4.5 Remarques conclusives
En premier lieu, l’étude présentée ci-dessus montre que les contenus narratifs de patients souffrant de douleurs musculo-squelettiques chroniques sont riches et méritent d’attirer l’attention des soignants. S’approcher de la narration permet non seulement une ouverture sur le vécu et l’expérience de l’atteinte, mais également sur la manière dont les patients rapportent leurs vécus. L’accueil de la plainte telle qu’elle est exprimée, «brute», permet d’aborder la singularité et la complexité de l’individu.
S’apparentant à certains principes de la psychanalyse et de la psychothérapie, le déploiement de l’histoire de chacun est ici valorisé au centre de la rencontre thérapeutique et cette prise en compte a la qualité de rendre possible un enrichissement du monde relationnel. «Il s’agit surtout de travailler avec et pour le patient à rétablir une cohérence…
Personne en effet ne devrait en être privé.» (Quartier, 2004).
L’analyse des résultats discutée ci-dessus met en évidence qu’il y a des vécus communs entre les patients des deux groupes, par exemple la chronicité ou l’insatisfaction face aux soins, mais également des expériences différentes et des modalités de dialogues différents (attributions traumatiques émotionnelles versus attributions traumatiques factuelles).
En effet, bien qu’il s’agisse d’une étude sur deux populations qui souffrent de problèmes musculo-squelettiques, les deux groupes ne sont pas strictement comparables. Ils se distinguent principalement par le genre: il y a une proportion plus élevée de femmes chez les fibromyalgiques – ce qui correspond d’ailleurs à la prévalence de la fibromyalgie dans la population de patients souffrant de cette atteinte. Cet écart limite bien évidemment la comparabilité des deux groupes. Cependant, l’objectif de cette étude n’était pas une comparaison stricte mais davantage une exploration des attributions auprès de patients souffrant de deux atteintes douloureuses chroniques. Si le but avait été de faire une étude comparative stricte, il aurait fallu recruter un plus grand nombre de femmes dans le groupe des patients lombalgiques chroniques ou alors plus d’hommes fibromyalgiques. Il serait par contre intéressant à l’avenir de mener une étude comparative stricte sur ce sujet et en faire l’objet d’une future recherche.
Par ailleurs, on constate le dégagement de sous-groupes comme celui des travailleurs de force pour qui parler d’usure ou d’accident au travail est plus acceptable socialement, familialement et culturellement que de parler des facteurs psychiques. Par contre, pour les femmes, quel que soit leur niveau éducationnel, exprimer leurs émotions semble plus aisé. La reconnaissance d’un symptôme diffus avec une composante psychologique, mais ancrée dans le corps, est chez elles une manière, semble-t-il acceptable, de dire leur détresse.
Ces deux manières distinctes de parler de soi font référence aux «prêts-à-porter culturels de la maladie», décrits par dans le domaine de l’ethnologie et de l’anthropologie sociale (Linton, 1936, Devereux, 1970, Kleinman, 1988, Helmann, 1995), c’est-à-dire aux symptômes socialement acceptables à un moment et dans un contexte donné pour expliquer un changement d’activité, de vie ou de mode relationnel (Herzlich, 1969, Boltanski, 1974).
Dans une perspective thérapeutique, la prise de conscience de la richesse des attributions causales représente un domaine important. Par un dialogue tenant compte des différentes visions du monde, un thérapeute et son patient sont à même d’élaborer une «co-construction»
d’un projet de soins. En effet, en légitimant les liens de causalité exprimés tout en les questionnant, le thérapeute induit des mouvements psychiques susceptibles d’amener un changement de version narrative. C’est par exemple, amener le patient à une ouverture sur une narration moins traumatique de son atteinte, voire vers l’élaboration de sa propre souffrance. Notre étude a montré non seulement la richesse de cette approche, mais également sa faisabilité en médecine somatique.
