Vesicourethralanastomosicstenosis(VUAS)isarareoccur- rence, affecting less than 3% of radical prostatectomy patients[1,2].Severalfactorscanmakeitssurgicalmana- gementchallenging:theoperativesiteisdifficulttoaccess, the abundance of scar andfibrotic tissues makesthe dis- sectiontechnically demandingandcontinence isat stake.
TheevolutionofVUASmanagementhasparalleledtheone ofurethralstrictureandinternalurethrotomyhasemerged as the first-line option of choice. For the management of recurrences, the lack of surgeons performing urethral reconstructions in France probably led several teams to

favorrepeatedinternal urethrotomiescombinedwithure- thral self-dilation in an attempt to minimize the risk of further recurrences [3]. The study by Mutelica et al is the first to evaluate the results of the AMS-800® artifi- cial urinary sphincter (AUS) in patients whom VUAS was managedby internal urethrotomy combinedwithurethral self-dilation[4].Theauthorsreportafour-foldhigherrateof explantationforerosion-infectioninpatientswithurethral selfdilation[4].
There are many counter-arguments to the use of repeated internal urethrotomies combined with urethral self-dilationasa managementof recurrenturethral stric- tures.First,althoughsomestudieshavesuggestedurethral self-dilationmaybeofvalue,itsbenefitremainsuncertain, the level of evidence is low, and no self-dilation proto- colhasclearly established itssuperiority[3]. In addition, repeatmicrotraumaresultingfromself-dilationandmulti- pleurethrotomiesmaygeneratefibrosisandresultinlonger stricturemorechallengingtotreat[5].Mostimportantly,a

recentseriessuggestedthaturethralself-dilationisassoci- atedwithpoorqualityoflifeinanon-neurologicalpatient population [6]. Finally, severalalternatives to endoscopic treatment have been described in recent years [7—9].
Severalserieshavereportedtheoutcomesofcompleteanas- tomosisreconstructionthroughaperinealorrobot-assisted abdominalapproachwithpostoperativeincontinencerates thatcanexceed50%butwithexcellentpatencyrates[7,8].
Therobot-assistedYVplasty,whichallowstobringhealthy tissuevia an anteriorbladder wallflap andmaximizethe preservation of vascularization, seemsto provide promis- ingresultsandmaybecomethetechniqueofchoiceinthis patientpopulation[9].
As the Voltaire’s saying goes, French urologists often regard the bestas the enemy of the good, favoring self- dilation over reconstruction for male urethral strictures.
Pendingfurtherstudiestoconfirmthepresentfindings,this study should encourage urologists to avoid urethral self- dilationin AUS malepatients and mencandidates toAUS implantation.Inviewofthedatareportedherein,itseems reasonabletooffersurgicalreconstructiontomalepatients withrecurrentVUASafterasingleinternalurethrotomyand toperform thisreconstruction prior toAUS implantation.
Furtherdataareneededtorefinethetherapeuticstrategy in thecomplex patientpopulation withVUAS and urinary incontinence.

La sténose de l’anastomose urétro-vésicale (SAUV) après prostatectomie radicale est un problème rare, touchant moins de 3 % des patients ayant eu une prostatectomie radicale [1,2]. Sa prise en charge chirurgicale peut être complexepourplusieursraisons:lesiteopératoireestdiffi- ciled’accès,lestissussontcicatriciels,lacontinenceesten jeu.LapriseenchargedelaSAUVasuivil’évolutiondela prise encharge des sténoses de l’urètreet l’urétrotomie interne (UI) a pu s’imposer comme une solution de pre- mierrecours idéale.Danslecasdelaprise enchargedes récidives,lemanquedechirurgienréalisantdesreconstruc- tionsurétralesenFranceaprobablementpousséleséquipes àpréférerlamultiplicationdesUIenyassociantlesautodi- latationsurétralesafindetenterdeminimiserlerisquede récidive[3].L’étudedeMutelicaetalestlapremièreéval- uantlesrésultatsdusphincterurinaire artificielAMS-800® (SAU)chezdespatientsayanteuuneSAUVtraitéparurétro- tomie interneassocié à des autodilatations urétrales [4].
Lesauteursrapportentuntauxd’explantationpourérosion- infectionquatrefoissupérieurchezlespatientseffectuant desautodilatationsurétrales[4].
Il existe de nombreux contre-arguments à la répéti- tiond’urétrotomiesinternesassociéesàdesautodilatations urétrales pour les sténoses de l’urètre récidivantes. Pre- mièrement, bien que certaines études aient suggéré un intérêt de l’autodilatation dans la prise en charge d’une sténose urétrale, leur bénéfice reste incertain, le niveau preuve est faible et aucun protocole d’autodilatation n’a clairement établi sa supériorité [3]. De plus, les micro-traumatismes répétés créés par la dilatation ou l’uréthrotomieitérativecréeunespongiofibroseetaboutit àdes sténosesurétrales plus longues etpluscomplexes à
traiter [5].Parailleurs,etc’estpeut-êtrele pointle plus important,unesérierécentesuggèrequel’auto-dilatation urétraleseraitassociéeàunedétériorationdelaqualitéde viedansunepopulationdepatientsnon-neurologiques[6].
Enfin,plusieursalternativesautraitementendoscopiqueont étédécritesaucoursdesdernièresannées[7—9].Plusieurs sériesontrapportélesrésultatsdelaréfectioncomplètede l’anastomoseparvoiepérinéaleouabdominalerobot-assisté estassociéeàdestauxd’incontinencepostopératoirepou- vantexcéderles50%maisavecunfaibletauxderécidive delasténose[7,8].LaplastieYVparvoierobot-assistéequi permetd’apporterdutissusainviaunlambeaudeparoivési- caleantérieurepédiculéeetdeconserverunmaximumde vascularisation sembleapporter desrésultats prometteurs etpourraitêtrelatechniquedechoixdanscetteindication àl’avenir[9].
Suivant le proverbe de Voltaire, lesurologues franc¸ais regardent souvent le mieux comme l’ennemi du bien, favorisant les auto-dilatations à une reconstruction pour les sténoses de l’urètre masculin. En attendant d’autres études pour confirmer ces résultats, cette étude encour- ageàéviterlesauto-dilatationschezunpatientcandidatà l’implantationd’unSAUouporteurd’unSAU.Auxvuesdes donnéesrapportées par l’équipe deStrasbourg,il semble raisonnabledeproposerunereconstructiondel’anastomose urétrovésicale en cas d’échec d’une urétrotomie interne chezlespatientsayant uneSAUVetdefaire cetterecon- structionavantl’implantationd’unSAU.D’autresdonnées sonttoutefoisnécessairespourétayerlastratégiethérapeu- tiquechezlespatientsayantuneSAUVetuneincontinence urinaire.
