governments concerned and the Organization before these programmes could be implemented. Modifications could, therefore, be expected as these programmes developed in the following years•

The Sub-Committee noted that a correction had to be made to the text covering the Sarawak malaria eradication project. The phrase "malaria programme until i960"

The attention of the Sub-Committee was drawn to the fact that the Organization w a s not in a position to allocate funds for a project unless a plan of operation had been agreed upon and signed by the government concerned and the Organization. The 1963 programme w a s , therefore, a tentative one and only when the criteria established by the Organization had been agreed on by the government would a definite allocation of funds be m a d e .

The Assembly resolution covering the gradual transfer of malaria programmes to the regular programme was discussed in some detail. The Assembly had proposed that in 1962 the Director-General would add into the regular budget the amount of

$ 2 ООО 000 for activities under M E S A , which meant that to the extent of

$ 2 ООО 000 these activities would be implemented in 1962 by funds made available from the regular budget of the Organization. For the 1963 programme, the Assembly

had agreed that the amount of $ 4 ООО 000 should be included in the regular budget of WHO for the same purpose. The entire expenditure under the Malaria Eradication

Special Account, which might exceed $ 4 000 000 would be incorporated into the regular budget from 196斗 onwards. As could be seen, therefore> there were three stages. The whole malaria eradication programme in the Special Account could not be transferred in one or two years. It w a s , therefore, necessary to continue to show in 1963 the expenditure under the Malaria Eradication Special Account^ instead of in the regular budget.

Decision: The Sub-Committee decided to recommend approval of the proposed programme and budget under the Malaria Eradication Special Account.

2.4 Supplementary list of projects (document WP/RC12/4

: pages 170-194)

The Sub-Committee was informed that requests for assistance from Member govern- ments usually exceeded the funds available and it was accordingly necessary to relegate some of these requests to the supplementary list for implementation in the future should savings become available. The list before the Sub-Committee consisted of requests made by Member governments in 196l, 1962 and 1963. Those projects w h i c h would be undertaken in 1961 and 1962 had been cleared by the respective governments before the programme and budget document had been prepared, and 1963 projects were those included in the original requests which could not be accommodated in the regular budget.

The Sub-Committee noted that two additional requests were to be added to the list (l) in connexion with an environmental sanitation project in China which would cost

$ 29 537 in 1962 arid $ 13 757 in 1963; (2) a fellowship in 1962 to permit a medical officer in Malaya to visit Indonesia, Taiwan and the Philippines for seven weeks to observe the treponematoses control programmes in those countries - this would cost

$ 900.

Decision; The Sub-Committee decided to report to the plenary session that it was satisfied with the supplementary list of projects as listed and amended.

2.5 Expanded Programme of Technical Assistance (document WP/RC.12/4: pages 1-1J2;

and Annex A, pages 1б2-1б9)

The Sub-Committee noted that the proposed programme under Technical Assistance for 1963 was based on information received from Member governments and was at this stage highly tentative. At the present time no country target figures had been established and the final amounts governments would receive for their health activities could not be forecast with any degree of precision. Requests under the Technical Assistance programme and budget were made by Member governments direct to the Technical Assistance Board, except in the case of inter-country programmes.
