Bipolar
multielectrodes RFA : the good alternative option for non resectable
patients with
large single HCC ?
Our choice : multipolar RFA
Ablation planning according to tumor size
3 cm as maximal distance between 2 adjacent electrodes
5 cm diameter 6 cm diameter
Arguments :
-Large ablation per apply (≈ 9 cm) -Very predictable (free margin)
-Quite insensitive to heat sink effect
Curative attempt in
quite safe conditions
Intra tumoral RFA
de l’ablation intratumorale au no touch
Ablation margins Quite predicable Poorly predicable
Spread of heating Centripetal Centrifugal
No touch RFA
From single to multi applicators and multipolar devices
Is there something to protect?
Yes : the diaphragm by artificial ascitis
Frontal view
More than 1X1 cm in

Key points
In elderly fragile patient with large solitary HCC endoarterial is a
rational first therapeutic approach in attempt to debulk the tumor and have a test of time of evolution of tumor disease. In case of good
response MP RFA as second
therapeutic step can complete the
results in safety conditions.

Multipolar RFA for infiltrative segmental HCC
• Often 6 electrodes to cover in a single apply the
maximum volume of the segment involved by the tumor
• 3 cm interelectrodes distance
• Try to insert electrode along the long axis of segment (axis of the segmental portal vein)
• Pull back technique (from the hilum side toward the peripheral subcapsular plan) and electrode repositioning if necessary
