DR HUSSEIN A. GEZAIRY REGIONAL DIRECTOR

WHO EASTERN MEDITERRANEAN REGION to the

23rd MIDDLE EAST REGIONAL CONGRESS OF THE INTERNATIONAL UNION AGAINST TUBERCULOSIS AND LUNG DISEASE (IUATLD) JOINTLY HELD WITH THE 13th ANNUAL NATIONAL CONFERENCE

Your Excellency the President of the Islamic Republic of Iran,

Excellencies, Honourable Guests, Ladies and Gentlemen, Dear Colleagues,

It gives me great pleasure to attend the 23rd Middle East Regional Congress of the International Union Against Tuberculosis and Lung Disease which is being jointly held with the 13th Annual National Conference in Teheran, Islamic Republic of Iran.

I wish to express my sincere appreciation to His Excellency Hojatolislam Akbar Hashemi Rafsanjani, President of the Islamic Republic of Iran, for his kind support to this Congress, which is a strong indication of his appreciation of the central role of health in the process of development. I wish also to extend my gratitude to the President of the Congress, His Excellency Dr Ali Akbar Velayati, and to His Excellency the Minister of Health and Medical Education, Dr Alireza Marandi. It goes without saying that I am delighted to be present here today in the company of such a distinguished scientific community.

Ladies and Gentlemen,

The burden of lung disease has been rapidly increasing at global, regional and national levels. As you are all aware, tuberculosis is the leading infectious killer of youth and adults. Acute respiratory infection is the leading infectious killer of children. Asthma is considered as a problem of public health concern. In addition, tobacco-related lung diseases are a growing health problem.

In view of such a serious global situation the World Health Organization has given priority to the promotion of lung health and has taken the initiative in conducting various activities in this field, in collaboration with national authorities and various agencies interested and working in these fields.

For many years now, the International Union Against Tuberculosis and Lung Diseases and WHO have worked closely in many fields especially in tuberculosis control. Recently, WHO and the International Union Against Tuberculosis have been promoting the use of the effective tuberculosis control strategy widely known as directly observed treatment, short-course, or DOTS. The two organizations are also joining hands in human resources development. Indeed, the Regional Training Course on Tuberculosis Control, which starts in 5 days time here in Teheran, has been jointly organized by the Union, together with WHO and the Iranian national authorities.

Dear Colleagues,

The Middle East Regional Congress of the International Union Against Tuberculosis and Lung Disease has always been instrumental in the promotion of lung health in this Region, and we have been pleased to collaborate in this activity.

In addition to reviewing the latest technical developments in the field of lung health, the Congress is covering a number of important issues and it is hoped that it will provide appropriate technical guidance to the participants. It will also promote collaboration between the national and international agencies in the many aspects of

lung health, including tuberculosis, smoking and lung disease, acute respiratory infections and asthma.

We are specifically concerned about tuberculosis which poses an emergency at both global and regional level. If appropriate action is not taken immediately, 4 million people can be expected to die from tuberculosis in the next 10 years in this Region alone. As DOTS is the only strategy which can reverse the rising trend of the global tuberculosis epidemic, I am pleased to note that the Congress is giving an opportunity for all participants to understand the DOTS strategy and to encourage them to implement it in their own countries.

One of the emerging problems in tuberculosis control is the appearance of multidrug-resistant tubercule bacilli and I am pleased to note that the Congress is giving it the necessary attention.

Proper diagnosis is an essential tool in control. There are some new techniques developed in this regard and I am confident that the Congress will provide all participants with clear ideas about the advantages, disadvantages and limitations of these latest techniques in the diagnosis of tuberculosis.

Dear Colleagues,

No one can speak of lung diseases without mentioning tobacco. It is noted with regret that, during the last 30 years, tobacco consumption has been on the increase and what makes this an important problem is the fact that most of the illnesses caused by tobacco use, such as cardiovascular diseases, lung cancers and chronic obstructive lung disease are incurable when the diagnosis is made. I sincerely hope that this Congress will boost efforts for the formulation, promotion and implementation of comprehensive policies and programmes for tobacco control in this Region and clarify the epidemiology and management of important diseases related to smoking, particularly cancer of the lung.

I also hope that the leading infectious killer of children under five years of age, namely acute respiratory infections, will receive the attention it deserves. The WHO programme emphasizes the need to ensure that children with pneumonia have access to treatment by well trained personnel and that irrational use of antibiotics is reduced.

As I have indicated, asthma is now considered to be a problem of major public health concern in the Region. The prevalence of asthma is increasing in the Region due to the very severe air pollution we are experiencing, particularly in the big cities.

I am also pleased to note that the Congress will provide an update on the clinical knowledge about asthma and skills such as diagnosis, treatment and monitoring, and will also enlighten the participants on the public health aspects of this disease.

Ladies and Gentlemen,

I am aware that there are many other important topics that will be discussed in the Congress which will provide not only a forum for discussing and updating our knowledge about lung health and the latest skills and techniques concerned with preserving lung health, but will also be an opportunity to exchange experiences in the promotion of lung health by the participants in their own fields and countries.

I wish to extend once again my thanks and appreciation to the Government of the Islamic Republic of Iran for hosting this Congress and for facilitating its activities, and for the kind invitation extended to me to address this distinguished scientific forum. I pray that the Congress will be successful and beneficial to all countries in the Region.
