THIRD COORDINATION MEETING ON PREVENTION OF CROSS-BORDER TRANSMISSION OF WILD POLIOVIRUS BETWEEN ISLAMIC REPUBLIC OF

IRAN, IRAQ, SYRIAN ARAB REPUBLIC AND TURKEY Istanbul, Turkey, 1–2 February 1999

Dear Colleagues, Ladies and Gentlemen,

It gives me great pleasure to welcome you all to this coordination meeting on the prevention of cross-border transmission of wild poliovirus. I wish to extend my sincere thanks to the Government of Turkey for hosting the meeting and for the excellent arrangements made for the purpose. I am very pleased to note the participation of the national staff involved in poliomyelitis eradication and representatives of our partners in the global poliomyelitis eradication initiative, namely UNICEF, the Centers for Disease Control and Prevention in Atlanta, USA and Rotary International.

It is gratifying to note that the joint activities of the WHO Eastern Mediterranean and European Regions to prevent cross-border transmission of wild poliovirus have given new impetus to the eradication initiatives on both regions. As an example, coordinated mopping-up immunization, which started initially in border-areas, has been converted into a high-risk area approach. In the Syrian Arab Republic, seven governorates have been targeted for mopping-up immunization although only four of those governorates border neighbouring south-east Turkey and northern Iraq. Further provinces were covered in Iraq.

Another positive achievement is the improved quality of mopping-up, as shown by the achievement of above 90% coverage for the first time in the three northern governorates of Iraq.

I am pleased to inform the meeting that reinforcement of the high-risk area approach has been used in the eradication initiative in other situations in the Region, e.g. in the border and high-risk areas of Afghanistan, the Islamic Republic of Iran and Pakistan and also in the North African Member States of the Eastern Mediterranean Region. This high- risk approach greatly contributed to the regional achievements of 1998.

Dear Colleagues,

The number of poliomyelitis cases reported in the Eastern Mediterranean Region in 1998 significantly decreased from that reported in 1997, in spite of much improved AFP surveillance in many countries. I am pleased to note that the situation in the countries of the Eastern Mediterranean Region represented at this meeting has also improved.

Available data from the Islamic Republic of Iran suggest that transmission of wild poliovirus has been reduced to a few focal areas. The four wild poliovirus-associated cases were all from southern provinces close to the Afghanistan and Pakistan borders.

In Iraq, 23 cases were reported based on clinical classification, with only one wild poliovirus isolated in 1998. In the Syrian Arab Republic one wild poliovirus was isolated in a remote area and there was no evidence of other cases.

This is the third meeting held during the past 12 months to coordinate eradication efforts in the geographical areas bordering your countries. I am pleased to note that in the previous two meetings you made very practical recommendations and that these were successfully implemented. I am confident that the third meeting will also contribute to the poliomyelitis eradication efforts in your countries, as well as regionally and globally, and will bring us that much closer to polio-free status in the very near future.

In conclusion, I wish to take this opportunity to once again thank the Government of Turkey for hosting this important meeting and to thank every individual participant for the efforts made. I also wish to thank the international agencies, particularly Rotary International and the Centers for Disease Control and Prevention for their continued support and contribution to the national and regional eradication initiatives.

I wish you all success in your deliberations and a pleasant stay in Istanbul.
