The e n t i r e c o h o r t i s r e - b l e d a p p r o x i m a t e l y e v e r y s i x months (Form 2 C ) , d e p e n d i n g on r e s o u r c e s and c i r c u m s t a n c e s . Serum s p e c i m e n s a r e p r e f e r a b l e , but f i l t e r - p a p e r b l o o d s c a n be u s e d i f h a n d l e d p r o p e r l y ; c o o r d i n a t i o n w i t h t h e l a b o r a t o r y i s n e c e s s a r y .
S u p p o r t and s u p e r v i s i o n a r e m a n d a t o r y . To e n c o u r a g e p a r t i c i p a t i o n , v i t a m i n s and i r o n p i l l s can be d i s t r i b u t e d i n t h e v i l l a g e s a t t h e t i m e o f t h e mass s e r o s u r v e y s . The s u c c e s s o f r e p e a t s e r o l o g i c a l s u r v e y s can be f a c i l i t a t e d by o f f e r i n g some c u r a t i v e s e r v i c e s when f e v e r s u r v e i l l a n c e i s b e i n g done.
A s p i r i n and c h l o r o q u i n e can be g i v e n t o e l i m i n a t e m a l a r i a as a cause o f t h e f e v e r . T h e s e s y s t e m s a l w a y s r e q u i r e t h e c o o p e r a t i o n and s u p p o r t o f a s t r o n g headman o r c h i e f , A v i l l a g e member who i s d e s i g n a t e d f o r c o n t i n u o u s s u r v e i l l a n c e s h o u l d be p r o p e r l y t r a i n e d and c o m p e n s a t e d .

Nomads o r o t h e r m o b i l e p o p u l a t i o n s w i l l r e q u i r e s p e c i a l s u r v e i l l a n c e p r o c e d u r e s u s i n g t h e same p r i n c i p l e s d e s c r i b e d i n t h i s m a n u a l . T h i s may i n c l u d e v i s i t s t o nomad camps, w a t e r i n g h o l e s , p e r i o d i c m a r k e t s , e t c . The p e r i o d i c p o i n t p r e v a l e n c e s u r v e y i s a good method f o r d e a l i n g w i t h t h e s e p o p u l a t i o n s .

3. MANAGEMENT OF CASES
3 . 1 I n i t i a l c o n s i d e r a t i o n s i n c l u d i n g b u r i a l
Management o f t h e p a t i e n t depends t o a c e r t a i n e x t e n t upon t h e l o c a t i o n o f t h e p a t i e n t when the d i s e a s e i s f i r s t d i a g n o s e d . As soon as t h e d i a g n o s i s o f monkeypox, L a s s a f e v e r , M a r b u r g o r E b o l a v i r u s i n f e c t i o n , o r any o t h e r d a n g e r o u s p a t h o g e n , i s s u s p e c t e d t h e p a t i e n t must be i s o l a t e d t o p r e v e n t s e c o n d a r y i n f e c t i o n ; i f the p a t i e n t i s s u s p e c t e d o f h a v i n g y e l l o w
f e v e r an e f f e c t i v e m o s q u i t o net s h o u l d be e m p l o y e d .
The p a t i e n t s h o u l d be i s o l a t e d and b a r r i e r n u r s i n g t e c h n i q u e s u s i n g a v a i l a b l e p e r s o n s , e q u i p m e n t , c l o t h i n g and m a t e r i a l s must be i n s t i t u t e d i m m e d i a t e l y i f M a r b u r g o r E b o l a v i r u s i n f e c t i o n . L a s s a f e v e r o r any o t h e r d a n g e r o u s p a t h o g e n i s s u s p e c t e d . P e r s o n s i n v e s t i g a t i n g and c a r i n g f o r monkeypox p a t i e n t s s h o u l d h a v e been s u c c e s s f u l l y v a c c i n a t e d o r r e v a c c i n a t e d a g a i n s t s m a l l p o x w i t h i n the p r e v i o u s t h r e e y e a r s ; o n l y t h o s e s t a f f c a r i n g f o r t h e p a t i e n t need to be v a c c i n a t e d a g a i n s t s m a l l p o x . P e r s o n s i n v e s t i g a t i n g and c a r i n g f o r p o s s i b l e y e l l o w f e v e r c a s e s s h o u l d h a v e been immunized a g a i n s t y e l l o w f e v e r w i t h i n the p r e v i o u s 10 y e a r s .
I f t h e p a t i e n t i s d i a g n o s e d a t home, c o n s i d e r a t i o n s h o u l d be g i v e n t o p r o v i d i n g t r e a t m e n t at t h i s s i t e u n l e s s b e t t e r c a r e c o u l d be g i v e n a t a h e a l t h u n i t . When a p a t i e n t i s k e p t a t home, t h o s e i n c o n t a c t s h o u l d t a k e e v e r y p r e c a u t i o n t o p r e v e n t t h e i r c o n t r a c t i n g the d i s e a s e . They s h o u l d be s e p a r a t e d from o t h e r s i n t h e v i l l a g e t o p r e v e n t community s p r e a d . B a r r i e r n u r s i n g t e c h n i q u e s and o t h e r p r o c e d u r e s d e s c r i b e d l a t e r ( s e c t i o n s 3.1.2, 3.1.3) s h o u l d be a d a p t e d t o t h i s s i t u a t i o n . I f the d i a g n o s i s i s made a t a r u r a l c l i n i c o r h e a l t h c e n t r e , t h e p a t i e n t s h o u l d be g i v e n t r e a t m e n t a t t h i s s i t e r a t h e r t h a n t r a n s p o r t i n g t h e p a t i e n t t o a h o s p i t a l . C a r e f u l l y c o n t r o l l e d b a r r i e r n u r s i n g s h o u l d be e s t a b l i s h e d i n p a r t o f t h e f a c i l i t y and no o t h e r a c t i v i t y s h o u l d t a k e p l a c e t h e r e . , .!.;>:,: . ; ; .
The t r a n s p o r t o f p e r s o n s s u s p e c t e d o f h a v i n g one o f the h a e m o r r h a g i c f e v e r s r e q u i r e s c o n - s i d e r a b l e c a r e d u r i n g t h e t r i p and p r e p a r a t i o n s must be made a t t h e r e c e i v i n g i n s t i t u t i o n . T r a v e l s t r e s s t o a s e v e r e l y i l l p e r s o n can be d e t r i m e n t a l . D u r i n g e p i d e m i c s o f h a e m o r r h a g i c
f e v e r s i n r u r a l a r e a s , or even c i t i e s , h o w e v e r , i t i s o f t e n d e s i r a b l e t o q u a r a n t i n e p a t i e n t s at a l o c a l h o s p i t a l o r m e d i c a l f a c i l i t y . T h i s w i l l d e c r e a s e the l i k e l i h o o d o f f u r t h e r c a s e s o c c u r r i n g i n t h e f a m i l y , a m a j o r r i s k i f t h e p a t i e n t i s c a r e d f o r at home. I n o r d e r f o r t h e p a t i e n t t o be t r a n s f e r r e d , a d e q u a t e t r a n s p o r t must be a v a i l a b l e , and t r a i n e d m e d i c a l s t a f f must be p r e s e n t i n o r d e r t o p r o p e r l y c a r e f o r t h e p a t i e n t . P a t i e n t s w i t h s u s p e c t e d o r p r o v e n y e l l o w f e v e r s h o u l d n o t be moved o u t o f t h e i n f e c t e d z o n e .
I t i s v e r y u n l i k e l y t h a t a s p e c i a l h i g h s e c u r i t y p l a s t i c " t r a n s p o r t i s o l a t o r " w i l l be a v a i l a b l e f o r such t r a n s p o r t . I f n o t , an open p i c k - u p t r u c k o r t h e b a c k o f any o t h e r v e h i c l e s h o u l d be u s e d ; t h e p a t i e n t i s p l a c e d on a s t r e t c h e r i n t h e r e a r . Two p e r s o n s s h o u l d a t t e n d the p a t i e n t . B o t h s h o u l d wear a d i s p o s a b l e gown ( i f a v a i l a b l e ) o r c l o t h gown. Shoes w i t h c o v e r i n g s ( c l o t h w i l l do) a r e i m p o r t a n t . The a t t e n d a n t s s h o u l d wear r u b b e r g l o v e s , masks and g o g g l e s o r g l a s s e s . R e s p i r a t o r y masks a r e d e s i r a b l e i f a v a i l a b l e . However, e x p e r i e n c e s t r o n g l y s u g g e s t s t h a t a s u r g i c a l mask made o f c l o t h o r h e a v y p a p e r w i l l s u f f i c e . The d r i v e r o f t h e v e h i c l e need wear o n l y a mask and g l o v e s and s h o u l d n o t come i n c o n t a c t w i t h t h e p a t i e n t
When t h e p a t i e n t i s removed f r o m t h e v e h i c l e , t h e bed o f t h e v e h i c l e s h o u l d be s p r a y e d w i t h 2% f o r m a l d e h y d e o r a h y p o c h l o r i t e s o l u t i o n ( 1 0 % h o u s e h o l d b l e a c h ) and t h e n t h o r o u g h l y r i n s e d w i t h c o p i o u s amounts o f w a t e r t o p r e v e n t harm t o t h e t r u c k bed ( s e e Annex 5 A ) . The p r o t e c t i v e c l o t h e s o f t h e a t t e n d a n t s a r e t h e n d e s t r o y e d i f t h e y a r e d i s p o s a b l e , o r d i s i n f e c - ted by s o a k i n g i n a f o r m a l d e h y d e o r h y p o c h l o r i t e s o l u t i o n and t h e n w a s h i n g i n w a t e r (Annex 5 A ) .
