9.9. Traitement des formes symptomatiques et inopérables :
Aspergillome pulmonaire

Durant la période de notre étude, l’intervention a été récusée chez 5 patients (exclus de notre étude) pour une altération profonde de l’état général (2 cas), une lésion bilatérale très évoluée (1 cas), une comorbidité (1 cas) et un seul patient avait refusé l’intervention chirurgicale.
Compte tenu de ces limitations, des traitements moins agressifs se sont développés, à visée palliative essentiellement, pour les patients présentant une contre-indication chirurgicale ou afin de temporiser avant une éventuelle solution chirurgicale.[106]
L’embolisation des artères bronchiques représente une alternative intéressante à la chirurgie en contexte d’hémoptysie menaçante, avec une efficacité de 90 à 95 % sur le saignement [93] . Les complications sont rares : 1 % environ de complications majeures (hémiplégies, paraplégies), 20 % de complications mineures réversibles.[106]
Le drainage endocavitaire selon Monaldi et l’injection d’amphotéricine B [94, 95, 96] avec ses inconvénients représentés par :
La difficulté de mise en place du drain, en rapport avec la petite taille de la cavité et sa topographie ou sa proximité du hile,

Le bronchospasme voire même l’inondation trachéo-bronchique lors de l’injection de l’amphotéricine B surtout en cas de communication avec l’arbre bronchique,
Le pneumothorax peu fréquent à cause des adhérences déjà constituées
Les lavages à l'Amphotéricine après mise en place d'un drain endocavitaire. [97]
Le remplissage de la cavité aspergillaire et l'injection de pâte Amphotéricine B par voie percutanée à l'aiguille fine : Cette technique préconisée par Krakowka il y 25 ans, se réalise en ambulatoire et est optimisée par le repérage tomodensitométrique. Krakowka rapporte des effets indésirables [98] :
Hémoptysies liées au geste, en général de faible importance, dans 50 % de ces cas,
Un reflux de pâte dans la trachée sans manifestation clinique importante (toux),
La pâte peut être déglutie et retrouvée dans l'œsophage après des quintes de toux,
Une pneumopathie secondaire abcédée qui semble lié à un reflux endobronchique,
Un épisode de collapsus passager (rattaché à une sensibilisation par passage endovasculaire d'Amphotéricine B). [98]

L’instillation d’antifongiques par voie percutanée ou endobronchique (amphotéricine B) a aussi été proposée : le taux de réponse radiologique est de l’ordre de 50 %. Ce traitement permettrait de raccourcir la durée d’évolution de la maladie [99]. La tolérance est bonne et les effets secondaires rares (fièvre à l’injection de l’amphotéricine B). Dans les quelques séries ayant évaluées l’itraconazole, un taux de réponse d’environs 50 % est observé et peu d’effets secondaires sont rapportés. [100]
Toutefois, aucune de ces techniques n’a fait l’objet d’une étude comparative prospective; leur efficacité reste à démontrer.

Notre étude indique que le taux de mortalité et de morbidité après le traitement chirurgical de l’aspergillome pulmonaire est acceptable. La chirurgie est efficace pour éviter la réapparition des hémoptysies.
Le Maroc est un pays où la tuberculose sévit à l’état endémique, il faut toujours penser à une greffe aspergillaire devant un ancien tuberculeux qui présente des hémoptysies.
Bien que, plus des études sont nécessaires, nos données appuient que la résection chirurgicale doit être envisagée chez les patients symptomatiques et asymptomatiques.

Le but de cette étude rétrospective était d’analyser les résultats du traitement chirurgical chez 53 patients opérés pour aspergillome pulmonaire .
Patients et méthode :
De 2002 à 2009, 58 patients ont été pris en charge dans le service de chirurgie thoracique à l’hôpital militaire d’instruction mohammed V pour aspergillome pulmonaire; 53 patients ont été traités chirurgicalement, 5 patients ont été jugés inopérables à cause de l’altération de leur état général et/ou de la comorbidité et/ou de la bilatéralité des lésions pulmonaires. L’hémoptysie était le symptôme le plus souvent observé (dans 75.47 % des cas).
L’exérèse chirurgicale a été réalisée par principe, avec 19 résections lobaires, 13 segmentaires, 11 wedge resection, 6 pleuro-pneumonectomies, 2 truffectomie + myoplastie, 1 cavernostomie, et une thoracoplastie .
Résultats :
les complications postopératoires ont été survenues dans 26.4 % des cas, sous forme de : hémothorax (2 cas), fuite d’air prolongée (6 cas), défaut de réhabitation (2 cas), empyème (1 cas), fistule broncho-pulmonaire (1 cas), infection de la paroi (1 cas) et de la détresse réspiratoire (1cas). Une réintervention a été nécessaire chez 2 patients. Aucun décés postopératoire n’a été noté .
Conclusion :
Le traitement chirurgical de l’aspergillome pulmonaire malgré son importante morbidité, doit être proposé systématiquement même chez les sujets asymptomatiques, quand il n’y a pas de contre-indication opératoire.

The aim of this retrospective study was to analyze the results of surgical treatment in 53 patients operated on for pulmonary aspergilloma.
Patients and methods:
From 2001 to 2009, 58 patients were treated for pulmonary aspergilloma at the
military hospital instruction Mohammed V ; 53 were operated on and surgery was contraindicated in the other patients with general failure and/or comorbidity and/or bilaterality of lung injury. Hemoptysis was the main symptom, present in 40 patients (75.47% ). Surgery was performed on principle with 19 lobectomies, 13 segmentectomies, 11 wedge resection, 6 pleuro-pneumonectomies, 2 truffectomies + myoplasties , 1 cavernostomy and 1 thoracoplasty.
Results:
Postoperative complications occurred in 26.4 % of the patients including : hemothorax (n=2), prolonged air leak (n=6), rehabitation defects (n=2 ), empyema (n=1), bronchopulmonary fistula (n=1), infection wall (n=1) and respiratory failure (n=1). Reoperation was necessary in 2 patients. postoperative mortality rate was 0%.
Conclusion:
The surgical treatment, in spite of its high morbidity, has to be proposed to all patients with pulmonary aspergilloma, even in asymptomatic patients when
there is no surgical contraindication. Pleuropneumonectomy is a very high risk procedure and its indications must be restricted.
