50-year-old healthy man presents to a fam- ily medicine clinic and mentions that he has never before had his eyes checked. He has no ocular complaints. His visual acuity is 20/20 bilaterally, with an intraocular pressure of 14 mm Hg (normal range 10 to 21 mm Hg).Cornea, pupil, and lens examination results are normal. However, fundus examination of the left eye reveals a feathery, superficial, white lesion extending from the disc margin. The macula is unchanged.
This most likely diagnosis is
1. Primary open-angle glaucoma 2. Optic nerve coloboma
3. Myelinated retinal nerve fibre layer 4. Optic nerve hypoplasia
