WHO EASTERN MEDITERRANEAN REGION to the

EIGHTH MEETING OF THE REGIONAL TECHNICAL ADVISORY GROUP ON POLIOMYELITIS ERADICATION, EASTERN MEDITERRANEAN REGION

Cairo, Egypt, 21–23 October 2010

Mr Chairman, Dear Colleagues, Ladies and Gentlemen,

It gives me great pleasure to welcome you all to the eighth meeting of the Regional Technical Advisory Group for poliomyelitis eradication. I wish to extend a warm welcome to the new members and to thank outgoing and continuing members for the valuable guidance they have been extending to the polio eradication programme in the Region. I wish also to welcome representatives of the polio partner agencies, namely UNICEF, the Centers for Disease Control and Prevention (CDC), Atlanta, Rotary International, USAID and Bill and Melinda Gates Foundation, as well as colleagues from WHO headquarters, Regional Office for Africa and field staff from the countries.

Since your last meeting, several national, regional and global developments have taken place. I wish specifically to refer to the valuable evaluation of the constraints facing polio eradication in the endemic and re-infected countries in which some of you have been key players.

I wish also to refer to the development of the strategic plan 2010–2012 which has been of significant importance not only to our region but globally.

Dear colleagues,

Despite the many challenges we are facing to achieve polio eradication, I wish to begin with a positive note and indicate that the polio-free status of the 18 polio free countries has been

sustained and Sudan has re-joined them after recovering from the outbreak that happened as a result of importation. Polio-free countries are sustaining protective levels of immunity through improvement of routine immunization and implementation of supplementary immunization activities, especially in foci of low immunization coverage, and are maintaining surveillance up to certification standards.

I look forward to your views and advice with respect to the appropriateness of the ongoing strategies, to maintain the polio-free status of these countries and to curb the impact of any possible importation.

Dear Colleagues,

The other two countries of our region, namely Afghanistan and Pakistan, remain endemic for polio.

In Afghanistan, viral circulation continues to be largely confined to the security- compromised areas in the south. Continuous circulation in these areas is due mainly to serious access problems caused by active conflict and a persistent ‘climate of fear’. Together with the partners, the programme is continuously introducing innovative measures to attain safe access to the children without compromising staff safety. You will have a chance to hear about these initiatives and their impact during your meeting. The commitment of national authorities––H.E.

the President, the Minister of Public Health, governors and other levels––is very clear.

Similarly the commitment of the political leadership in Pakistan was repeatedly reaffirmed.

However, honourable members of the Regional TAG who have been with us in the three last meetings will recall their analysis of the reasons for deterioration in the epidemiological situation, which showed that this was partly attributable to inadequate commitment at the health delivery level, complicated by escalation of insecurity affecting accessibility to children in some areas for prolonged periods, together with low quality performance. Several efforts were made by national authorities supported by partners to address the situation. Comprehensive district-specific plans were developed in February this year for the 15 high-risk districts and implementation has been initiated. These plans have led to better results in the high priority districts except in the province of Khyber Pakhtoon and the Federally Administered Tribal Areas (FATA), where the security situation continues to pose serious limitation. The programme is attempting to mitigate this

through several innovative approaches. I wish to refer to one of them, namely convincing the Governor of Khyber Pakhtoon to constitute a Crisis Task Force for Polio Eradication for FATA and the military is fully on board to help the programme in security-compromised areas. These interventions have started to show an impact on the epidemiological situation.

Unfortunately, before the effect of the interventions became established, the programme faced another serious setback due to devastating floods resulting in serious damage to health infrastructure and massive population movement. It was also accompanied by understandable change in priority of the government machinery. This has had a negative impact on the polio eradication efforts for several weeks, coinciding with the season of highest transmission.

Emergency measures were taken to the extent possible under the prevailing circumstances but it is clear that the result is an epidemic which is beginning to be spread widely all over Pakistan.

We are looking for the TAG’s advice on the appropriateness of the current strategies in addressing the situation in Afghanistan and Pakistan, both on issues related to improving accessibility in security compromised areas and on the planned supplementary immunization activities to address the prevailing situation.

Dear colleagues,

Collaboration between neighbouring countries receives very special attention. A shining example is the collaboration between Afghanistan and Pakistan and between Horn of Africa countries. A recent declaration of cooperation between Afghanistan/Pakistan/Islamic Republic of Iran and Iraq and WHO (G5), in a number of disease control programmes, including polio, was issued after a recently held meeting in Tehran.

Without taking more of your time, once again, I wish to thank you for your continuous support to our efforts to achieve the long awaited goal of polio eradication and it remains for me to wish you a pleasant stay in Cairo.
