5. Perspectives d’applications futures en radiothérapie
Les statines sont très utilisées en pratique courante en cardiologie, indication systématique après un infarctus sans avoir recours à des tests biologiques pour confirmer la dyslipidémie (Varela, Pereira et al. β008). Récemment un rôle antifibrosant confirmé (modèle animal) pour cette famille médicamenteuse a ouvert une indication dans l’hépatite C chronique (Vilahur, Casani et al. β014). Une étude plus large de la fibrose musculaire a confirmé sur un modèle animal le rôle important des statines (Simon, King et al. β014).

Cependant peu ou pas d’étude de l’effet des statines sur la radiosensibilité intrinsèque des cellules humaines ont été publiées. En revanche le laboratoire d’accueil possède une longue expérience dans ce type d’étude radiobiologique, notamment concernant la radiosensibilité de lignées cellulaires issues des tissus pulmonaire, cérébral, mammaire et cutané. De plus, la plus large des études de radiosensibilité d’individus traités par radiothérapie et leur classement en catégories de radiosensibilité a été réalisé dans ce laboratoire (voir Figure 16 § A. β.4.β.1). Ce large panel de caractérisation radiobiologique des différents tissus (surtout par les fibroblastes) a permis à notre équipe de mieux connaitre les mécanismes de la radiosensibilité. L’une des principales conclusions est que les statines et les bisphosphonates ont un effet radioprotecteur sur des fibroblastes humains en général et en particulier sur les cellules HIF gastro-intestinales étudiées dans cette thèse. Ainsi, nous avons confirmé sur un modèle, certes restreint, de lignées cellulaires humaines la réalité de l’effet des statines comme « radio protecteur » ou « réparateur » des CDB de l’ADN endommagé d’un tissu sain irradié (HIF rectal). Les statines réduisent de moitié le nombre des cassures non réparées car mal reconnues.
Cependant l’effet des statines sur le tissu tumoral irradié reste méconnu à ce jour et il serait souhaitable que leur utilisation ne radio-protège pas les tumeurs. Cet aspect reste donc à étudier.
En revanche un certain nombre de recommandations pour la pratique peuvent déjà être proposées :
1) la poursuite de l’usage des statines pendant une radiothérapie n’est pas susceptible de modifier la tolérance au traitement d’un individu de radiosensibilité normale mais peut réduire la sensibilité d’un individu hypersensible du groupe II et réduire ses risques d’effets secondaires à long terme. On n’a donc pas de raison de faire arrêter au patient d’éventuels traitements par statines tant qu’un impact délétère sur la sensibilité tumorale n’aura pas été mis en évidence.
β) si une situation de radiosensibilité est suspectée et a fortiori documentée comme étant du groupe II, un teste complémentaire in-vitro par le ZOPRA pourrait être proposé afin d’offrir à ce patient une alternative conservative plutôt que la contre-indication à
la radiothérapie. Cette alternative pourrait être de réaliser la radiothérapie prévue sous couverture d’un traitement type ZOPRA à doses aussi élevées que possible.
Par ailleurs cette possibilité d’intervention sur la radiosensibilité humaine dans le cadre de traitement par rayonnements ionisants ouvre des perspectives particulièrement intéressantes :
1) sur le plan pharmacologique il serait intéressant de développer des molécules optimisées pour cet effet sur ATM ;
β) sur le plan radioprotection opérationnelle il serait intéressant d’étudier si cette approche pharmacologique pourrait aussi renforcer la radiorésistance « normale » d’un individu et ainsi par exemple limiter l’impact d’expositions accidentelles ou concertée en radioprotection, sans parler d’une radioprotection de complémentaire pour les individus sensibles (le mode d’identification et de caractérisation de cette sensibilité étant évidemment à discuter par ailleurs) travaillant dans un contexte d’exposition aux rayonnements plutôt que de prôner leur éviction, ce qui serait une mesure discriminatoire.
γ) Enfin cette capacité d’intervention renouvelle et justifie l’intérêt pour le développement de tests simples économiques et rapides de caractérisation de la radiosensibilité individuelle humaine, puisqu’il y aurait maintenant quelque chose à y gagner aussi bien dans le domaine de la radioprotection individuelle ou collective que dans celui de la prévention des effets secondaires des radiothérapies.
