1/Présentation de l'UMR639 de l'INSERM
L'UMR 639, "Immunogénétique de la polyarthrite rhumatoïde" a pour but de découvrir les mécanismes du déclenchement de la polyarthrite
rhumatoïde. Elle est associée de très près au service de rhumatologie de l'hôpital de la Conception qui s'occupe principalement de polyarthrite rhumatoïde.
2/ Buts et résultats des 4 dernières années
Nous avons entrepris il y a 4 ans:
1/ De subdiviser la polyarthrite rhumatoïde en sous groupes selon les autoanticorps présents et le génotype HLA-DR des patients.
2/ D'analyser le microchimérisme et son influence dans la polyarthrite.
3/ De monitorer la charge en EBV chez les patients sous biothérapies et de prédire l'activité des biothérapies par des tests génétiques.
Le Project 1 a avancé très vite
Nous avons identifié des nouveaux autoanticorps dans le sérum de patients atteints de polyarthrite en criblant des puces à protéines. Ces nouveaux autoanticorps sont très importants, car découverts à un moment crucial. En effet, il est établi que 60% des patients atteints de polyarthrite rhumatoïde ont dans leur sérum des anticorps anti protéines citrullinées, détectés par le test de routine des anti CCP et qui servent de test diagnostique.
Cependant, 40% des patients sont négatifs pour ce test. Il est critique pour le diagnostic et la compréhension de la PR de découvrir d'autres autoanticorps chez ces patients. La découverte de 3 nouveaux
autoantigènes (Calpastatine, Peptidyl arginyl deiminase 4 et BRAF) et le mapping d'épitopes sur ces autoantigènes nous ont permis de concevoir
des nouveaux outils de diagnostic. Ces outils identifient 50% des polyarthrites, en l'absence d'anti CCP.
3 brevets sont à divers stades d'avancement concernant ces outils.

Le Projet 2 parti de zéro est bien lancé
A son arrivée en 2004, Nathalie Lambert, qui avait quitté un poste de chef de groupe au Fred Hutchinson Cancer Center de Seattle pour rejoindre le laboratoire, a développé des contacts et des collaborations nationales et internationales et a commencé à collecter des échantillons.
Elle a ensuite développé des techniques de dosage du microchimérisme, permettant de détecter, chez un sujet donné, des petites quantités
d'ADN provenant d'un autre individu.
Nathalie a montré que dans la polyarthrite rhumatoïde, des cellules
microchimériques peuvent apporter des allèles HLA-DR qui prédisposent à la polyarhrite rhumatoïde. Cette découverte a fait l'objet d'un article original dans "Arthritis and Rheumatism", la meilleure revue mondiale de Rhumatologie, en 2009. Cet article est accompagné d'un article de
couverture.
Le Projet 3, très "translationel" a été poursuivi et publié dans deux articles majeurs.
Nous avons publié une étude de surveillance de la charge en virus d'Epstein Barr chez des patients atteints de polyarthrite rhumatoïde et traités par biothérapies. Cette étude de 150 patients sous méthotrexate ou anti TNF, suivis pendant cinq ans, a montré que la charge virale était stable, ce qui écartait un trouble majeur de l'immunité cellulaire induit par les anti TNF ou le méthotrexate. Ce travail a été publié dans "Arthritis and Rheumatism" en 2007. Il a été utilisé par la FDA dans son
évaluation des différentes biothérapies.
Un deuxième outil développé est l'étude du polymorphisme du promoteur du gène du TNF. Selon que le polymorphisme en -308 est A ou G, on a affaire à un haut ou bas transcripteur et la réponse à un traitement anti TNF est bonne ou mauvaise. Cet article a été, lui aussi, publié dans
"Arthritis and Rheumatism" en 2007.
