XII.3 Information et formation

Ce sont deux aspects essentiels pour lutter contre les BMR à l’hôpital. Les informations produites par le laboratoire de microbiologie, la pharmacie et les services cliniques doivent être connectées et exploitées par la commission des antibiotiques. Cela permet d’optimiser la prise en charge des patients infectés, de surveiller l’incidence de la résistance et d’en analyser les éventuels facteurs favorisants et les conséquences. Les programmes de formation initiale et continue des professionnels de santé doivent :

- assurer une information sur l'épidémiologie locale et la politique antibiotique de l’hôpital à chaque professionnel de santé présent dans l'hôpital (particulièrement à son arrivée).

Au niveau extrahospitalier la population doit être sensibilisée sur le non-intérêt de l’utilisation abusive et/ou inappropriée des antibiotiques. De même les pharmaciens d’officine doivent être conscients de la nécessité de respecter de la réglementation sur la dispensation des antibiotiques [125].

La prévalence des bactéries multirésistantes au niveau des Hôpitaux au Maroc est « alarmante ».

Bien qu’il soit urgent de renouveler l’arsenal thérapeutique, nous devons être conscients que l’âge d’or de l’antibiothérapie est bel et bien terminé.

La situation est loin d’être satisfaisante. L’utilisation des antibiotiques en médecine humaine comme en médecine vétérinaire n’est pas encore irréprochable. Un certain nombre de prescriptions sont injustifiées ou ne correspondent pas aux recommandations, les tests de diagnostic rapide ne sont pas utilisés, les coopérations entre professionnels de santé restent anecdotiques. Or, cette mauvaise utilisation des antibiotiques est lourde de conséquences en termes de santé publique. La problématique essentielle reste de trouver les solutions à proposer pour lutter contre la diffusion de la résistance aux antibiotiques. La lutte contre ces bactéries résistantes peut se faire par la prévention qui consiste entre autre, à comprendre leurs modes de transmission, à trouver les déterminants de la résistance, et par la suite développer et mettre en place des outils de détection et de surveillance en temps réel.

Pour finir, je citerai Louis Pasteur : « Au lieu de chercher à tuer les microbes, mieux vaut ne pas les introduire dans notre organisme ». Encore une fois de plus comprenons par là que prévenir vaut mieux que guérir.

La résistance bactérienne aux antibiotiques est apparue rapidement après leur introduction dans le traitement des maladies infectieuses. L’émergence de cette résistance est un phénomène naturel, mais qui est accéléré par le mauvais usage des antibiotiques chez l’homme.

A partir des années 1950, de nombreux antibiotiques ont été découverts ou synthétisés et pour chaque nouvelle classe développée, nous avons assisté par la suite à l’émergence de nouveaux mécanismes de résistance, entraînant la diffusion de bactéries pathogènes de plus en plus difficiles à traiter.

La première partie de ce travail comprend des généralités sur les antibiotiques, les types de résistances bactérienne : la résistance naturelle et la résistance acquise, l’épidémiologie et les mécanismes par lesquels les bactéries se défendent contre l’action des antibiotiques.

Les facteurs qui favorisent l’émergence de cette résistance se résument à un mauvais usage de l’antibiotique, une prescription inappropriée des antibiotiques et à l’utilisation des antibiotiques dans le monde animal.

Dans une deuxième partie nous avons détaillé l’évolution de la résistance bactérienne aux antibiotiques des principales bactéries isolées en bactériologie humaine.

Ces bactéries additionnent les résistances à diverses familles d’antibiotiques et deviennent ainsi des multirésistants. Cette évolution conduit à des impasses thérapeutiques.

Enfin, dans la troisième partie, nous avons montré qu’une surveillance de la résistance bactérienne est indispensable car elle apporte non seulement une aide évidente au choix thérapeutique, mais aussi des informations précieuses pour l'épidémiologie et les stratégies de prévention.

Dans cette partie, nous avons précisé aussi les points majeurs du conseil dans le monde des antibiotiques qui permet de rationaliser l’usage des antibiotiques et de guider les médecins dans leur prescription, ainsi que le rôle majeur du pharmacien d’officine dans la lutte contre la résistance bactérienne aux antibiotiques.

The bacterial resistance towards antibiotics appeared rapidly after their introduction in the treatment of infectious diseases. The emergence of this resistance is a natural phenomenon, but it is accelerated due to the mis-use of antibiotics for humans and animals.

Starting from the 1950s, numerous ATB were discovered or synthesized, and for each new developed class, we assisted to the emergence of new resistance mechanisms, inducing the diffusion of pathogenic bacteria, more and more hard to treat.

The first part of this work includes generalities about ATB, along a definition, types of bacterial resistance: the natural resistance and the acquired resistance, the epidemiology and mechanisms through which the bacteria defends itself against the antibiotic’s action. The factors that promote the emergence of this resistance are sued up to an antibiotics mis-use, an inappropriate prescription and the use of ATB for animals (veterinary medicine or agriculture).

In the second part, we detailed the evolution of the bacterial resistance to antibiotics of the main isolated bacteria in human bacteriology.

These bacteria add up resistances to various antibiotics families and become multi-resistant. This evolution leads to therapeutical dead ends.

Finally, in the third part, we proved that a bacterial resistance’s surveillance is essential, because it brings, not only an obvious aid to the therapeutical choices (healing or prophylactic antibio-therapy), but also precious informations concerning the epidemiology and the preventive strategies.

In this part, we also precise the major points of the advising in the antibiotics’s world, that helps streamline the antibiotics’s use and guide the doctors in their prescription, as well
