DEVELOPMENT OF PROGRAMME BUDGETING AND MANAGEMENT OF WHO'S RESOURCES AT COUNTRY LEVEL

Report by the Director-General

The attached document contains proposals for changing the W H O programme budgeting procedure at regional and country levels, as well as the method of preparation and the form of presentation of the programme budget document. These proposals are intended to promote further collaboration with Member States in programme planning and i n c r e a s e the programme o r i e n t a t i o n of the development of WHO pro-

grammes and management of WHO's r e s o u r c e s a t country l e v e l . Regional committee resolutions endorsing these proposals are annexed to the report.

1. I n t r o d u c t i o n

1.1 Programming, budgeting and management concepts in W H O have been under continuing r e v i e w , development and improvement in the regional committees, the Executive B o a r d , the W o r l d Health A s s e m b l y , and at all levels of the Organization since its founding in 1948.

1.2 A particularly important development took place in 1972, when the Twenty-fifth W o r l d Health A s s e m b l y , in resolution W H A 2 5 . 2 3 a d o p t e d a new "programme budget" form of presenta- tion based on the principles of a programme-oriented approach to planning and b u d g e t i n g , set f o r t h in the D i r e c t o r - G e n e r a l ' s r e p o r t on t h i s s u b j e c t . ^ These p r i n c i p l e s can be summarized as "programming by objectives and budgeting by

p r o g r a m m e s "

1.3 These programme budgeting principles have been applied to the development of the Fifth and Sixth General Programmes of Work and to the formulation of biennial programmes corresponding to the major functions and objectives of the Organization. The form of presentation of the WHO programme budget document has been made more programme-oriented. The use of global

programme statements supported by budget tables has facilitated programme review by the Executive Board and the W o r l d Health Assembly.

1.4 In the light of experience, it is now proposed to further extend the application of these principles of programme budgeting to the planning and development of programmes of technical cooperation with governments and to the management of W H O

s resources at country level. This report contains proposals affecting W H O planning procedures with governments, the allocation of W H O resources within regions, the W H O budget preparation cycle at country level, and the form of presentation of programme budget proposals for review by regional c o m m i t t e e s , the Executive Board and the World Health Assembly.

2 . P r o b l e m s a s s o c i a t e d w i t h the fragmented, p r o j e c t a p p r o a c h

2 . 1 W H O _ s c o u n t r y p r o g r a m m e s u s u a l l y c o n s i s t of a set of a c t i v i t i e s k n o w n as p r o j e c t s . T h e p r o j e c t c o n c e p t d a t e s b a c k to the v e r y b e g i n n i n g s of the O r g a n i z a t i o n , a n d is g e n e r a l l y a p p l i e d i n i n s t i t u t i o n s and programmes o f the U n i t e d N a t i o n s s y s t e m . I t stemmed from the

" s e c t o r a l " a p p r o a c h a d o p t e d i n i t i a l l y to deal w i t h f r a g m e n t e d r e q u e s t s from g o v e r n m e n t s for s p e c i f i c a s s i s t a n c e in v a r i o u s fields s u c h as m a l a r i a , m a t e r n a l a n d c h i l d h e a l t h , or

e n v i r o n m e n t a l s a n i t a t i o n .

2.2 R e c e n t a n a l y s e s in W H O , i n c l u d i n g the f i n a n c i a l a n d e f f e c t i v e n e s s a u d i t s of c o u n t r y p r o j e c t s in two r e g i o n s c a r r i e d out b y the E x t e r n a l A u d i t o r , h a v e d e m o n s t r a t e d that a s i g n i f i c a n t p e r c e n t a g e of c o u n t r y p r o j e c t s i n c l u d e d in the b u d g e t w e r e n e v e r i m p l e m e n t e d , a n d t h a t , of t h o s e i m p l e m e n t e d , a s i g n i f i c a n t n u m b e r d e v i a t e d m o r e than 5 0 % from the b u d g e t , m a i n l y as a r e s u l t of c h a n g e s in r e q u e s t s from g o v e r n m e n t s . A s a c o n s e q u e n c e , a g r e a t d e a l of t i m e a n d e f f o r t m u s t b e e x p e n d e d on m a k i n g d e t a i l e d b u d g e t r e v i s i o n s u p to a n d d u r i n g the o p e r a t i n g y e a r . T h e a n a l y s e s suggest that a l l o c a t i o n a n d b u d g e t i n g of r e s o u r c e s b a s e d o n the a c c u m u l a t i o n o f d e t a i l e d a n d f r a g m e n t e d p r o j e c t p r o p o s a l s are n o t a n e f f e c t i v e m e t h o d o l o g y for p l a n n i n g the h e a l t h p r o g r a m m e s of W H O o r its M e m b e r S t a t e s .

2.3 T r a d i t i o n a l l y the d e t a i l e d p l a n n i n g of c o u n t r y p r o j e c t p r o p o s a l s has i n c l u d e d the p r e p a r a t i o n of d e t a i l e d cost e s t i m a t e s for project s t a f f , e q u i p m e n t , s u p p l i e s , t r a n s p o r t , f e l l o w s h i p s and o t h e r o b j e c t s of e x p e n d i t u r e two or t h r e e y e a r s p r i o r to the b e g i n n i n g of the r e l e v a n t o p e r a t i n g y e a r . T h e a t t e m p t in the W H O p l a n n i n g p r o c e s s to f o r m u l a t e these d e t a i l s so far in a d v a n c e , in o r d e r to be in a p o s i t i o n to submit d e t a i l e d p r o j e c t cost e s t i m a t e s in p r o g r a m m e b u d g e t d o c u m e n t s in time for r e v i e w b y the r e g i o n a l c o m m i t t e e s , the E x e c u t i v e B o a r d a n d the W o r l d H e a l t h A s s e m b l y , a p p e a r s i n c o n s i s t e n t w i t h the r e q u i r e m e n t s of t h e c o u n t r y h e a l t h p l a n n i n g p r o c e s s a n d the n e e d for f l e x i b i l i t y on the part of the O r g a n i z a t i o n a n d M e m b e r S t a t e s .

2 . 4 T h e d e t a i l e d a d v a n c e p l a n n i n g of c o u n t r y p r o j e c t s h a s the further u n d e s i r a b l e e f f e c t of m a k i n g such p l a n n i n g out of p h a s e w i t h the rest of W H O

s own p l a n n i n g c y c l e a n d w i t h the n a t i o n a l b u d g e t p r e p a r a t i o n c y c l e of m o s t M e m b e r S t a t e s . B y the time r e g i o n a l a n d head- q u a r t e r s p r o g r a m m e s a r e b e i n g p l a n n e d , c o u n t r y p r o j e c t s h a v e a l r e a d y b e e n d e c i d e d u p o n , t h e r e b y d i s c o u r a g i n g m u t u a l d i a l o g u e a n d a d a p t a t i o n of p r o p o s a l s b e t w e e n the d i f f e r e n t e c h e l o n s o f W H O . It w o u l d a p p e a r p r e f e r a b l e to plan a h e a d o n a b r o a d e r , m o r e programme- o r i e n t e d b a s i s , a n d m o v e to d e t a i l e d p r o j e c t p l a n n i n g c l o s e r to a n d as a p a r t of the imple- m e n t a t i o n p r o c e s s , w h e n n a t i o n a l a u t h o r i t i e s are p r e p a r i n g the n a t i o n a l h e a l t h b u d g e t , w h e n t h e a v a i l a b i l i t y o f e x t e r n a l funds is m o r e likely to b e k n o w n , a n d w h e n there h a s b e e n a n o p p o r t u n i t y for a b e t t e r p l a n n i n g d i a l o g u e b e t w e e n a l l e c h e l o n s of W H O .

2.5 It h a s f r e q u e n t l y b e e n s t a t e d in W H O that " p l a n n i n g b e g i n s at c o u n t r y level"• This is

i n d e e d t r u e - but the n a t u r e of such p l a n n i n g is c h a n g i n g in a f u n d a m e n t a l w a y . The

t r a d i t i o n a l r e l a t i o n s h i p of a n " a s s i s t i n g agency" r e s p o n d i n g to ad h o c p r o j e c t r e q u e s t s of

a n " a s s i s t e d g o v e r n m e n t " is b e i n g r e p l a c e d b y a c o n c e p t of joint p r o g r a m m e p l a n n i n g w i t h

c o u n t r i e s . This c o n c e p t i m p l i e s t h a t c o u n t r y h e a l t h p l a n n i n g is a c o l l a b o r a t i v e process in

w h i c h n a t i o n a l h e a l t h a u t h o r i t i e s , WHO c o u n t r y r e p r e s e n t a t i v e s , r e g i o n a l o f f i c e s and WHO h e a d q u a r t e r s a r e a l l a s s o c i a t e d . Such a c o u n t r y h e a l t h p l a n n i n g p r o c e s s b e g i n s w i t h m u l t i -

s e c t o r a l problem a n a l y s i s , s e t t i n g o f o v e r a l l p r i o r i t i e s and p o l i c i e s , and i d e n t i f i c a t i o n o f the g e n e r a l s t r a t e g y o f a t t a c k and p a t t e r n o f c o l l a b o r a t i o n . I t i s o n l y w i t h i n t h i s frame- work t h a t d e t a i l e d p r o j e c t s and a c t i v i t i e s a r e e v e n t u a l l y f o r m u l a t e d . Thus the c o l l a b o r a t i v e

t e c h n i c a l c o o p e r a t i o n programming c o n c e p t i s f u n d a m e n t a l l y d i f f e r e n t from the fragmented p r o j e c t approach t o " t e c h n i c a l a s s i s t a n c e " r e q u e s t s i n t h e p a s t , and makes i t a l l the more i m p o r t a n t f o r the programming p r o c e s s e s and budget c y c l e s o f WHO to be brought i n t o c l o s e c o o r d i n a t i o n w i t h t h o s e o f Member S t a t e s .

2 . 6 The E x e c u t i v e Board p u r s u e d t h i s theme o f p r o g r a m m e - o r i e n t e d b u d g e t i n g and management i n i t s o r g a n i z a t i o n a l s t u d y on the i n t e r r e l a t i o n s h i p s between the c e n t r a l t e c h n i c a l s e r v i c e s o f WHO and programmes o f d i r e c t a s s i s t a n c e t o Member S t a t e s , i n w h i c h the Board s t r e s s e d the i m p o r t a n c e o f p r o g r a m m e - o r i e n t e d p l a n n i n g a t c o u n t r y l e v e l . As t h e E x e c u t i v e Board o b s e r v e d i n i t s o r g a n i z a t i o n a l s t u d y , " t h e a c c u m u l a t i o n i n W H O - a s s i s t e d c o u n t r y programmes o f an i n c r e a s i n g number o f i n d i v i d u a l p r o j e c t s , however e f f i c i e n t l y p l a n n e d and e f f e c t i v e l y d e l i v e r e d , has u n d o u b t e d l y r e s u l t e d i n a patchwork o f a c t i v i t i e s , l a c k i n g g e n e r a l purpose and b a l a n c e and b e a r i n g i n s u f f i c i e n t r e l a t i o n s h i p t o n a t i o n a l development. The s u p p o r t g i v e n by the O r g a n i z a t i o n t o n a t i o n a l h e a l t h p l a n n i n g has been a move i n the r i g h t d i r e c t i o n but i t has a l l too o f t e n been c o n c e i v e d i n d e p e n d e n t l y from o n g o i n g p r o j e c t a c t i v i t i e s and has t h e r e f o r e f a i l e d , i n most i n s t a n c e s , t o i n t e g r a t e them i n t o c o h e r e n t and m e a n i n g f u l

programmes".1 The E x e c u t i v e Board c o n c l u d e d , " i t i s e v i d e n t t h a t , a f t e r i t s f i r s t 25 y e a r s o f e x i s t e n c e , WHO s h o u l d n o t r e t a i n the c o n c e p t o f fragmented p r o j e c t s but must d e f i n i t e l y o r i e n t i t s d i r e c t a s s i s t a n c e t o c o u n t r i e s towards a s i n g l e approach w i t h i n w h i c h programmes s h o u l d be i d e n t i f i e d as s p e c i f i c s e t s o f a c t i v i t i e s t o be implemented i n a g i v e n p e r i o d but i n r e l a t i o n t o o v e r a l l o b j e c t i v e s " • 1

2 . 7 The E x t e r n a l A u d i t o r summarized the s i t u a t i o n i n h i s r e p o r t t o the W o r l d H e a l t h Assembly f o r the f i n a n c i a l year 1975, and c o n c l u d e d , "My recommendations emphasized the need f o r

p l a n n i n g to be p r o g r a m m e - o r i e n t e d , as d i s t i n c t from p r o j e c t - o r i e n t e d , w i t h a c l e a r e r d e f i n i - t i o n o f o b j e c t i v e s a g a i n s t w h i c h e v a l u a t i o n c o u l d be made, A s u b s t a n t i a l p a r t o f the p r e s e n t work w i t h f r e q u e n t budget r e v i s i o n s c o u l d be a v o i d e d . I f the r e g i o n a l committee t a k e s

d e c i s i o n s on p l a n n i n g and b u d g e t i n g o n l y o n b r o a d l i n e s , and i f the d e t a i l e d p r o j e c t p l a n n i n g takes p l a c e a t a l a t e r s t a g e and as p a r t o f the programme i m p l e m e n t a t i o n , i t w o u l d be p o s s i b l e to a r r i v e a t a smoother and more r a t i o n a l p l a n n i n g procedure"•^

2.8 I t i s s u b m i t t e d t h a t t h e time has come t o d i s c a r d t h e p r a c t i c e o f b u i l d i n g up the programme and budget on a s e r i e s o f fragmented p r o j e c t s and t o o r i e n t WHO1 s t e c h n i c a l

c o o p e r a t i o n towards a s i n g l e programme a p p r o a c h w i t h i n w h i c h p r o j e c t s a r e i d e n t i f i e d , p l a n n e d and implemented i n r e l a t i o n to o v e r a l l programme o b j e c t i v e s , and i n c l o s e r harmony w i t h the n a t i o n a l h e a l t h p l a n n i n g p r o c e s s and w i t h programming a t a l l l e v e l s o f WHO. To t h i s end a new programme b u d g e t i n g p r o c e d u r e i s s u g g e s t e d f o r c o n s i d e r a t i o n by the E x e c u t i v e Board and the W o r l d H e a l t h Assembly.

3, Suggested programme b u d g e t i n g p r o c e d u r e and budget c y c l e

3 . 1 I n s t e a d o f r e v i e w i n g d e t a i l s o f proposed p r o j e c t s and a c t i v i t i e s o f a s s i s t a n c e t o c o u n t r i e s two or t h r e e y e a r s p r i o r to the o p e r a t i n g p e r i o d , i t i s proposed t h a t the R e g i o n a l D i r e c t o r s i n i t i a t e the new programme budget c y c l e s i n the r e g i o n s by c o n s i d e r i n g w i t h WHO r e p r e s e n t a t i v e s and r e g i o n a l a d v i s e r s b r o a d p o l i c y t r e n d s , g e n e r a l p r i o r i t i e s i n h e a l t h ,

r e g i o n a l a n d n a t i o n a l n e e d s and t h e i r s t a t e d p r i o r i t i e s , and t h e a p p r o p r i a t e b r o a d l i n e s o f

p a r t i c i p a t i o n by WHO and other p a r t n e r s in i n t e r n a t i o n a l h e a l t h work, in accordance with g u i d e l i n e s s e t by the General Programme of Work for a S p e c i f i c Period and v a r i o u s r e s o l u t i o n s of the r e g i o n a l committees, the Executive Board and the World Health Assembly. This d i a l o g u e would provide the b a s i s for o v e r a l l programme guidance to the WHO r e p r e s e n t a t i v e s and r e g i o n a l

a d v i s e r s and t h e s e t t i n g by t h e R e g i o n a l D i r e c t o r s o f p r o v i s i o n a l c o u n t r y p l a n n i n g f i g u r e s

f o r WHO t e c h n i c a l c o o p e r a t i o n , within the t e n t a t i v e r e g i o n a l a l l o c a t i o n e s t a b l i s h e d by the D i r e c t o r - G e n e r a l for each r e g i o n , as i n d i c a t e d in s e c t i o n 4 below.

3 . 2 Following the e s t a b l i s h m e n t of p r o v i s i o n a l country planning f i g u r e s a t l e a s t two y e a r s

p r i o r t o t h e o p e r a t i o n p e r i o d , WHO r e p r e s e n t a t i v e s and r e g i o n a l a d v i s e r s w o u l d b e g i n t h e f o r m a l programming p r o c e s s w i t h n a t i o n a l a u t h o r i t i e s , w o r k i n g c l o s e l y w i t h t h e m i n i s t r y o f h e a l t h t o g e t h e r w i t h o f f i c i a l s i n r e l a t e d s o c i o e c o n o m i c f i e l d s a s may be a p p r o p r i a t e , t o

secure mutual agreement on: (a) the main developmental t h r u s t s in p u b l i c h e a l t h planned for

t h e n e x t s e v e r a l y e a r s; (b) a c t i o n p r o p o s e d a t n a t i o n a l l e v e l t o g i v e e f f e c t t o t h e G e n e r a l Programme o f Work and r e s o l u t i o n s o f t h e W o r l d H e a l t h A s s e m b l y , t h e E x e c u t i v e B o a r d and regional committees; (с) areas i n which external cooperation w i l l be sought and i n which the p a r t i c i p a t i o n of WHO appears p a r t i c u l a r l y d e s i r a b l e; (d) programme o b j e c t i v e s , t a r g e t s and

expected output of WHO cooperation proposed for the biennium

and (e) a t e n t a t i v e e s t i m a t e

o f t h e c o s t o f WHO t e c h n i c a l c o o p e r a t i o n , by programme.

3 . 3 The c o n c l u s i o n s f r o m t h e programming p r o c e s s w i t h n a t i o n a l a u t h o r i t i e s o u t l i n e d above w o u l d be summarized i n t h e form o f n a r r a t i v e c o u n t r y programme s t a t e m e n t s , s e t t i n g o u t b r o a d

programme t r e n d s , o b j e c t i v e s and modes o f a c t i o n , g i v i n g t h e c o u n t r y p l a n n i n g f i g u r e s b r o k e n down by programme, b u t n o t g i v i n g d e t a i l s o f s t a f f , e q u i p m e n t , s u p p l i e s , t r a n s p o r t, f e l l o w - s h i p s o r o t h e r o b j e c t s o f e x p e n d i t u r e . T h e s e c o u n t r y programme s t a t e m e n t s and p l a n n i n g f i g u r e s by programme w o u l d be s u b m i t t e d , as p a r t o f t h e p r o p o s e d r e g i o n a l programme b u d g e t ,

to the r e g i o n a l committee f o r review. I f endorsed by the r e g i o n a l committee, the country

p l a n n i n g f i g u r e s 一 b r o k e n down by programme - w o u l d be c o n s o l i d a t e d i n t o t h e p r o p o s e d g l o b a l

programme a l l o c a t i o n s to be included in the D i r e c t o r - G e n e r a l ' s proposed programme budget for c o n s i d e r a t i o n by the Executive Board and the World Health Assembly, a s i n d i c a t e d in s e c t i o n 5

b e l o w .

3 . 4 The d e t a i l e d plans of o p e r a t i o n or work n e c e s s a r y f o r the implementation of s p e c i f i c c o o p e r a t i o n a c t i v i t i e s a t country l e v e l would continue to be developed j o i n t l y with n a t i o n a l a u t h o r i t i e s , for review with the Regional D i r e c t o r a t a meeting of WHO r e p r e s e n t a t i v e s ,

r e g i o n a l a d v i s e r s and n a t i o n a l c o o r d i n a t o r s as may be a p p r o p r i a t e , i n t h e y e a r i m m e d i a t e l y

preceding the o p e r a t i n g p e r i o d . I t i s only a t t h i s point that the broad programme p r o p o s a l s ,

r e f e r r e d t o i n p a r a g r a p h 3 . 3 above and i n c l u d e d i n t h e r e g i o n a l programme b u d g e t document, w o u l d be c a s t i n d e t a i l e d terms o f human r e s o u r c e s , e q u i p m e n t , s u p p l i e s , t r a n s p o r t arid o t h e r o b j e c t s o f e x p e n d i t u r e , w i t h s u p p o r t i n g c o s t e s t i m a t e s .

3 . 5 The d i a l o g u e with n a t i o n a l a u t h o r i t i e s on the d e t a i l e d plan of o p e r a t i o n or work would

c o n t i n u e d u r i n g the o p e r a t i n g p e r i o d , as p a r t o f t h e p r o c e s s o f o n g o i n g management, e v a l u a t i o n

and pre-planning of WHO t e c h n i c a l cooperation a c t i v i t i e s . While accounting c o n t r o l s would monitor expenditure within a u t h o r i t y to spend, s u f f i c i e n t f l e x i b i l i t y could be e x e r c i s e d to

a d a p t t o c h a n g i n g needs a n d c o n d i t i o n s * S a v i n g s i d e n t i f i e d d u r i n g t h e o p e r a t i n g p e r i o d

would continue to be a v a i l a b l e for reprogramming within the country, between c o u n t r i e s , or

even between r e g i o n s , as o v e r a l l programme o p e r a t i o n s and p r i o r i t i e s m i g h t r e q u i r e .

3.6 Annex I contains a chart i l l u s t r a t i n g the proposed programme budget c y c l e f o r the development of WHO country programmes for 1980-1981. The proposed procedure can be

summarized i n 10 s t e p s :

Step 1 The Director-General would g i v e general programme guidance and n o t i f y t e n t a t i v e

r e g i o n a l a l l o c a t i o n s t o e a c h R e g i o n a l D i r e c t o r i n t h e s e c o n d h a l f o f 1977, a t l e a s t two y e a r s p r i o r t o t h e b e g i n n i n g o f t h e o p e r a t i n g p e r i o d 1980-1981.
