- Amélioration significative du BPH Impact index à 4 et 12 semaines pour le Tadalafil de 0,8 et 0,6 pts et la Tamsulosine de 0,9 et 0,6 pts respectivement
- Amélioration des scores de qualité de vie de l’IPSS et du TSS-BPH avec le Tadalafil mais pas avec la Tamsulosine.
- Amélioration du sous score DE de l’IIEF5 avec le Tadalafil (4 pts ; p=0,022) mais pas avec la Tamsulosine (p=0,546).
- Amélioration du Q max avec le Tadalafil (2,4 ml/s ; p=0,009) et la Tamsulosine (2,2 ml/s ; p=0,014).

Profil d’effets secondaires similaires aux études antérieures.Enseignements pour la pratique clinique - Le Tadalafil en prise quotidienne est plus efficace sur les SBAU secondaires à l’HBP que le placebo.
Son efficacité est similaire à celle de la Tamsulosine même si cette étude ne permet pas de comparer directement l’efficacité du Tadalafil et de la Tamsulosine.
- Chez les patients ayant à la fois une dysfonction érectile et des SBAU en rapport avec une HBP, le Tadalafil 5mg en prise quotidienne est une option thérapeutique à considérer.

There were no significant differences between the active treatment groups and placebo for any TEAE or in the incidence of serious AEs or discontinuations because of AEs (Table 5). The most common TEAEs with tadalafil were headache (n= 5) followed by nasopharyngitis (n= 5), back pain (n= 4), dizziness (n= 4), and dyspepsia (n= 4), while with tamsulosin the most common events were headache (n= 7) and dizziness (n= 6). One subject in the tamsulosin group reported two TEAEs related to ejaculatory dysfunc- tion (retrograde ejaculation and semen volume decreased).
There were no clinically significant changes in laboratory measurements or vital signs. For PVR, mean reductions from
baseline compared with placebo were observed with both active treatments but were not statistically significant (Table 4).
4. Discussion
In this, the first international, placebo-controlled study evaluating tadalafil or tamsulosin (as an active control) for LUTS/BPH, tadalafil 5 mg once daily for 12 wk resulted in significant and clinically meaningful improvements in LUTS/BPH similar to tamsulosin 0.4 mg once daily. Based on analyses of secondary end points, this was also the first tadalafil study to show significant improvement in LUTS/
BPH after 1 wk and a significant increase in Qmaxat 12 wk.

Commentaires du CTMH
Il s’agit de la première étude internationale prospective randomisée contre placebo, évaluant
l’efficacité du Tadalafil et de la Tamsulosine sur les SBAU associés à une HBP. Le Tadalafil 5 mg en prise quotidienne apportait une amélioration significative des SBAU comparable à la Tamsulosine LP 0,4 mg même si l’effectif des patients recrutés était insuffisant pour qu’une comparaison directe puisse être réalisée.
De plus, cette étude retrouvait une efficacité du Tadalafil dès la première semaine de traitement et une amélioration du Q max à 3 mois ce qui reste controversé. En effet, la majorité des études antérieures sur les IPDE5 ne rapportaient pas d’effet supérieur à celui du placebo sur le Q max. En revanche dans l’étude de recherche de dose l’amélioration du Qmax était dose dépendante, (Tadalafil 5 mg > Tadalafil 2,5 mg) alors que dans l’étude d’Egerdie et al. seul le Tadalafil à la dose de 2,5 mg a atteint le seuil de significativité statistique pour prouver une amélioration du Qmax.
Plusieurs mécanismes d’action ont été proposés pour expliquer l’efficacité des IPDE5 dans le
traitement des SBAU : diminution du tonus musculaire lisse prostatique entrainant une diminution de l’OSV, amélioration de la perfusion tissulaire ou diminution de l’activité nerveuse afférente de la vessie modulant le reflexe mictionnel.
D’autres études à long terme sont nécessaires pour évaluer l’impact des IPDE5 sur la prévention de la rétention aigue d’urine et le recours à la chirurgie.
Les données cliniques disponibles évaluant l’efficacité des inhibiteurs de la phosphodiesterase 5 et en particulier le Tadalafil sur les SBAU liés à l’HBP sont convaincantes. C’est pourquoi le CTMH
recommande l’utilisation du Tadalafil 5 mg en prise quotidienne dans cette pathologie et ce d’autant plus que les patients souffrent à la fois de SBAU et de DE, 2 affections fréquemment associées.
