by diffusing through the phospholipid bilayer of lipid membranes, while excluding most other molecules. The water channel made by aquaporins (AQP) is hourglass-shaped, narrow in the middle and wider at each end, and lined with charged chemical groups, which selectively allow the passage of water molecules (Figure 3).
So far, aquaporins have been mostly studied in the fetus in rela- tion to the homeostatic control of amniotic fluid volume.9 Since transplacental water flow to the fetus occurs through aquaporin water channels, functional disturbance of AQP may result in oligo or polyhydramnios. Aquaporins 1, 3, 8, 9, and 11 are expressed in the human fetal/ maternal membranes (amnion and chorion) and are dif- ferentially expressed throughout gestation.10 The importance of AQP has also been demonstrated in animal models. In larval zebra- fish (Danio rerio), data on water influx suggest a high turnover rate of water between the fish and the environment, which can be reduced by the AQP inhibitor phloretin and AQP1a1 knockdown. In the zebrafish larva AQP1a1 protein is expressed in cells on the yolk sac epithelium but apparently not in skin. Interestingly, in this animal model, knockdown of the epithelial TJ protein claudin-b significantly increased water influx.11
Molecular mechanisms regulating water flux in neonatal skin are not well understood. During the period of abrupt transition from an aqueous to a gaseous milieu at birth, the potential involvement of AQP in addition or in cooperation with TJ proteins in the regulation of transcellular and paracellular water movement deserves to be con- sidered. Very strikingly, a marked contraction of extracellular water occurs after birth, with a weight loss of between 7% and 15% of body weight by the end of the first week, and is more marked in
premature babies. As noted in cited pioneering works, neonatal water loss occurs in majority transepidermally. Transepidermal water loss (TEWL) is a good marker of gestational age at birth.13 In general, TEWL is a good alternat ive to more invasive procedures, and can now be measured with small hand devices. It assesses the rate at which water vapor passes through the skin layers into the environ- ment. TEWL is accepted as a robust indicator of epidermal barrier function. Normal TEWL is 4 to 8 g/ m2/ h – with lower values gener- ally indicating an effective SC barrier and high values meaning the barrier is damaged or genetically deficient . As gestational and post- natal age progresses, TEWL decreases (Figure 4).
In neonatal skin, an increased epidermal expression of AQP3 has been clearly shown in the context of skin inflammation (erythema toxicum), though a constitutive expression was also noted in neo- nates without skin inflammation. The expression of AQP3 was noted for the whole epidermis.14 AQP3 is an aqua-glyceroporin, known to transport glycerol as well as water. AQP3 is abundant and functional in keratinocyte plasma membranes. AQP3 expression in the epider- mis stops one layer of cells below the SC, consistent with water dis- tribution in this tissue, and suggesting a strong link with the TJ liquid barrier.15 Studied in the context of a dominantly inherited pal- moplant ar keratoderma showing typical features of water retention (aquagenic ketatoderma) AQP5 is present throughout the palmar epi- dermis and shows strong localization to the plasma membrane of the keratinocytes of the stratum granulosum, suggesting specific func- tions of aquaporins in water trafficking according to body sites.16

Vernix caseosa consists of cells coated with lipids. It is biochemically a complex mixture of 80% water, 10% protein, and 10% lipids including barrier lipids such as ceramides, free fatty acids, phospho- lipids, and cholesterol. It develops from the hair follicle during the last trimester of pregnancy and is produced head-to-t oe and back- to-front.
