Français :
En France, le nombre d'IVG de diminue pas et concerne trop souvent des échecs de
contraception. Le DIU est une contraception de première intention très efficace, de longue
durée d’action, indiqué quelle que soit la parité. 1,3% des femmes nullipares de 15 à 49 ans
l'utilisent. Les raisons du très faible recours des nullipares au DIU sont complexes et
impliquent notamment les représentations des femmes.
L’objectif de cette étude qualitative a été d’explorer les représentations des femmes nullipares
concernant la contraception par DIU.
Des entretiens semi-dirigés ont été réalisés avec des femmes nullipares, nulligestes, n’utilisant
pas de contraception par DIU. L'étude a été conduite en recherche de variation maximale,
jusqu'à saturation des données. Une analyse thématique des verbatim puis une triangulation
ont été réalisées.
Seize entretiens ont été conduits entre mai et octobre 2013. Leur analyse a fait émerger 5
thèmes : le DIU est une contraception INENVISAGEABLE, ABSTRAITE, INVASIVE, ne
laissant pas de place à un CONTROLE PERSONNEL, malgré des AVANTAGES
RECONNUS.
Les représentations négatives sur le DIU restent prépondérantes en lien avec sa faible
utilisation. Il semble exister un schéma contraceptif figé au sein duquel il est difficile pour les
femmes nullipares de considérer chaque moyen contraceptif individuellement et sur un même
plan d’intérêt. Cette étude met en avant l'importance d'un conseil contraceptif systématique et
complet au cours des consultations de contraception en médecine générale incluant la
présentation du DIU et la diffusion de supports.

INTRODUCTION: Despite an increase in the contraception diffusion, the number of
termination of pregnancies is not declining, and additionally, are often a failure of
contraception. The Intra-uterine Device is a long acting, first line contraception, which can be
used by women whatever their parity is. However, only 1,3% of women aged 15-49 use it in
France. Reasons of this low use are complex and involve women's representations about this
method of contraception.
OBJECTIVE: To explore the nulliparous women's representations about the Intra-uterine
Contraceptive Device.
METHOD: One to one semi structured interviews were conducted with nulliparous women
who never used an IUD and never terminated a pregnancy. Data were subjected to a
qualitative study. The sampling was designed to search a maximal variation. The thematic
analysis was conducted by triangulation of the searchers.
RESULTS: 16 interviews were realized between May and October 2013. The following
analytical themes were identified: the IUD is an unthinkable, abstract, invasive contraception,
with a lack of personal control in spite of admitted advantages.
CONCLUSION: The pejorative representations of the IUD remain dominating for nulliparous
women, explaining the low utilization of this contraceptive method. It seems to exist a rigid
contraceptive pattern. Thus it is difficult for women to consider individually all contraceptive
methods on the same level of interest. This study highlight the importance of a systematic and
complete contraceptive counseling including a presentation of the IUD and the diffusion of
already existing supports during the contraception's clinic.
