Conclusion:

Richter syndrome is a rare transformation of chronic lymphocytic leukemia into aggressive lymphoma. It occurs in 2% to 10% of cases. Most commonly, Richter syndromes harbor features of diffuse large B-cell lymphoma but cases of Hodgkin lymphomas or

prolymphocytic lymphomas have been described. Only few cases of plasmablastic transformation with established clonal link have been reported. Here, we could establish the diagnosis of high grade plasmablastic transformation given the clonal link made with CLL.

Flow cytometry analysis revealed an atypical population of monotypic cells expressing weak CD45, HLADR-, Kappa+, CD19-, CD20-, CD117+, CD38+ and CD138+.

Restrict pattern analysis showed a clonal lymphoid population which harbors the same clonal pattern at 2 different steps of the disease. At diagnosis of plasmablastic transformation, bone marrow aspiration

smear was coagulated but showed atypical plasmacytoïd large cells (May-Grunwald-Giemsa x100).

Plasmablastic transformation showed neoplastic cells expressing strong CD138+, MUM1+, CD79a+/- and Ig Kappa+. Immunoblastic large cells

were CD20-, PAX5-, EBV- and HHV8-.
