RECOMMANDATIONS DE LA SOCIETE MAROCAINE D’INFECTIOLOGIE PEDIATRIQUE ET DE VACCINOLOGIE (SOMIPEV) (12)
Recommandations en cas de fièvre isolée
* Ne pas prescrire d’antibiotique pour une fièvre nue sans point d’appel clinique en l’absence de détresse vitale : un antibiotique n’est ni un antipyrétique (pour le malade), ni un anxiolytique (pour les parents ou le médecin) !
* Se donner les moyens de trouver la cause de la fièvre : hémocultures, cytobactériologie si la bandelette urinaire est positive, prélèvements locaux selon les points d’appel.
Critères de bon usage des antibiotiques
₋ Réévaluation de l’antibiothérapie (24ème et 72ème heure) notée dans le dossier
₋ Poursuite de l’antibiotique au-delà de 3-4 jours après confirmation par un médecin sénior ₋ En cas d’association d’antibiotiques, justifier dans le dossier son maintien au-delà de 3 jours ₋ Durée prévisionnelle de traitement notée dans le dossier
₋ Antibiothérapie adaptée aux données microbiologiques permettant le plus souvent une désescalade (sinon justifier)
