2. Complications de l’arthrite septique Du genou
Les complications de l’arthrite septique sont fréquentes. Elles sont dues le plus souvent au retard diagnostique et/ou à un traitement mal adapté. Parmi ces complications on retrouve : [ 81]
Les complications aigues :
Septicémies
Choc septique
Autres localisations infectieuses (endocardite, spondylodiscite…). Les complications chroniques :
La raideur ou l’ankylose articulaire.
L’algodystrophie. Le décès (8%).
La mortalité des arthrites septiques reste élevée : près de 8 % en moyenne dans les séries de la littérature [4]. Dans deux études de communauté, elle est de 11,5% dans l’une [41] et de 13% des adultes dans l’autre [53].

L'arthrite septique du genou est une urgence thérapeutique et tout retard diagnostic grève lourdement l’avenir du malade tant sur le plan fonctionnel que vital.
La ponction du liquide articulaire est l’examen clé pour le diagnostic positif, elle doit être faite avant toute antibiothérapie
D’une part, l’analyse cytologique du liquide articulaire est riche d’apport. Un aspect macroscopique purulent et une cellularité >100 000/mm³ oriente d’emblée vers le diagnostic de l’arthrite septique du genou. Sur le plan bactériologique, l’examen direct est très utile lorsqu’il est positif, il permet d’orienter le clinicien en situation d’urgence, cependant il est très peu sensible comparé à la culture, cette dernière reste l’examen de choix. D’autres marqueurs biologiques, qui sont plus sensible et plus spécifiques sont actuellement utilisés pour le diagnostic de l’arthrite septique comme la PCR ou le dosage des marqueurs immunologiques (TNF-α et IL-6).
Cependant, vu le risque de faux positifs, l’existence d’un germe dans le liquide articulaire n’est pas synonyme d’une arthrite septique, il faut toujours se méfier des contaminations.
D’autre part, l’absence de germe dans le liquide articulaire ne peut pas exclure le diagnostic de l’arthrite septique, en effet 20% des arthrites septiques sont à bactériologie négative. Cette situation pose un véritable problème de diagnostic pour le clinicien.
La prise en charge de l’arthrite septique est multidisciplinaire. Elle repose essentiellement sur l’antibiothérapie et le drainage articulaire. L’antibiothérapie probabiliste doit être instaurée en urgence, après réalisation des prélèvements

biologiques. Elle doit tenir compte du terrain du malade et de l’émergence de souches de bactéries résistantes.
Malgré les progrès réalisés dans l’arsenal thérapeutique, le pronostic des arthrites septiques reste réservé, avec un taux de mortalité de 8% en moyenne.

L’arthrite septique du genou est une urgence médicale diagnostic et thérapeutique.
Elle est définie par la prolifération intra articulaire d’un micro-organisme pathogène dont le principal mode de contamination est hématogène.
Cliniquement nous retrouvons principalement la douleur articulaire, l’œdème et le syndrome infectieux (fièvre +frisson).
La ponction articulaire est l’examen clé pour le diagnostic positif.
Le diagnostic microbiologique est complexe, nécessite une collaboration entre orthopédiste, infectiologue et biologiste. L’analyse cytologique du liquide articulaire est riche d’apport. Un aspect macroscopique purulent et une cellularité >100 000/mm³ oriente d’emblée vers le diagnostic de l’arthrite septique du genou. Sur le plan bactériologique, l’examen direct est très utile lorsqu’il est positif.
La prise en charge est multidisciplinaire, repose essentiellement sur l’antibiothérapie et le drainage articulaire.
Un algorithme de prise en charge a été proposé : ponction évacuatrice initiale, traitement médical isolé s'il n'existe pas de synovite, synovectomie de réduction dans le cas contraire. Un traitement mis en œuvre précocement et rapidement efficace semble la clé du succès.

Septic arthritis of the knee is a medical diagnostic and therapeutic emergency.
It is defined by the intra-articular proliferation of a pathogenic microorganism whose main mode of contamination is haematogenous.
Clinically we find mainly joint pain, edema and infectious syndrome (fever + thrill).
Joint puncture is the key examination for positive diagnosis.
The microbiological diagnosis is complex, requires a collaboration between orthopedist, infectiologist and biologist.
The management is multidisciplinary, based mainly on antibiotic therapy and joint drainage.
A management algorithm has been proposed: initial evacuation puncture, isolated medical treatment if there is no synovitis, reduction synovectomy in the opposite case. A treatment that is implemented early and quickly is the key to success.
