Statins’ role in prevention Do statins have a role in primary prevention of cardiovascular events?

QUESTIONS

What is the overall health effect when statins are prescribed for primary prevention? Should the dose of a statin be titrated to achieve target lipid levels?

ANSWERS

If serious cardiovascular adverse events are viewed in isolation, 71 primary prevention patients with cardiovascular risk factors would have to be treated with statins for 3 to 5 years to prevent one myocardial infarction or stroke. This cardiovascular benefi t is not refl ected in two measures of overall health effect: total mortality and total serious adverse events. Therefore, statins have not been shown to provide overall health benefi ts in primary prevention trials.

Statins’ benefi t for secondary prevention confi rmed

QUESTION

What is the optimal dosing strategy?

ANSWER

Statins provide a cardiovascular and total mortality benefit for patients with clinically evident occlusive vascular disease (secondary prevention) and cholesterol levels of >3.5 mmol/L. Large randomized controlled trials are required to test various statin dosing strategies for secondary prevention before making fi rm recommendations.

Quels sont les effets des statines prescrites comme mesure de prévention primaire sur la santé générale? La dose d’une statine devrait-elle être titrée pour obtenir les taux lipidiques visés?

RÉPONSES

Si l’on envisage séparément les incidents cardiovasculaires indésirables graves, il faudrait traiter en prévention primaire 71 patients présentant des facteurs de risque cardiovasculaire pendant 3 à 5 ans pour prévenir un seul infarctus du myocarde ou un accident vasculaire cérébral. Par conséquent, il n’a pas été démontré que les statines apportent des bienfaits sur la santé générale dans les études sur la prévention primaire.

Les statines procurent des bienfaits sur le plan cardiovasculaire et de la mortalité totale aux patients souff rant d’une maladie vasculaire occlusive cliniquement évidente (prévention secondaire) et ayant une cholestérolémie de >3,5 mmole/l. Il faudrait eff ectuer des études contrôlées randomisées d’envergure pour mettre à l’essai diverses posologies pour la prévention secondaire avant de présenter des recommandations probantes
