Objectifs Pédagogiques
• Connaitre les nouvelles molécules de traitement de l'hépatite C avec ou sans Interféron
• Connaître les indications des nouveaux traitements en 2015 et leur coût
• Connaître les effets indésirables et les précautions d'emploi des
nouveaux antiviraux

• Homme de 53 ans
• Mère : Maladie de Willebrand, VHC +, cirrhotique avec vascularite sévère (Rituximab) guérie par boceprevir + P/R au prix d’une mauvaise tolérance
• A reçu Cryo précipité pour chirurgie orthopédique (fracture du fémur, 1986)
• Anti VHC + en 1998, Génotype 3a
• Syndrome dépressif, sous Séroplex depuis 2012
Observation 1
• Personnel navigant Air France. Longs courriers.
• Surveillance régulière :
2010 : Fibrotest 0,50 (F2) 2011 : Fibrotest 0,51 (F2) 2012 : Fibroscan 7,7 kPa 2013 : Fibroscan 8,2 kPa
• Refus de traitement pour raisons professionnelles/
craintes +++

• Affolé par un Fibrotest à 0,73 (F3/F4) (septembre 2014)
• A entendu parler de nouveautés thérapeutiques Réévaluation :
• 90 kg (IMC 27) / 50g d’alcool

• ALAT = 1,5 N GGT = 3,5 N
• Plaquettes: 253 000 TP 94% Albumine 39 g/L
• Echo : foie homogène, de contours réguliers; pas de stéatose
Devant son angoisse, réévaluation de la fibrose :
• Fibrotest 0,63 soit équivalent Metavir F3
• Fibroscan 8,9 kPa (IQR 1,9 kPa Tx réussite 91%) soit équivalent Metavir F2
