I. INTRODUCTION

All countries worldwide are affected by one or more forms of malnutrition. To battle with malnutrition in all its forms is one of the biggest global health challenges. Women, elderly, infants, children, and adolescents are at particular risk of malnutrition (1).

Undernutrition is a state of energy, protein and/or micronutrient deficiency producing unwanted weight loss of more than 10% in 6 months as well as a measurable change in bodily functions (2).

Often perceived as a problem in developing countries, undernutrition is in reality also common among elderly, precarious or hospitalized populations. In hospitals, it affects up to 70% of the elderly and 60% of cancer patients. It is also very common in intensive care units (3).

Statistics show that:

 1.9 billion adults are overweight or obese, while 462 million are underweight (1).

 52 million young children aged five or less are wasted, 17 million are severely wasted and 155 million are stunted, while 41 million are overweight or obese (1).

 Around 45% of deaths are linked to undernutrition. These mostly occur in low- and middle-income countries. At the same time, in these same countries, rates of childhood overweight and obesity are rising (1).

2 The social, developmental, economic, and medical impacts of the general hamper of malnutrition are to take seriously, for the benefit of individuals, their families, communities and also countries.

Numerous factors can be the cause of malnutrition, mostly the ones related to poor diet or severe and repeated infections, especially in underdeveloped countries. Inappropriate diet and illness are much associated with the general standard of living, the environmental conditions, and whether a population can meet its basic needs such as food, housing, and health care.

Thus, malnutrition is a health outcome as well as a risk factor for disease and worsened malnutrition, and it can expand the risk both of morbidity and mortality.

At the end of the 20th century and beginning of a new millennium, hospital malnutrition continues to be prevalent. Malnutrition has been related to:

 Increased morbidity, mortality, length of hospital stay and costs (4–7).  Malnourished patients stayed in the hospital for 16.7 days vs. 10.177 days

compared to well-nourished(8).

Hospital costs in malnourished patients were increased up to 308.9% (8). Undernutrition is a complex pathophysiological situation, resulting from a deficit in energy and/or protein intake more or less associated with metabolic disturbances secondary to the disease. This means that if the disease promotes and/or worsens undernutrition, undernutrition worsens the disease and limits the effectiveness of treatments. Undernutrition is a global public health issue since more than a billion human beings are affected. It is one of the four major causes

3 of death with an estimate of more than a billion and a half of annual deaths.

Undernutrition is also rampant endemic in hospitals in the most economically developed countries where patients pay a heavy price in terms of morbidity and mortality, while society sees the most additional hospital costs of undernutrition increase with the progress of medicine.

Any pathology can be accompanied by undernutrition, but the extreme ages of life are the most affected. The development of "quality procedures" must lead to the development of screening and treatment strategies for hospital undernutrition. The treatment of undernutrition in hospitalized patients involves improving the supply of food care (dietitians), the rational use of oral supplementation, enteral or even parenteral nutrition (9).

According to the World Health Organization (WHO), cardiovascular diseases are the leading cause of mortality worldwide, taking an estimated 17.9 million lives each year(10), with some variation in epidemiology due to the factors associated with lifestyle, behavioral risk factors, genetic and racial differences (11). In daily practice, the most frequent emergency consultation motif is the heart failure syndrome. Different cardiology societies determined numerous criteria for defining heart failure (HF).

The prevalence of undernutrition during heart failure varies between 10% and 68%, depending on the heart disease considered and the stage of the disease. Undernutrition is often linked to anorexia, eating difficulties in advanced forms due to the increase in postprandial oxygen consumption which can be a real stress test.

4 It is in these situations that splitting the feed can be helpful. Undernutrition is a prognostic factor correlated with survival in forms of chronic heart failure. It is not always easy to correct. Indeed, when the loss of lean mass is a physiological mechanism of adaptation to chronic hypoxia, any attempt at re-nutrition is generally a failure when it does not aggravate hypoxia (12).

However, precedently, no systematic review of mortality risk related to malnutrition in patients admitted with acute heart failure has been performed. We systematically reviewed the observational epidemiology literature to determine the all-cause mortality in undernourished patients with acute heart failure or at risk of malnutrition through a meta-analysis of observational studies.

6 Decreased intake or assimilation of nutrients will result in classic malnutrition (e.g. undernutrition), but there is a growing understanding that disease-associated inflammatory and other mechanisms may also contribute to malnutrition (i.e. undernutrition).

No single existing approach has achieved boundless global acceptance, making it difficult to compare findings beyond regions across the world or even between hospitals within regions.

The malnutrition criteria for deliberation were identified from existing universally used approaches. Then they were graded by election among the Global Leadership Initiative on Malnutrition participants (13).

There was a vigorous approbation of three phenotypic criteria (weight loss, low body mass index, and reduced muscle mass) and two etiologic criteria (reduced food intake or assimilation, and inflammation or disease burden).

1. Phenotypic malnutrition criteria (13)

 Weigh loss: more than 5% within the past 6 months or more than 10% beyond 6 months;

 Low Body Mass Index (BMI): less than 20 Kg/m2 if the patient has 70 years old or younger or less than 22 Kg/m2 if the patient is aged more than 70 years old;

 Reduced muscle mass: validated body composition measuring techniques.

2. Etiologic malnutrition criteria (13)

The etiological criteria are two:

 Inflammation: from acute disease or injury, or chronic disease-related;  Reduced food intake or assimilation: less than 50% of energy requires

more than a week or any reduction that exceeds two weeks, or any gastrointestinal condition that impacts absorption and/or food assimilation.

The requirement for the diagnosis of malnutrition speculated the existence of at least 1 phenotypic criterion and 1 etiologic criterion.

3. Approach to malnutrition diagnosis, severity grading, and classification

Nutritional screening and assessment to differentiate malnourished and non-malnourished patients are the first steps in successful nutritional management in all patients (14,15).

Anthropometric parameters (16), such as BMI, triceps skinfold measurement and mid-arm circumference, and biochemical parameters (17), such as albumin, prealbumin, and cholesterol, are traditional nutritional evaluation indexes and have long been used in isolation to evaluate nutritional status (18,19). However, the use of these indexes alone cannot provide comprehensive and accurate indications of nutritional status (17,19).

Therefore, the Academy of Nutrition and Dietetics (the Academy) and the American Society for Parenteral and Enteral Nutrition (ASPEN.) also recommend the use of two or more of the following multidimensional evaluation technologies for improved nutritional evaluations: inadequate energy intake,

8 weight loss, waste of muscle mass, loss of subcutaneous fat, localized or generalized fluid accumulation that may occasionally mask weight loss and diminished functional status as measured by handgrip strength(20).

Moreover, numerous nutritional screening tools composed of multiple objective nutritional parameters and subjective data from case histories have been developed to enable improved nutritional status evaluations (21).

Generally, multidimensional evaluation tools refer to nutritional assessment tools and nutritional screening tools (18). Nutrition assessment and screening constitute the first of four steps of the nutrition care process and are thus very important. Nutritional screening and nutritional assessment are terms that are often used interchangeably in the literature and in practice despite their differences (21).

The nutrition screening is described by ASPEN as ‘‘a process to identify

an individual who is malnourished or who is at risk for undernutrition to determine if a detailed nutrition assessment is indicated’’ (22) and incorporates many instruments: Nutritional Risk Screening (NRS) (23), the prognostic nutritional index (PNI) (13), the Nutritional Risk Index (NRI) (24) and the Geriatric Nutritional Risk Index (GNRI) (25).

The nutritional assessment has been defined by ASPEN as ‘‘a

comprehensive approach to diagnosing nutrition problems that uses a combination of the following: medical, nutrition and medication histories; physical examination; anthropometric measurements; and laboratory data’’ (26) and includes the Mini Nutritional Assessment (MNA) (21).

1. Definition of acute heart failure

Heart failure (HF), known as well as congestive heart failure (CHF) or congestive cardiac failure (CCF), is when the heart is unable to pump sufficiently to maintain blood flow to meet the body's needs (27,28).It is the inability of the heart to keep up with the demands on it and, specifically, failure of the heart to pump blood with normal efficiency.

Acute heart failure has two forms: according to the 2016 ESC HF guidelines that if a chronic stable HF patient deteriorates it was described as “decompensated”, meantime if it was the first admission it was expressed as “de novo”(29).

 Newly arisen (“de novo”) acute heart failure:

Generally, de novo (literally 'of new') is Latin expression used in the English language to hint 'from the beginning', 'anew'. In the case of acute heart failure, it means a firstly occurred episode.

 Acutely decompensated chronic heart failure (ADCHF) (29):

Acute decompensated heart failure is described as the rapid or graduate outbreak of the signs or symptoms of heart failure demanding unplanned office visits, emergency room visits, or hospitalization (30). Handless of the critical precipitant of the exacerbation, pulmonary and systemic congestion due to increased left- and right-heart filling pressures is a nearly universal finding in ADHF (31).

13 Another important factor that can be added for a better definition of the HF is the ejection fraction (EF) given its importance in the diagnosis and the surveillance of the heart condition:

 Diastolic HF with normal EF: HF with preserved EF (HFpEF)

Patients may seem to have a normal heart to pump a normal portion of the blood that enters it. However, heart muscle thickening may cause the ventricle to hold an abnormally small volume of blood. Thus, even if the heart’s output may continue to emerge to be in the normal range, its limited resources are insufficient to meet the body’s requirements.

 Systolic HF otherwise called: HF with reduced ejection fraction (HFrEF) Here the heart muscle is unable to contract adequately and consequently expels less oxygen-rich blood into the body. Patients with this form of the disease will have lower-than-normal left ventricular ejection fraction on an echocardiogram (32).

2. Epidemiology of undernutrition and risk of undernutrition (26)

There is a lack of epidemiological investigations of the prevalence of undernutrition in HF because a consensus regarding nutritional assessment tools has not been reached. The prevalence of undernutrition and the risk of undernutrition varied greatly among the 17 systematically retrieved studies from 16 to 90 %. We speculate that differences in the subpopulations of HF patients were the main cause of this discrepancy. Studies have demonstrated that the prevalence of undernutrition is higher in both the advanced HF and acute decompensated HF patient subpopulation, at approximately 75–90 %. The prevalence of undernutrition ranges from 16 to 62 % in stable heart patients

14 according to five studies. These findings explain why the incidence of undernutrition or the risk of undernutrition increases with disease severity (4). This speculation is also consistent with Sargento and Al. (33) and Narumi and Al. (34); these studies indicated that nutritional status as assessed using nutritional assessment and screening tools is also associated with the Nt-Pro BNP, the most important surrogate marker of disease severity in HF. Earlier nutrition screening and interventions are essential for preventing HF cachexia and reducing mortality (35,36). Other possible reasons for the differences in the prevalence of undernutrition include the acquisition of data using different nutritional assessment and screening tools and the relatively small sample size

3. Physiopathology of malnutrition in heart failure

Malnutrition is considered a high risk for cardiac failure, but the evidence is limited (37). It seems to be a mediator of disease progression and determines a poor prognosis especially in advanced stages (38). Malnutrition mechanisms in heart failure may result from the following mechanisms:

3.1. Decreased Energy supply

Reduced energy intake and negative energy balance have been associated with lower intake of all macronutrients, including total and saturated fat, protein, and carbohydrate intake (41–43). Anorexia is characterized as both an inadequate food intake and suppressed appetite, can be attributed to the underlying pathophysiology of their disease, defined by a chronic inflammatory state and fluid overload, which leads to nausea, loss of appetite and early satiety from gastrointestinal edema, and hepatic congestion (44,45).

15 The development of appetite suppression, early satiety, and insulin resistance is further exacerbated by neurohormonal etiologies of anorexia (46). Malnutrition in patients with HF is a consequence of anabolic and catabolic imbalance, favoring the catabolic state with impaired gastrointestinal absorption as well as reduced food intake, adding the factor of anorexia, and the psychological disorders.

3.2. Gastrointestinal and hepatic disorders

Moreover, gastrointestinal malabsorption due to gastrointestinal edema or congestion and the hypoperfusion of the bowel in patients with HF are associated with poor nutrition (47,48). Both hepatic congestion and gastric distention contribute to an early postprandial feeling of fullness, and they also could lead to intolerance of food intake.

The reasons for appetite loss are multifactorial, and the drugs frequently prescribed to patients with HF could affect appetite (49). Intestinal congestion and redistribution of blood flow away from the splanchnic circulation due to increased sympathetic activity may contribute to intestinal mucosal barrier dysfunction, which causes lipopolysaccharide translocation into the systemic circulation, and may worsen malnutrition in patients with acute heart failure (AHF) (50).

3.3. Cytokine production (51)

Elevated levels of inflammatory mediators have been identified in patients with heart failure, including heart failure with reduced and preserved ejection fraction, as well as acute decompensated heart failure (52). The link between heart failure and inflammation was first recognized in 1990 by Levine and Al. (53) who reported elevated levels of tumor necrosis factor (TNF) in heart failure patients.

Both innate and adaptive immune responses are activated in the heart in response to tissue injury that results from pathogens or environmental injury (e.g. ischemia or hemodynamic overloading). Whereas the innate immune system provides a global, non-specific defense against pathogens and/or tissue injury, the adaptive immune system provides a highly specific response that is mediated by B and T cells.

The “cytokine hypothesis”(54)for heart failure postulates that heart failure progresses, at least in part, as a result of the deleterious effects exerted by endogenous cytokine cascades on the heart and the peripheral circulation. The literature suggests that TNF modulates myocardial function through an immediate pathway that is manifest within minutes and is mediated by activation of the neutral sphingomyelinase pathway.

Several reports have demonstrated enhanced expression and release of inflammatory cytokines such as tumor necrosis factor (TNF)α, interleukin (IL)-1, IL-6, IL-18, and cardiotrophin-(IL)-1, as well as several chemokines (e.g. monocyte chemoattractant peptide (MCP)-1/CCL2, IL-8/CXCL8 and macrophage inflammatory protein-1α/CCL3) in HF patients (55–59).

3.5. Electrolytes and micronutrients disorders’

Another futile energy expending cycle involves the sodium, potassium transporter Na+ K+ ATPase, an enzyme complex that uses ATP to pump Na out of cells(64). The failing heart is an energy-compromised organ, characterized by “metabolic remodeling” (65).

In addition, ischemia and oxidative stress may further reduce energy expenditure, resulting in cardiomyocyte injury (66). A larger number of micronutrient deficiencies have been shown to be associated with significantly shorter event-free survival, therefore underlining the importance of micronutrient status and high-quality dietary intake.

Such information is particularly important within the context of an association between sodium restriction and decreased intake of other micronutrients, including calcium, phosphate, thiamine, and folate, in addition to reduced intake of energy and carbohydrates (67).

While deficiencies in HF patients have been described for micronutrients that are involved in carbohydrate and fatty acid metabolism, calcium homeostasis, renin production, and antioxidant properties, their effects on energy utilization, cardiac contractility, endothelial function, and reactive oxidative stress have not been fully described in patients with HF (44,45,68–72).

21 Therefore, HF disease pathophysiology predisposes patients to a disadvantage in nutritional status, as patients with HF are experiencing malnutrition from an energy imbalance arising from increased energy requirements and decreased energy intake.

3.8. Insulin resistance

Doehner and Al. showed that insulin resistance correlated with higher leptin levels in non-cachectic patients with HF (76). Insulin is an essential hormone for utilizing glucose and facilitating anabolism, thus potentially contributing to muscle mass regulation.

Impaired glucose metabolism has been observed in 43 % of patients with HF, which contributes to further muscle wasting (9). The high rates of ATP production and turnover are critical in maintaining cardiac contractility to deliver blood and oxygen to the other organs. Under normal conditions, cardiac ATP is mainly derived from fatty acid (FA) oxidation (FAO), with glucose metabolism contributing less (77). However, under stress conditions, FAO may be reduced, which is concomitant with increased glucose utilization (78).

3.9. Renal insufficiency

Heart failure pathophysiology is characterized by systolic and diastolic abnormalities of the myocardium that result in reduced cardiac output (CO) and reduced renal perfusion. These trigger compensatory mechanisms, including left ventricular dilatation and activation of the sympathetic and renin-angiotensin-aldosterone systems (RAAS) which lead to a vicious cycle of systemic vasoconstriction, increased blood pressure and heart rate, and sodium and water retention despite existing volume overload.
